Compared with rabies in developing countries, human rabies is rare in the United States, but animal rabies is common. In the United States, most human rabies cases are associated with rabid bats, whereas in developing countries, dogs are the most common reservoir and vector species. In March 2008, a case of imported human rabies in a recently arrived, undocumented Mexican immigrant was laboratory confirmed by public health officials in California. The rabies virus isolated from the patient was a previously uncharacterized variant most closely related to viruses found in Mexican free-tailed bat…

We used hospital discharge data to estimate incidence and distribution of coccidioidomycosis-associated hospitalizations in California. For 1997-2002, the average annual rate of hospitalization was 3.67 per 100,000 population. County of residence, older age, black race, male sex, HIV infection, and pregnancy were strongly associated with increased risk for hospitalization.

Coccidioidomycosis is an infection caused by the fungus Coccidioides immitis, which resides in the soil in some areas of Arizona, California, Nevada, New Mexico, Texas, and Utah. Infection can occur when airborne, infective arthroconidia are inhaled. Symptomatic coccidioidomycosis, which occurs in approximately 40% of all infections, has a wide clinical spectrum, including mild influenza-like illness, severe pneumonia, and disseminated disease. Beginning in 1991, the number of cases of coccidioidomycosis reported annually to the California Department of Health Services (CDHS) increased dramati…

Lyme disease is a recently described clinical entity with cutaneous, neurologic, articular and cardiac manifestations. Since the original description of the disease in 1977, more than 500 cases have been reported. Although the vast majority of patients have been from the area near Lyme, Connecticut, we have seen four patients from northern California with various aspects of Lyme disease. This diagnosis should be considered in patients who have traveled to tick regions and who have a distinctive skin lesion (erythema chronicum migrans). Clinical and epidemiologic evidence suggests the disease i…

Methamphetamine and cocaine use have been associated with a vulnerability to HIV infection among men who have sex with men and among men who have sex with women but not specifically among Mexican migrants in the United States. The California-Mexico Epidemiological Surveillance Pilot was a venue-based targeted survey of male and female Mexican migrants living in rural and urban areas in California. Among men (n = 985), the percentage of methamphetamine/cocaine use in the past year was 21% overall, 20% in male work venues, 19% in community venues, and 25% in high-risk behavior venues. Among wome…

Prevalence of leptospirosis was determined in California sea lions (Zalophus californianus) stranded live along the central California (USA) coast between January 1981 and December 1994. Clinical signs of renal disease were seen in 764 (33%) of 2338 animals examined; 545 (71%) of these 764 animals died, with similar gross lesions of nephritis. In silver impregnation stains of sections of formalin-fixed kidney, numerous loosely coiled spiral organisms were observed. Leptospira pomona kenniwicki was cultured from four kidney samples in 1991. Epizootics of leptospirosis occurred in 1984, 1988, 19…

Exposure to tobacco smoke is an important risk factor for chronic obstructive pulmonary disease. We investigated the relationship between chronic obstructive pulmonary disease hospitalization counts (and hospitalization-related charges) in California and sociodemographic and smoking measures, employing geospatial techniques that permit more sensitive scrutiny at the zip code level while controlling for spatial confounding. We analyzed 1,707 zip code tabulation areas in California for chronic obstructive pulmonary disease hospitalization rates and related hospitalization charges (using 1999 hos…

Approximately 4.75% of the animals sent to slaughter from a southern California feedlot during a 9-mo period were found to be infected with the cysticercus of Taenia saginata. An investigation of possible transmission sources revealed that the feedlot was the most likely site of infection. One feedlot worker was found to be infected with T. saginata, and the pen infection patterns and other physical relationships indicated that he was the source of infection. The following recommendations were made: 1) treatment of the infected worker; 2) education of employees as to the mode of transmission a…

Previously unreported white spotting was found in two subspecies of the California vole, Microtus californicus. The pattern of spots on the ventral coat of the animals differs between the subspecies, and there is variation in the expressivity of the white spots. Expression of white spotting is greatly reduced by the epistatic action of another coat colour gene, the recessive buffy (bf). The incidence of white spotting, its variation in expression, and its inheritance were investigated in this study. The reproductive performance of white spotted voles were also analysed, and effects of fertilit…

In coccidioidin skin test surveys among persons of high school age in Saugus, Canoga Park, Banning and Palm Springs areas the average incidence of positive reaction was 15 per cent. Although considerably less than the 68 per cent incidence reported among high school students of Kern County, it is high enough to indicate pockets of relatively high endemicity in Southern California below the San Joaquin Valley. Histoplasmin tests were performed on most of the persons tested with coccidioidin in this survey. The over-all incidence of positive reaction in the group was 7.6 per cent. Most of the su…
