Neural crest development in fetal alcohol syndrome.

Fetal alcohol spectrum disorder (FASD) is a leading cause of neurodevelopmental disability. Some affected individuals possess distinctive craniofacial deficits, but many more lack overt facial changes. An understanding of the mechanisms underlying these deficits would inform their diagnostic utility. Our understanding of these mechanisms is challenged because ethanol lacks a single receptor when redirecting cellular activity. This review summarizes our current understanding of how ethanol alters neural crest development. Ample evidence shows that ethanol causes the "classic" fetal alcohol synd

CREST syndrome: clinical manifestations and dental management.

Scleroderma is a multisystem connective tissue disease of unknown etiology, which is characterized by inflammation, vascular, and fibrotic changes of the skin as well as various internal organs. This article will review the systemic form of scleroderma known as CREST syndrome, its clinical manifestations including the dentofacial complications, as well as provide information to assist the prosthodontist in managing the dental treatment for patients with this disease.

A case of CREST syndrome associated with sick sinus syndrome.

Cardiac manifestations of the CREST syndrome or limited systemic sclerosis (ISSc) are very rare. We report a case of CREST syndrome associated with sick sinus syndrome. Histopathology of cardiac muscle revealed fibrotic changes, suggesting that such changes may be pathogenetically related to CREST syndrome.

The CREST syndrome and sarcoidosis. Another example of an overlap syndrome.

The association of CREST Syndrome and sarcoidosis has not been reported before. We describe a 68 year-old woman who first developed CREST Syndrome which was followed by the appearance of sarcoidosis. This clinical picture is another example of an overlap syndrome.

Piebaldism-Waardenburg syndrome: histopathologic evidence for a neural crest syndrome.

Piebaldism, an autosomal dominant trait, is characterized by patchy hypopigmentation of the face, anterior chest, abdomen, and limbs, heterochromia/bicolored irises, congenital megacolon, and deafness. A 4-month-old Inuit (Eskimo) boy with these manifestations also had left pulmonic artery stenosis, ocular ptosis, and unilateral duplication of the renal collecting system. Evidence is presented for both qualitative and quantitative derangement of neural crest derivatives in this syndrome. Histologically, hypoganglionosis, hyperganglionosis, and ectopic ganglia in lamina propria (neuronal coloni

[CREST syndrome and pulmonary hypertension: a dark prognosis].

The CREST syndrome initially described as a limited, more indolent form of diffuse scleroderma, is characterized by calcinosis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasias. Subsequent studies showed that visceral abnormalities were not uncommon. Pulmonary abnormalities are frequent especially pulmonary hypertension, this one being a major cause of this syndrome's mortality. The authors present the case of white woman, 61 years old , with dyspnoea, cyanosis and peripheral edema with 12 months of evolution. She was admitted for pulmonary thromboembolism suspi…

Aerosolized iloprost in CREST syndrome related pulmonary hypertension.

OBJECTIVE: To assess the outcome of patients with CREST syndrome associated severe pulmonary hypertension treated by aerosolized iloprost in a noncomparative study. METHODS: Five patients with CREST syndrome associated severe pulmonary hypertension were treated with 100 microg/day of aerosolized iloprost. New York Heart Association functional class and exercise tolerance (6 min walk test) were assessed at baseline, after one month, and then every 6 months. A right heart catheterization was performed at baseline in all but one patient. Systolic pulmonary artery pressure (PAP) was measured with…

[Seeping gastrointestinal hemorrhage from telangiectasia in CREST syndrome].

A 55-year-old man with CREST syndrome had marked telangiectasias of the palms, the face and the gastrointestinal tract. The latter caused slow bleeding and anemia. The telangiectasias were definitely associated with the CREST syndrome. Iron substitution was sufficient therapy. If a need for further therapy should arise, endoscopic sclerotherapy could be employed. Gastrointestinal bleeding in the CREST syndrome is rarely described, probably because it is unrecognized or interpreted as hemorrhagic gastritis. The possibility of gastrointestinal telangiectasias and concomitant bleeding should be k…

Superior oblique muscle paralysis in the CREST syndrome.

A 54 year old male with the CREST (Calcinosis, Raynauds, Esophageal hypomotility, Sclerodactyly, Telangiectasia) variant of progressive systemic sclerosis (PSS) presented with vertical diplopia diagnosed as a left superior oblique muscle paralysis. Although cranial nerves abnormalities have been reported in PSS, to our knowledge this is the first report of the CREST syndrome in association with a IV nerve palsy in whom no other etiology could be found.

The 'CREST' syndrome. Comparison with systemic sclerosis (scleroderma).

The CREST syndrome refers to a disorder comprising the manifestations of calcinosis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia. Thirteen CREST patients (two with CRST) were compared with 26 patients with systemic sclerosis but without the full manifestations of the CRST syndrome. No significant difference was found between the groups in the age of onset of Raynaud's phenomenon, degree of multiphasic digital color changes, ulcerations of fingers, sclerodactyly, or in the frequency of abnormal esophageal peristalsis or dysphagia. Laboratory results were simi…

Gastrointestinal Hemorrhage in CREST Syndrome: A Case Report.

Systemic scleroderma is an autoimmune disorder caused by microvascular dysfunction, excessive collagen deposition, and progressive fibrosis affecting the skin and other organs. CREST syndrome, also known as limited scleroderma, is an acronym for calcinosis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasias. Among patients with systemic sclerosis (SSc), anticentromere antibodies are typically associated with the CREST syndrome variant. These antibodies are considered disease-specific and are known to correlate with the extent of disease progression. The pathogenes…

Surgical management of digital calcinosis in CREST syndrome.

As a limited form of sclerodermy, CREST syndrome is characterized by calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia, which determine the acronym CREST. Calcinosis is a particularly difficult entity to treat given the paucity of effective options described in the literature. Treatment of finger calcinosis has a wide range of possibilities depending on the extent of calcifications and the involvement of deep structures. From a surgical point of view, whereas simple removal is adequate in minor outpatient cases, a radical debridement in the major and m…

[Granulomatous uveitis and CREST syndrome: a case study].

PURPOSE: To report a case of recurrent granulomatous panuveitis associated with CREST syndrome. OBSERVATION: A 74-year-old patient with CREST syndrome presented with unilateral granulomatous panuveitis in a pseudophakic eye. She had undergone cataract surgery 6 months before. The patient reported a vision loss that had been evolving for 1 month. Visual acuity was noted at 20/400. The initial clinical examination highlighted retrodescemetic precipitates and granulomatous precipitates on the IOL. A vitreous tyndall was noted. Funduscopic examination revealed papillary edema and cystoid macular …
