This short communication describes a woman in her 30s with cauda equina syndrome (CES) following a vaginal delivery. She presented with urinary retention, bowel incontinence, and radiating pain in the lower extremities. Magnetic resonance imaging demonstrated a herniated intervertebral disc at the L5/S1 level, leading to surgical intervention. This report highlights the rare occurrence of postpartum CES and underscores the importance of prompt diagnosis, distinguishing it from more common postpartum conditions, and timely management to prevent permanent neurological deficits.

Cauda equina syndrome is rarely associated with pregnancy, with few cases reported in the literature. The majority of cases describe antenatal presentations, with only one case manifesting post-partum, three weeks after a normal vaginal delivery. We outline a case of cauda equina syndrome following caesarean section in a patient with known lumbar disc disease, and discuss the contribution of the mode of delivery and anaesthesia as precipitants of disc herniation causing cauda equina compression. We conclude that vigilance, particularly in the post-partum setting, be attributed towards the neur…

We discuss a case of saddle embolism with a clinical presentation similar to cauda equina syndrome in a 79-year-old woman with a history of ischaemic heart disease. Saddle embolus is very rare but one of an array of visceral causes for back and leg pain. This case highlights diagnostic difficulties, particularly in patients with multiple disorders. A high index of suspicion for vascular conditions must be exercised in cases of arterial dysfunction presenting with back pain.

BACKGROUND: Spinal epidural abscesses are uncommon but are well documented in the literature as a serious causes of mortality and long-term neurological morbidity.
AIM: We describe a case of a 57-year-old female who presented to Accident and Emergency with an acute exacerbation of long standing mild lower back pain with symptoms of cauda equina syndrome and a systemic infective process.
CONCLUSION: The diagnosis of this condition requires a high index of suspicion. This report demonstrates that with adequate decompression and appropriate antimicrobial treatment a good outcome is achievable.

We report three cases of cauda equina syndrome following spinal anesthesia with dibucaine. In two cases, the lumbar puncture was repeated and additional doses of dibucaine were administered to obtain adequate sensory blockade. In the last case, spinal anesthesia worked well with single injection of dibucaine. In all cases patients complained of varying degrees of bladder and bowel dysfunction, perineal sensory loss and lower extremity motor weakness on the next day, and the diagnosis of cauda equina syndrome was made. With only one case, the symptom disappeared four months later, but the rest …

Short-lasting Unilateral Neuralgiform headache attacks with Conjunctival injection and Tearing (SUNCT) is a syndrome predominant in males, with a mean age of onset around 50 years. The attacks are strictly unilateral, generally with the pain persistently confined to the ocular/periocular area. Most attacks are moderate to severe in intensity and burning, stabbing or electrical in character. The mean duration of paroxysms is 1 minute, with a usual range of 10 to 120 seconds (total range 5 to 250 seconds). Prominent, ipsilateral conjunctival injection and lacrimation regularly accompany the atta

The high incidence of non-specific, benign back trouble often delays the diagnosis of specific causes of back pain and thus result in neurol … ad to the correct diagnosis only after presentation with neurological deficits. Acute myelopathy or cauda equina syndrome are indications for immediate diagnostic assessment and treatment in a spine centre.

Susac syndrome is a rare neurologic disorder first described by Susac et al. in 1979. Clinically, Susac syndrome consists of a triad includi … cribed to our knowledge. We present a case of Susac syndrome in a middle-aged male with symptoms of cauda equina syndrome, and spinal MRI showing diffuse enhancement of the nerve roots of the cauda equina.

The diagnostic sub-categorization of cauda equina syndrome (CES) is used to aid communication between doctors and other healthcare professionals. It is also used to determine the need for, and urgency of, MRI and surgery in these patients. A recent paper by Hoeritzauer et al (2023) in this journal examined the interobserver reliability of the widely accepted subcategories in 100 patients with cauda equina syndrome. They found that there is no useful interobserver agreement for the subcategories, even for experienced spinal surgeons. This observation is supported by the largest prospective stud…

INTRODUCTION: Cauda equina syndrome (CES) is a rare condition that affects the nerves in the spine supplying the bladder, bowel and sexual function. Identification and subsequent urgent action is required to avoid permanent damage to these essential organs. Delays in diagnosis can have devastating and life changing consequences for patients and result in high cost negligence claims.
PURPOSE: The purpose of this masterclass is to examine the current evidence and provide an evidence-based, clinically reasoned approach in the safe management of patients presenting with CES. It will include a foc…

No previous studies have examined the physical characteristics of patients with cauda equina syndrome (CES). We compared the anthropometric features of patients who developed CES after a disc prolapse with those who did not but who had symptoms that required elective surgery. We recorded the age, gender, height, weight and body mass index (BMI) of 92 consecutive patients who underwent elective lumbar discectomy and 40 consecutive patients who underwent discectomy for CES. On univariate analysis, the mean BMI of the elective discectomy cohort (26.5 kg/m2 (16.6 to 41.7) was very similar to that …
