Therapeutic cancer vaccines: development, challenges, and future perspectives.

Therapeutic cancer vaccines are designed to induce tumor-specific immunity and durable antitumor surveillance. However, their clinical efficacy remains limited by antigen heterogeneity, defective antigen presentation, immune exhaustion, and immunosuppression within the tumor microenvironment (TME). Advances in sequencing, immunopeptidomics, artificial intelligence (AI)-assisted neoantigen prioritization, nucleic acid engineering, dendritic cell (DC) biology, and combination therapy have shifted the field from empirical antigen exposure toward mechanism-guided vaccine design. This review outlin…

Natural carrier systems in cancer vaccines and immunotherapy.

Cancer, marked by uncontrolled cell growth, is a leading global cause of death. Conventional cancer treatments such as chemotherapy and radiation, for all their use, often come with severe side effects. This has really pushed the development of immunotherapies and cancer vaccines, which try to leverage the body's immune system to fight tumors. Effective delivery of therapeutic agents to immune cells or tumors is critical, and natural carriers such as exosomes, archaeosomes, virosomes, and dendritic cells offer promising solutions. These systems excel in biocompatibility, low toxicity, and immu…

Peptides-Coated Oncolytic Vaccines for Cancer Personalized Medicine.

Oncolytic Viruses (OVs) work through two main mechanisms of action: the direct lysis of the virus-infected cancer cells and the release of tumor antigens as a result of the viral burst. In this sc.enario, the OVs act as in situ cancer vaccines, since the immunogenicity of the virus is combined with tumor antigens, that direct the specificity of the anti-tumor adaptive immune response. However, this mechanism in some cases fails in eliciting a strong specific T cell response. One way to overcome this problem and enhance the priming efficiency is the production of genetically modified oncolytic …

Neoantigen Cancer Vaccines: Real Opportunity or Another Illusion?

In this communication, we will analyze some important factors and immunological phenomena related to neoantigen cancer vaccines, with particular emphasis on recently published Phase I clinical trials. Several obstacles and issues are addressed that challenge the current paradigm and inquire if neoantigens, which are essentially single-use vaccine candidates, are legitimate targets to induce protective immune responses with regard to the evolving mutational landscape. We also share insights into the striking similarities between cancer and antigenically variable pathogens and suggest that any s…

Therapeutic cancer vaccines revamping: technology advancements and pitfalls.

Cancer vaccines (CVs) represent a long-sought therapeutic and prophylactic immunotherapy strategy to obtain antigen (Ag)-specific T-cell responses and potentially achieve long-term clinical benefit. However, historically, most CV clinical trials have resulted in disappointing outcomes, despite promising signs of immunogenicity across most formulations. In the past decade, technological advances regarding vaccine delivery platforms, tools for immunogenomic profiling, and Ag/epitope selection have occurred. Consequently, the ability of CVs to induce tumor-specific and, in some cases, remarkable …

Radiation and Anti-Cancer Vaccines: A Winning Combination.

The emerging combination of radiation therapy with vaccines is a promising new treatment plan in the fight against cancer. While many cancer vaccines such as MUC1, p53 CpG oligodeoxynucleotide, and SOX2 may be great candidates for antitumor vaccination, there still remain many investigations to be done into possible vaccine combinations. One fruitful partnership that has emerged are anti-tumor vaccines in combination with radiation. Radiation therapy was previously thought to be only a tool for directly or indirectly damaging DNA and therefore causing cancer cell death. Now, with much preclini…

Human papillomavirus, vaccines, and protection from cervical cancer.

Current published data makes clear the relationship between genital human papillomavirus (HPV) infection and cervical cancer. Although there is an opportunity for screening programs that could obviate the disease, cervical cancer still remains the second most common cancer among women worldwide. The subtypes HPV 16 and 18 affect the anogenital tract mucosal surfaces, and accounts for nearly 70% of all cervical cancers, and 90% or more of anal cancer cases. When the 493,000 new cases of cervical cancer and 274,000 deaths per year are taken into consideration, the importance of treatment and pro

Enhancement of dendritic cells as vaccines for cancer.

Dendritic cells are the most potent antigen-presenting cells known; owing to their ability to stimulate antigen-specific cytolytic and memory T-cell responses, their use as cancer vaccines is rapidly increasing. While clinical trials provide evidence that dendritic cells vaccines are safe and elicit immunological responses in most patients, few complete tumor remissions have been reported and further technological advances are required. An effective dendritic cell vaccine must possess and maintain several characteristics: it must migrate to lymph nodes, have a mature, Th1-polarizing phenotype …

Paradigm shifting vaccines: prophylactic vaccines against latent varicella-zoster virus infection and against HPV-associated cancer.

We compare the design, mechanism of action, and clinical efficacy of two recently licensed paradigm shifting vaccines. Zostavax is the first vaccine licensed to prevent disease in patients already infected with a pathogen, and is contrasted with Gardasil and Cervarix, the first vaccines designed and licensed specifically to prevent cancers.

Personalized neoantigen-targeting cancer vaccines in ovarian cancer: a phase 1 trial.

Personalized neoantigen-targeting cancer vaccines in the adjuvant setting are of increasing interest across malignancies. Here we report the results of a phase 1 clinical trial testing the feasibility and safety of personalized neoantigen-targeting cancer vaccines for high-grade epithelial ovarian cancer. 'NeoVax' consisted of pools of synthetic long peptides targeting patient-specific neoantigens admixed with the immunostimulant poly-ICLC, was combined with delayed administration of anti-PD-1 antibody, and was tested in ten patients (one newly diagnosed patient (Cohort A) and nine patients at…

The promising cancer treatment approach in cancer immunotherapy: dendritic cell-based vaccines.

Dendritic cells (DCs), as the most protent professional antigen-presenting cells (APCs), possess an exceptional capacity to initiate and modulate adaptive immune responses, rendering DC-based vaccines a highly promising approach in tumor immunotherapy. This review explores the biological properties of DCs, establishing a foundation for the rational development of DC vaccine. We further provide a comprehensive overview of current mainstream DC vaccine platforms, with analysis of three representative strategies: (i) DC vaccines loaded with tumor lysates; (ii) personalized neoantigen-pulsed DC va

An Insight into Immunological Therapeutic Approach against Cancer: Potential Anti-cancer Vaccines.

The development of a cancer vaccine comes with its complications and designing and developing a vaccine against foreign invaders such as bacterial and viral particles is not as complex and multi-faceted as the preparation of immunotherapy for host-infected cells which resemble our own body cells. The entire research and development framework of designing a vaccine for cancerous cells lies entirely on the remarkable aspect of notifying specific interactions and acclimatising the immune system. This review aims to compile the several fronts research-based methodology applies to in terms of devel

Infections and cancer: debate about using vaccines as a cancer control tool.

In 2012, Infectious Agents and Cancer commissioned a thematic series collection of articles on Prevention of HPV related cancer. The articles have attracted wide interest and stimulated debate, including about the utility of vaccines in cancer control. The application of vaccines to cancer control fulfills a promise envisioned at the turn of the 20th century when remarkable experiments showed that some cancers were caused by infections. This suggested the possibility of applying infection-control strategies to cancer control. Vaccines represent the most practical cost-effective technology to p

Involvement of immunomodulators in the development of cancer vaccines.

Previous research shows that cancer vaccines hold great potential as immunotherapeutic agents which activate the body's tumor-cell elimination powers through immune recognition. Cancer vaccines encounter restricted success because of three limiting factors that include immunosuppressive tumor microenvironment (TME) dynamics and deficient antigen presentation and tumor immune evasion methods. Immunomodulators function as vital instruments which help patients overcome vaccination resistance barriers in addition to strengthening vaccine-generated immunity. The review investigates how different im…

Programming the immunological properties of mRNA vaccines for cancer.

Messenger RNA (mRNA) vaccines are a transformative platform for inducing antigen-specific T cell and B cell responses that are now being trialled in oncology. Here we propose an immunological framework that reconciles four axes controlling the efficacy of mRNA cancer vaccines: adjuvanticity versus immunopathology, antigen immunogenicity versus tolerance, adaptive immune memory versus exhaustion, and beneficial versus maladaptive trained immunity. We argue that mRNA vaccines should be viewed as programmable constructs in which nucleoside chemistry, delivery platforms and dosing schedules can be…
