Minimally invasive approach has become key in cardiac surgery over the last decade and nowadays involves all areas of expertise, as it was initially developed for mitral valve surgery and has quickly expanded also to surgery for the treatment of aortic valve disease and to myocardial revascularization. Compared to standard sternotomy, the most evident difference is minimalization of surgical incision, which reduces chest trauma with several benefits for patients. Minimally invasive cardiac surgery requires dedicated instrumentation/devices as well as an additive learning curve. Different perfu

OBJECTIVE: This study aimed to develop and evaluate a 3-degree-of-freedom (DoF) robotic system for the safe delivery of cardiac sheaths through challenging anatomical structures, including the fossa ovalis and pathways with tight curves.
METHODS: The robot and its kinematic model were built on a previously proposed single-DoF actuation module and bending model. A sheath delivery strategy (SDS) was developed, combining two control methods: tip position control to approach an optimal entry point and point-constrained control to maintain consistent navigation through this point, minimizing tissu

In recent decades, there has been a growing interest in applying robotic technology to cardiac surgery. The advantages of robotic technology, such as magnified vision, precise robotic instruments and 360° movement of robotic arms, have attracted the cardiac community to reduce the invasiveness of the procedure, postoperative risks, and improve clinical outcomes. While in the United States the adoption of robotic assistance immediately gained widespread acceptance (approximately 1900 cases per year), in Europe the beginning has been slower with a significant growth in the last 10 years. The rob

An alternative technique for minimally invasive aortic valve replacement and atrial septal defect repair is described. After a 5-cm skin incision, a key-lock type sternotomy is made. Excellent exposure of the right atrium and aortic valve was achieved. The configuration of the mini-sternotomy (or the lock) limits the movement of the sternal surfaces (or the key) on the lateral and craniocaudal directions.

The function of the respiratory muscles (RM) is affected positively or negatively by a variety of surgical procedures. Cardiac, thoracic and upper abdominal surgery impair the RM function and lead to postoperative complications such as hypoxia, atelectasis, aspiration and infections. Preoperative assessment of RM function is cardinal to avoid or attenuate these complications. Three types of surgical procedures, lung transplantation, lung volume reduction surgery and surgery for obesity have been shown to improve RM function. A mechanism by which these types of operation have shown beneficial e

Significant improvement and development have occurred in minimally invasive cardiac surgery over the past 20 years. Although most studies have consistently demonstrated equivalent or improved outcomes compared with conventional cardiac surgery, with significantly shorter recovery times, adoption continues to be limited. In addition, cost data have been inconsistent. Further ongoing trials are needed to help determine the exact roles for these innovative procedures.

Robotics entered the cardiovascular field in the late 1990s with a robot-assisted coronary artery bypass graft. Since then, the use of robots has become a common part of cardiovascular surgery in several types of interventions. The experience in transcatheter interventions has been slower, and the application of robotics to percutaneous coronary interventions has shown some encouraging results but also some technical limitations. Following the growth of structural heart interventions, attention has recently switched to the potential application of robotics in this field. So far, several cases

Despite increasing trends in catheter-based cardiac surgical procedures, more than 278 000 Americans had traditional cardiac surgery in 2013. Of those surgical procedures, approximately 133 000 involved valvular repair or replacement. Aortic valve replacement was by far the most common valvular operation, followed by mitral valve repair or replacement. This review article will discuss characteristics of valvular pathologies and postoperative concerns for each the 4 cardiac valves.
