Staphylococcal septicaemia in burns.

This study analyses staphylococcal septicaemia in a series of 1516 burn patients who were admitted to the burn unit of the Al-Babtain Centre for Burns and Plastic Surgery, Ibn Sina Hospital, Kuwait over a period of 6.5 years (1 June 1992-31 December 1998). One hundred and nine patients (7.2%) developed clinically and microbiologically proven septicaemia, of which 80 (73.4%) showed one or the other type of Staphylococcus in their blood. Fifty (62.5%) of them were males and 30 (37.5%) females, with a mean age of 26 years and the mean total body surface area of burns (TBSA) of 45% (range 1-93%). …

Inhalation burns in children.

Survival from serious burns in children has improved substantially in recent years. Mortality is predominantly determined by the total body surface area burned and the often unrecognised inhalation injury. A retrospective review of 4,451 consecutive children with thermal injuries over a 10-year period was undertaken to determine the incidence, clinical presentation, and pathology of inhalation injury and its contribution to morbidity and mortality. Inhalation burns were diagnosed clinically and confirmed endoscopically and post-mortem in 97 (2.2%) children; 77 sustained fire burns (mean age 4 …

Ocular hydrofluoric acid burns.

A 30-year-old man sustained exposure of his eyes, face, and neck involving 4% of body surface area to hydrofluoric acid. He was treated with immediate lavage and topical calcium gluconate. Because free fluoride ions from ocular and facial exposures can form complexes with body stores of calcium and magnesium, the patient was transferred to a burn unit for cardiac and electrolyte monitoring. He was also treated with calcium gluconate skin injections, pulmonary nebulizer therapy, and topical antibiotics and corticosteroids. In another case, a 25-year-old man with less severe exposure to hydroflu

[Initial treatment of burns].

Burns, scalds and caustic burns all bring about an increase in microvascular permeability, with volume losses into the interstitium and at the surface of the body. The first aid measures to be taken include removal of the source of heat, maintenance of the vital functions, institution of infusion therapy with Ringer's lactate, and sterile covering of burn wounds, followed by rapid transportation of the victim to the nearest hospital. The crucial immediate step to be taken at the hospital, when large-area burn wounds are present, is rapid parenteral administration of fluid. Local management of …

Late problems in burns.

In summary, the commonest late physical complications of the burn injury have been reviewed and an approach to their management described. An understanding of the pathophysiology of the conditions is invaluable in planning one's approach to these problems. The techniques to control and correct chronic wound and hypertrophic scar and contractural deformities have been described. We present only a brief summary of the techniques found most reliable in the management of a large group of burned children over many years.
