Described in the paper is an analysis of research related with hypertension, atherosclerosis, primary cardiomyopathy and inherited arrhythmia. Methodological approaches as well as possible errors and causes of them are elucidated thoroughly in using different genetic schemes. Pharmacogenetic approaches to an effective treatment of cardiovascular diseases are described. Strategies for further research of cardiovascular pathologies are contemplated.

I have tried to highlight some of the extraordinary advances in cardiology over the last 25 years. Many conditions, however, such as primary pulmonary hypertension or cardiogenic shock remain as refractory to treatment now as then. There is no room for complacency. In addition the increasing cost of technology and the increasing demand for interventional cardiology are not being met. We are far behind our European colleagues in being able to provide this service.

Cardiovascular diseases (CVD), especially coronary artery disease (CAD), are the leading causes of morbidity and mortality worldwide. Elimination of modifiable risk factors has the potential to prevent up to 80 % of CVD. Therefore, disease prevention is based on the assessment of individual total CVD risk by using risk scores such as SCORE-algorithm that include the known main cardiovascular risk factors such as age, sex, smoking status, arterial hypertension und cholesterol.
