Bronchiolitis obliterans in children.

PURPOSE OF REVIEW: In this review, we discuss recent advances in our understanding of the etiology, pathology and pathogenesis, clinical presentation, diagnosis, treatment, and outcome of bronchiolitis obliterans in the nontransplant, pediatric population. RECENT FINDINGS: The diagnosis of bronchiolitis obliterans in children can be made with confidence based on clinical presentation, particularly with a history of adenovirus bronchiolitis or pneumonia, fixed obstructive lung disease on pulmonary function testing, and characteristic changes of mosaic perfusion, vascular attenuation, and centr

Antibiotics for bronchiolitis in children.

BACKGROUND: Bronchiolitis is a serious, potentially life-threatening respiratory illness commonly affecting young babies. It is most often caused by Respiratory Syncytial Virus (RSV). The diagnosis is usually made on clinical grounds (especially tachypnoea and wheezing in a child less than two years of age). Antibiotics are not recommended for bronchiolitis unless there is concern about complications such as secondary bacterial pneumonia. Despite this, they are used at rates of 34 to 99% in uncomplicated cases. OBJECTIVES: To evaluate the use of antibiotics for bronchiolitis. SEARCH STRATEGY

Bronchiolitis: the pathologist's perspective.

The term "bronchiolitis" refers to a broad morphologic spectrum of inflammatory events that are centered on small conducting airways. Bronchiolitis may be an isolated pathologic finding, although it is often a secondary consequence of diseases affecting other parts of the conducting apparatus or pulmonary acinus. Divergent causes of bronchiolitis may have similar microscopic findings. Hence, a pathologic diagnosis is nonspecific and not clinically meaningful unless placed in the context of relevant clinical and radiographic findings. Etiologically, most cases of bronchiolitis are infectious in

Diagnosis and management of bronchiolitis.

Bronchiolitis is a disorder most commonly caused in infants by viral lower respiratory tract infection. It is the most common lower respiratory infection in this age group. It is characterized by acute inflammation, edema, and necrosis of epithelial cells lining small airways, increased mucus production, and bronchospasm. The American Academy of Pediatrics convened a committee composed of primary care physicians and specialists in the fields of pulmonology, infectious disease, emergency medicine, epidemiology, and medical informatics. The committee partnered with the Agency for Healthcare Rese

Bronchiolitis obliterans: an update.

PURPOSE OF REVIEW: Bronchiolitis obliterans (BO) occurs in both post-lung transplant and nontransplant-related individuals, and is characterized by mainly irreversible airflow obstruction that is often ultimately progressive. RECENT FINDINGS: While post-lung transplant BO is a major cause of lung allograft dysfunction, and hence is better characterized than nontransplant-related BO, it is likely that many similarities in pathogenesis and treatment apply to both categories. SUMMARY: Optimal management for BO remains to be established, and the role of retransplantation in this disease requires

Bronchiolitis: update 2001.

In this review, reports from last year on the following topics are summarized: (1) reviews of bronchiolitis in infants; respiratory syncytial virus (RSV)-associated illness, including possible viral mechanisms of alteration of airway function and results of an epidemiologic study of bronchiolitis-associated mortality. Studies evaluating (2) the use of serum eosinophilic cationic protein as a marker for development of subsequent persistent wheezing infants; (3) parental bronchial responsiveness as an indicator of genetic susceptibility to acute bronchiolitis; (4) prophylactic use of monoclonal

Bronchiolitis in Kartagener's syndrome.

The association of diffuse bronchiolitis in patients with Kartagener's syndrome (KS) has not been reported previously. The aim of this study was to present the morphological characteristics of bronchiolitis in patients with KS. Eight patients (four males, four females; mean age 37.9+/-18.7 yrs), clinically diagnosed as KS with the classical triad of chronic pansinusitis, bronchiectasis and situs in versus with dextrocardia, were evaluated. Routine chest radiography showed bronchiectasis and dextrocardia in all patients. Chest computed tomography (CT) showed diffuse centrilobular small nodules

Severe bronchiolitis in children.

Acute viral bronchiolitis is one of the most common causes of paediatric respiratory disease, especially in the first year of life. Young infants and those with pre-existent cardiorespiratory problems are at an increased risk of acute respiratory failure necessitating mechanical ventilation. A prospective study was carried out from November 1988 to October 1989, to analyse the clinical features, aetiology and outcome of patients with severe bronchiolitis. 52 (29.9%) out of 174 cases of acute bronchiolitis admitted were found to be severe. A virus aetiology was identified in 75% of the cases, r

Bronchiolitis in rheumatoid arthritis.

Bronchiolitis in association with rheumatoid arthritis has been reported, to our knowledge, in 18 patients to date. In some cases use of penicillamine has been strongly associated with the development of bronchiolitis. Most of the reported cases are described as having marked irreversible airways obstruction and hyperinflation. We describe a patient with rheumatoid arthritis whose respiratory tract symptoms began during gold therapy. Physiologic studies showed marked lung hyperinflation without pathologic findings in forced dry spirometric study. On open lung biopsy a mild degree of granulomat

Pathological and clinical relevance of selective recruitment of Langerhans cells in the respiratory bronchioles of smokers.

BACKGROUND: Smoking causes an influx of inflammatory cells including Langerhans cells (LCs) into the airways and lung parenchyma, thus inducing histological changes, such as emphysema and fibrosis. We examined the distribution and quantity of Langerhans cells in relation to clinical and pathological findings and explored the association between smoking and accumulation of Langerhans cells in the respiratory bronchioles. METHODS: Fifty-three patients who underwent lung resection for primary diseases, including lung cancer, were recruited. Histological and immunohistochemistry analyses were uti…

Lung ultrasound in bronchiolitis.

BACKGROUND: Bronchiolitis is the most common acute viral infection of the lower respiratory tract in infants. Clinical severity is associated with different risk factors; however, no clinical, laboratory, or radiological findings are able to predict the course of the disease in full-term infants. Lung ultrasound (LUS) is a valid technique for the diagnosis and evaluation of pediatric respiratory diseases. AIMS: The aim of our study was to correlate an LUS score with a clinical score, to describe lung ultrasound findings in cases and controls, and to compare LUS findings with chest X-ray (CXR)

Bronchiolitis and Bronchiolar Disorders.

Bronchioles are noncartilaginous small airways with internal diameter of 2 mm or less, located from approximately the eighth generation of purely air conducting airways (membranous bronchioles) down to the terminal bronchioles (the smallest airways without alveoli) and respiratory bronchioles (which communicate directly with alveolar ducts and are in the range of 0.5 mm or less in diameter). Bronchiolar injury, inflammation, and fibrosis may occur in myriad disorders including connective tissue diseases, inflammatory bowel diseases, lung transplant allograft rejection, graft versus host diseas…

Follicular bronchiolitis in a child.

BACKGROUND: Follicular bronchiolitis (FB) is an uncommon but important pulmonary manifestation in children. METHODS: In this report, we pre … l examination revealed that lymphoid follicles with reactive germinal centers distributed along the bronchioles. The boy responded favorably to corticosteroid therapy and recovered well. CONCLUSIONS: Diagnosis of FB should be considered when a child presents with chronic bronchial obstruction. Open lung biopsy is necessary for confirmation of the diagnosis.

Current therapies in bronchiolitis.

Viral bronchiolitis is the most common cause of hospitalization among infants. Despite its prevalence, no consistently effective therapy has been found to date, providing the driving force behind much of the ongoing research into this illness. In this review, we present a summary of the most recent published trials of interventions for bronchiolitis. Included are studies evaluating bronchodilators, corticosteroids, positive pressure ventilation, as well as 3 newer therapies for bronchiolitis: heliox, mucolytics, and leukotriene receptor antagonists.

[Bronchiolitis and pulmonary hypertension].

OBJECTIVES: The study was undertaken to assess whether previously healthy infants with bronchiolitis develop elevated pulmonary artery pressures. MATERIAL AND METHODS: We performed 2D and Doppler echocardiography in 94 infants, aged between 2 and 12 months, from November 2007 to December 2009. 47 infants had bronchiolitis and 47 were in the control group. We excluded children with congenital heart diseases and other conditions associated with pulmonary hypertension. The method of time to peak velocity corrected for heart rate was used to estimate pulmonary arterial pressure (PAP). A Student's
