The genetics of bulimia nervosa.

Although historically bulimia nervosa has been considered to be a disorder influenced primarily by sociocultural forces such as societal emphasis on thinness and attractiveness, over the past decade it has become increasingly clear that biology and genetics contribute substantially to its etiology. Currently, it is believed that combined study of genetic and environmental risk factors may ultimately hold the clue to the question of why when so many young individuals are exposed to societal risk factors, only a small number actually develop frank eating disorders. Genetic research on eating dis…

Bulimia in children and adolescents.

Although there have been many recent advances in research, much work remains to be done in the area of child and adolescent eating disorders. More data are needed regarding the normal development of eating behavior, resilience and risk factors for eating pathology, and treatment studies in children and adolescents. The best studied areas include epidemiology, short-term treatment for bulimia nervosa (BN), and outcome in anorexia nervosa. A case report of the single blind use of repetitive transcranial magnetic stimulation in a patient with BN has been reported, but its safe use in children and…

Smoking and bulimia nervosa.

OBJECTIVE: Little is known about the long-term physical consequences of bulimia nervosa. Some physical morbidity may result from high rates of cigarette smoking. The aim of the present study was to test three hypotheses regarding smoking among women with bulimia nervosa. METHOD: The subjects were 102 women with DSM-IV bulimia nervosa, 204 matched normal controls, and 102 matched controls with affective or anxiety disorders. All three groups were recruited from the same community sample. Interview measures were used for diagnosis and for information on smoking. RESULTS: A higher proportion of…

Pharmacotherapy of Bulimia Nervosa.

This review article summarizes the progress that has been made over the past 14 years in the drug treatment of bulimia nervosa, an eating disorder characterized by recurrent binge eating and purging that has continued for 3 months with a regularity of at least twice a week. This recently described eating disorder has been the subject of intensive research scrutiny with regard to both pharmacotherapy and psychotherapy, and results indicate a role for both types of treatment. The linkage of bulimia to major depressive illness is reflected in the overlap that is seen in neurotransmitter dysregula…

Outcome in bulimia nervosa.

OBJECTIVE: The authors sought to synthesize existing data on outcome for individuals diagnosed with bulimia nervosa in order to better understand long-term outcome and prognostic factors. METHOD: They reviewed 88 studies that conducted follow-up assessments with bulimic subjects at least 6 months after presentation. Findings are summarized for the areas of mortality, recovery, relapse, crossover, and prognostic variables. RESULTS: The crude mortality rate due to all causes of death for subjects with bulimia nervosa in these studies was 0.3% (seven deaths among 2,194 subjects); however, ascer…

Anorexia and bulimia nervosa.

Anorexia and bulimia nervosa are eating disorders characterised by abnormal eating behaviour, a morbid concern over body weight and a distorted perception of body size. It commonly occurs in young postpubertal girls in developed societies. Aetiological factors are a complex interplay of biological, psychological and social factors. Bizarre eating patterns can lead to life-threatening medical situations which require urgent in-hospital care. Comprehensive management should be effected by a multi-disciplinary team with contributions from the internist, the psychiatrist, the nurse, the psychologi…

Hypercholesterolemia in bulimia nervosa.

Patients with bulimia nervosa may present with serum cholesterol levels higher than the recommended range. Giving advice on dietary modification to reduce cholesterol levels runs contrary to the cognitive behavioral strategies used to treat bulimia nervosa. This would not be a problem if it were found that cholesterol levels fell with clinical improvement in the eating disorder.

Serotonin and bulimia nervosa.

Serotonin (5HT) is one of several neuromodulators of feeding. Experimentally reducing 5HT activity in animals increases food intake, while increasing 5HT activity has the opposite effect. Studies suggest that women with bulimia nervosa show signs of reduced 5HT activity, which may be related to binge eating. Data supporting the theory that reduced central nervous system 5HT activity may play a role in the pathophysiology of bulimia nervosa is reviewed. Disturbances of 5HT activity and the relationship to other psychopathology in bulimia nervosa, such as depression, substance abuse, and impulsi…

Bulimia nervosa in adolescence.

Investigations of bulimia nervosa have focused primarily on adult samples, although bulimia nervosa commonly has its onset in adolescence. Pediatricians are often questioned about its etiology, course, and treatment. In an attempt to provide pediatricians with answers, we integrate findings from recent epidemiological and treatment studies with a clinical report of 18 women who developed bulimia nervosa during their teens and sought treatment at our eating disorders clinic.

Hyperamylasemia in bulimia nervosa.

To determine the value of total serum amylase levels and salivary and pancreatic isoenzyme levels as biologic indices of behavioral disturbance in bulimia nervosa, the authors monitored these levels in 40 bulimic patients participating in a placebo-controlled trial of desipramine and in 25 controls. In the patients, the total and salivary amylase levels were significantly elevated and a significant correlation existed between the frequencies of binge eating and vomiting and the level of salivary amylase. However, the ability to discriminate patients from controls on the basis of serum amylase …

Dysphagia in bulimia nervosa.

The findings on 13 patients with bulimia nervosa referred for evaluation of salivary glands and swallowing patterns are presented. Each patient completed a medical, oral, and social history questionnaire. A complete oral examination supported by appropriate dental radiographs and photographs was conducted. Unstimulated and stimulated parotid and submandibular saliva was collected. The presence or absence of pharyngeal and velar gag reflexes was ascertained. Real-time ultrasound scanning and barium swallow studies were used to evaluate the oral-motor functions while swallowing on 6 of the subje…

Bulimia: the emergence of a syndrome.

The literature on bulimia, which has regarded it as a psychiatric entity or as a form of disordered eating, suggests that this behaviour is now widespread among the obese, among those with anorexia nervosa and those of normal weight. While physiological, psychological and sociocultural explanations and forms of treatment have been proposed, a holistic perspective on the context in which it occurs can distinguish bulimia nervosa and bulimarexia from 'simple' bulimia.

Bulimia symptoms and other risk behaviors during pregnancy in women with bulimia nervosa.

OBJECTIVE: The current study examined the change in bulimic symptoms as well as alcohol, drug, and tobacco use during pregnancy in subjects with bulimia nervosa. METHOD: A self-report questionnaire was used to collect retrospective data on eating disorder symptoms and substance use during pregnancy from 129 participants in a long-term study of bulimia nervosa. Follow-up data were collected for a total of 322 pregnancies occurring over a 10-15-year period. RESULTS: Overall, subjects reported that body dissatisfaction worsened, but binge eating and purging improved during pregnancy. However, t…
