BACKGROUND: Disorders of the coagulation pathway are triggered in patients with severe burn and inhalation injuries in the early stages. There are multiple early coagulation indices identified to correlate with adverse outcomes.
METHOD: A retrospective analysis of patients with severe burn and inhalation injuries from 12 centers in mainland China was performed to identify early changed coagulation indices with predictive value associated with four major 28-day adverse outcomes (death, anticoagulation, mechanical ventilation, continuous renal replacement therapy) by logistic regression. The op

Tobacco combustion exposure worsens rheumatoid arthritis (RA). Non-combustible tobacco devices, as heat-not-burn tobacco (HNBT), are emerging as harm reduction to smokers by releasing nicotine and lower combustible tobacco products. Nevertheless, HNBT toxicity remains unclear. Hence, here we investigated the impacts of the tobacco combustible product (cigarette smoke; CS) or HNBT vapor exposures on antigen-induced arthritis (AIA) in C57BL/6 mice. Animals were exposed to airflow, HNBT vapor, or CS during 1 h/twice a day, under the Health Canada Intense (HCI) smoking regime, between days 14 to 2

Only a handful of published reports exist today that describe neurological complications following smoke inhalation injury. In this study, we characterize acute pathophysiological changes in the brain of sheep exposed to smoke inhalation, with- and without third-degree skin burn that models the injuries sustained by human victims of fire accidents. Blood-brain barrier integrity and hemorrhage were analyzed throughout the brain using specific histological stains: Hematoxylin & Eosin, Luxol fast blue, Periodic acid-Schiff (PAS), and Martius, Scarlet and Blue (MSB). Our data show that, following

Smoke inhalation in burn patients is a serious medical problem around the world. Inhalation injury increases mortality in addition to increasing infections, ventilator-days, and hospital stays. There are also large numbers of patients subjected to smoke inhalation without burns from cooking fires, burning crops and forest fires. The injury results in a fall in arterial oxygenation as a result of airway blockade, increased pulmonary transvascular fluid flux and loss of hypoxic pulmonary vasoconstriction. The changes in cardiopulmonary function are mediated at least in part by reactive oxygen an

We report a case of combined severe subglottic and lower tracheobronchial stenoses after an inhalation burn injury. The patient was successfully treated by the insertion of a bifurcated silicone rubber stent. The proximal limb of the silicone tube was used to stent the subglottic stenosis after it had been resected, a treatment option not previously reported. It is speculated that the diffuse airway damage resulted from a combination of thermal injury, mechanical trauma and infection. The relatively late symptomatic presentation of the lower tracheal stricture is stressed and the treatment opt

BACKGROUND: One-third of major burns are complicated by burns-related smoke inhalation injury (BII), which increases mortality risk. Fibreoptic bronchoscopy is the gold standard for BII diagnosis, with the Abbreviated Injury Score (AIS) being the most widely used grading system. While BII severity has been linked to poorer outcomes, this is the first systematic review and meta-analysis evaluating the association between bronchoscopic grading of BII and mortality, pneumonia, and acute respiratory distress syndrome (ARDS).
METHODS: Primary human clinical studies assessing BII severity via bronc

BACKGROUND: Burn inhalation injury (BII) is a major cause of burn-related mortality and morbidity. Despite published practice guidelines, no consensus exists for the best strategies regarding diagnosis and management of BII. A modified DELPHI study using the RAND/UCLA (University of California, Los Angeles) Appropriateness Method (RAM) systematically analysed the opinions of an expert panel. Expert opinion was combined with available evidence to determine what constitutes appropriate and inappropriate judgement in the diagnosis and management of BII.
METHODS: A 15-person multidisciplinary pan

OBJECTIVE: Inhalational burns frequently lead to dysphonia and airway stenosis. We hypothesize local dexamethasone delivery via a novel drug-eluting electrospun polymer-mesh endotracheal tube (ETT) reduces biomechanical and histologic changes in the vocal folds in inhalational burn.
METHODS: Dexamethasone-loaded polymer mesh was electrospun onto ETTs trimmed to transglottic endolaryngeal segments and secured in nine Yorkshire Crossbreed swine with directed 150°C inhalation burns. Uncoated ETTs were implanted in nine additional swine with identical burns. ETT segments were maintained for 3 and

A retrospective study was conducted on 314 burn patients with inhalation injury admitted to the National Burn Hospital during the period 2015-2019. The results showed that adult and male was predominant (81.9% and 77.7%), with burns mostly caused by flame (93.6%) and burn extent of 68.6±24.3% and deep burn area of 44.6±25.2% total body surface area. Rate of required mechanical ventilation patients was 95.22%. Common complications were multiple organ failure (41.9%), pneumonia (29.9%), ARDS (25.5%) and septic shock (23.8%), with a mortality rate of 85.4%. Multivariate logistic analysis indicate

Inhalation injury is predictive of dysphagia post burns; however, the nature of dysphagia associated with inhalation burns is not well understood. This study describes the clinical profile and recovery pattern of swallowing following inhalation burn injury. All patients admitted 2008-2017 with confirmed inhalation burns on laryngoscopy and managed by speech-language pathology (SLP) were included. Initial dysphagia presentation and dysphagia recovery pattern were documented using the FOIS. Co-presence of dysphonia was determined clinically and rated present/absent. Persistent laryngeal/pharynge

Although persistent hoarseness has been recognized in patients who have sustained burn and/or smoke inhalation injuries, there is little documentation to support this observation. Furthermore, there is no quantification of either the pervasiveness of the problem or the severity of the dysphonia resulting. It was the intent of this study to examine the laryngeal condition and voice production of a group of patients who were long-term survivors of burns and inhalation injuries. Only 10 patients (8 male and 2 female) of a larger cohort were willing to return for this examination. They were ambula

Forty-one severely burned patients were divided into inhalation injury and non-inhalation injury groups. It was found that in the inhalation injury group, TXB2 level and TXB2/6-keto-PGF1 alpha ratio in both plasma and lung tissues were significantly elevated, circulatory platelet aggregate ratio markedly decreased, and blood viscosity greatly increased. Histopathologically, congestion, oedema, haemorrhage and thrombosis were observed in lung tissues taken from patients succumbing to respiratory failure. The changes in TXB2 and TXB2/6 keto-PGF1 alpha ratio accorded with the clinical course of t

UNLABELLED: The role of extravascular lung water (EVLW) in the pathogenesis of inhalation injury (INH) when associated with concomitant major burn (B) remains controversial. Previous experimental models have investigated isolated INH without surface burn. This study measured the effects of isolated and combined INH on EVLW and pO2 in a porcine experimental model. The beneficial effects of early resuscitation with dextran-40 (DEX) were assessed, using a control group receiving standard Parkland formula (LR). In the first part of the study (INH vs. INH + B), a group of animals with a standardize

BACKGROUND: Burn injuries complicated by smoke inhalation significantly increase clinical severity and can lead to acute respiratory distress syndrome (ARDS). Inhalation injury is predominantly caused by exposure to toxic combustion gases, with thermal damage from steam being relatively uncommon. Thrombocytopenia is frequently observed in severe burns and is associated with poor outcomes, whereas platelet transfusion can carry clinically important risks. Recombinant human thrombopoietin (rhTPO), a hematopoietic growth factor used clinically for thrombocytopenia, may represent an alternative st

BACKGROUND: The morbidity and death rates from burn injuries in children are considerably raised by lung issues resulting from inhalation injuries.
PURPOSE: The purpose of this study was to ascertain how Wii aerobic exercise affected children who had inhaled injuries following thermal burns.
METHODS: Children with inhalation injuries (N = 76) were split into two equal groups for this randomized controlled experiment. One group received conventional chest medical treatment along with Wii aerobic exercise. The control group, on the other hand, merely received routine chest medical treatment. O

Thermal injury refers to trauma to tissues as a result of exposure to a source of sufficiently high or low temperature. This mechanism of trauma may arise from direct contact with a heat source, e.g., scalding of skin secondary to immersion in hot water or following exposure to a stimulus below freezing temperature. Gases stored under pressure in a liquid state undergo adiabatic expansion upon decompression, which results in a rapid and significant decrease in temperature. Nitrous oxide is an example of a gas typically stored as a liquid in canisters prior to use. Tissue exposed to decompresse

INTRODUCTION: Burns are tissue traumas caused by energy transfer and occur with a variable inflammatory response. The consequences of burns represent a public health problem worldwide. Inhalation injury (II) is a severity factor when associated with burn, leading to a worse prognosis. Its treatment is complex and often involves invasive mechanical ventilation (IMV). The primary purpose of this study will be to assess the evidence regarding the frequency and mortality of II in burn patients. The secondary purposes will be to assess the evidence regarding the association between IIs and respirat

Damage to the respiratory system caused by inhalation of toxic products of combustion and the subsequent development of adult respiratory distress syndrome (ARDS) are the main lethal factors in burns patients. Treatment with exogenous surfactant is one of the therapeutic options. However, no clinical reports have yet been published about this therapy. We report on two patients with burn injuries of second to third degree affecting 42% and 48% respectively of the total body surface plus inhalation injury complicated by ARDS. Both patients were treated with bovine surfactant (Alveofact). The the
