Is it safe to SCUBA dive with asthma?

Introduction: Internationally it is estimated that six million people participate in self-contained underwater breathing apparatus (SCUBA) diving each year. Registries suggest a significant proportion of divers have a current or historical diagnosis of asthma. Previously individuals with asthma were prohibited from diving, however, several contemporary guidelines suggest a select population of patients with asthma may be able to dive with an acceptable degree of risk. Areas covered: Divers with asthma may be at an increased risk of a variety of diving-related medical injuries including; pulmon

Further studies on the mechanism of human histamine-induced asthma: the effect of an aerosolized H1 receptor antagonist (diphenhydramine).

This study was designed to better define the mechanism of histamine-induced bronchoconstriction in humans by pharmacologic manipulation of the postulated bronchial histamine receptor sites. Histamine challenges were performed on a heterogeneous group of adult asthmatic subjects. The cumulative units of histamine required for induction of a sustained 20% or greater decrease in FEV1 from baseline were determined. The effect of pretreatment with an aerosolized H1 receptor antagonist, diphenhydramine hydrochloride, was then studied. Analysis of the data showed that the administration of an H1 rece

[Proceedings: Indications and limitations of breathing gymnastics].

Chronic obstructive lung disease represents the most important field of application for respiratory physiotherapy. The technic comprises two successive stages: inhalation of a bronchospasmolytic and secretolytic aerosol (e.g. Alupent or Ventolin, diluted in tacholiquine or Bisolvon) followed by breathing exercises. These exercises are divided into three parts: respiratory movements such as diaphragmatic or abdominal breathing, drainage of bronchial secretions, and exercises to relax the muscles of the shoulders, the thorax and the skin, all these movements being executed in the rhythm of breat

Inhaled volatile anesthetic gas for severe bronchospasm in the emergency department.

Bronchospasm is caused by reversible constriction of the smooth muscles of the bronchial tree. This causes obstruction of the lower airways, which is commonly seen at the emergency department (ED) in patients with acute exacerbation of asthma or chronic obstructive pulmonary disease. Ventilation may be difficult in mechanically intubated patients with severe bronchospasm due to airflow limitation, air trapping, and high airway resistance. The beneficial effects of volatile inhaled anesthetic gas had been reported due to its bronchodilation properties. In this case series, we would like to shar

Anesthetic considerations in children with asthma.

Due to the high prevalence of asthma and general airway reactivity, anesthesiologists frequently encounter children with asthma or asthma-like symptoms. This review focuses on the epidemiology, the underlying pathophysiology, and perioperative management of children with airway reactivity, including controlled and uncontrolled asthma. It spans from preoperative optimization to optimized intraoperative management, airway management, and ventilation strategies. There are three leading causes for bronchospasm (1) mechanical (eg, airway manipulation), (2) non-immunological anaphylaxis (anaphylacto

Perioperative complications of anesthesia.

An overview of several perioperative complications and their management strategies is presented. Operative hypothermia, malignant hyperthermia, bronchospasm, and side effects of spinal opioid agents are discussed. Ramifications of these complications may extend well beyond the operative period and influence patient outcome. Therefore, it is necessary that the surgeon have a fundamental understanding of the pathophysiology and modalities of treatment in the context of anesthesia and surgery.

[Cholinergic systems in bronchospasm].

A neuronal cholinergic system is widely distributed in larger airways, particularly around bifurcations. These neurons form efferent pathways from the brainstem ending in the motor end-plate on smooth muscle, and ciliated and mucus-producing cells. Impulses originating in various sensory endings in the airways, from the central nervous system and from many sensory receptors throughout the body, influence the activity of the cholinergic system thereby controlling the patency of the airways and modulating the rate and depth of breathing. A non-neuronal cholinergic system present on mast cells fa

Further investigation of the association between gastroesophageal reflux and bronchoconstriction.

A double-blind modification of the intraesophageal acid perfusion challenge (Bernstein procedure) was performed in asthmatic subjects with and without gastroesophageal reflux, nonasthmatic subjects with reflux, and normal subjects. Conventional spirometric functions and total respiratory resistance (Rrs) were measured prior to and after the infusion. There were no changes in pulmonary functions except in the asthmatic subjects who had had a positive acid challenge. The greatest changes occurred in Rrs, which increased significantly with reflux symptoms (p less than 0.01) and decreased toward b

Not asthma, but GERD: case report.

Asthma is a disorder of the lungs characterized by increased responsiveness of the airways, as manifested by episodes of wheezing and increased resistance to expiratory airflow because of varying degrees of smooth muscle contraction, edema of the mucosa, and mucus in the lumen of the bronchi and bronchioles. The stimuli vary widely and include antigens, infection, air pollutants, respiratory tract irrtants, exercise, and emotional factors. This condition is completely different from distress breathing because of laryngotracheal spasm. One of its causes is the gastric content reflux through the

[Management of status asthmaticus in children].

Acute bronchial asthmatic episodes represent one of the most common respiratory emergencies in medical practice. Its maximum expression "status asthmaticus", is an entity of low incidence in our medium, still it is a risk to the physical integrity of the patient. The management of an acute attack of asthma can be focused on doing away with the bronchial spasm and decreasing the inflammatory effects, as well as improve the movement of secretion and this way, maintain adequate alveolar ventilation. The full knowledge of the physiopathology behind the illness and medical treatment used to improve…

Bronchoconstrictor reactivity to NKA in allergic dogs: a comparison to histamine and methacholine.

Airway hyperresponsiveness to neurokinin A (NKA) occurs in inflammatory airway diseases like asthma. In this study, bronchoconstrictor reactivity to NKA was measured in beagle dogs neonatally sensitized to and challenged with ragweed. Comparisons were made to histamine and methacholine. Lung resistance (R(L)) and dynamic lung compliance (C(Dyn)) were measured in anesthetized, spontaneously breathing dogs before and after aerosol challenge with NKA, histamine or methacholine. The concentration of these agents increasing R(L)by 25% above baseline (PC(25)) was calculated before and 24 h after aer
