The efficacy of captopril in the treatment of Raynaud's phenomenon was observed in an open study in 53 patients with primitive Raynaud's disease and in 18 patients with Raynaud's associated with scleroderma. Each patient was given captopril 25 mg three times a day for three months. The drug significantly decreased the frequency and the severity of ischaemic attacks in patients with Raynaud's disease, but did not affect the attacks in patients with scleroderma. Captopril inhibits the metabolism of endogenous opioids in vitro and potentiates their effects in vivo. We examined the hypothesis that endogenous opioids contribute to the actions of captopril in man. Twenty patients with primary or secondary Raynaud's phenomenon were, after a 4-week treatment-free interval, given captopril ( Lopirin ) at a dosage of 25 mg three times daily for 84 days. Marked subjective improvement was reported by 14 patients. Digital-artery pressure, blood flow at rest and blood flow during reactive hyperaemia increased. Blood flow at rest increased significantly after cooling (10 degrees C for three minutes).

Captopril 12.5 mg twice daily was initially administered given to a woman with chronic renal failure and hypertension. Three weeks later, she developed chills, high fever and sore throat. Hemogram showed severe neutropenia; the white cell count showed 600/cu mm; bone marrow aspirate and biopsy revealed a paucity of myeloid series. Antineutrophil antibody was not detected in the serum. Captopril, an orally active angiotensin I-converting inhibitor, is an effective drug in the treatment of hypertension in adults and children. The use in newborn infants has been, nevertheless, short. We report five cases of hypertension in newborn infants poorly responsive to large doses of other potent antihypertensive agents: furosemide, propanolol and hydralazine. A case of necrotizing blepharitis due to a side-effect of captopril medication is described. Blepharitis was the first clinical sign of drug-induced agranulocytosis. The history of drug intake as part of the anamnesis was the most important diagnostic tool for the ophthalmologist since the course of agranulocytotic blepharitis depends on the elimination of the drug.

Two cases of nephrotic syndrome and biopsy-proved membranous glomerulopathy (membranous glomerulonephritis) were encountered during captopril treatment of 53 hypertensive subjects in our institution. A 37-year-old patient with severe hypokalemia (approximately 1.7 mEq/L) and muscular weakness had a very high serum renin level. No renin-producing tumor was found and she was diagnosed with Bartter Syndrome The parathyroid hormone level was high due to concomitant renal calcium loss. Tc-99m-DTPA captopril scintigraphy was performed to confirm the diagnosis.
