The place of digital intravenous angiography in cerebral infarcts.

The authors describe their technique of intravenous digital subtraction angiography (IVDSA) and discuss the results in cerebral infarcts. IVDSA is a good first stage examination for the recognition of stenosis or occlusion of cervical vessels to the brain, as it is an only marginally invasive and easy technique. It is, however, good neither for the study of focal cervical not of intracranial areas.

Cerebral angiography in vasculitis affecting the nervous system.

Of 199 patients with clinico-laboratory evidence of vasculitis, 42 were submitted to cerebral angiography; 35 angiograms were abnormal, but only 21 were characteristic of the particular disease process. A pattern consistent with vasculitis diagnosed in 19 angiograms, was due to neoplasm in 4 and neurofibromatosis, DXT and amphetamines each in 1 case. Moya-Moya type collaterals shown in 20 angiograms were due to the idiopathic disease in 10 but due to non-inflammatory pathologies in 7. Fibromuscular dysplasia shown in 28 angiograms was an incidental finding in 20 of these. Of 56 cerebral angiog…

Contrast-induced neurotoxicity following diagnostic catheter cerebral angiography.

A man in his 60s was found to have a 7 mm incidental anterior communicating artery aneurysm on CT while being investigated for debilitating migraines, and subsequently underwent catheter cerebral angiography for endovascular treatment planning. Approximately 1 hour post angiogram, the patient reported reduced vision and a headache. His symptoms rapidly progressed to a severe headache, increased agitation, confusion, cortical blindness and combative behaviour. CT and MRI revealed no acute intracranial abnormality, and he was transferred to the intensive care unit for supportive care. His condit…

Arterial dissections complicating cerebral angiography and cerebrovascular interventions.

BACKGROUND AND PURPOSE: Iatrogenic dissections are an uncommon complication of cerebral angiography. We retrospectively reviewed 12 cases of arterial dissections complicating cerebral angiography and cerebrovascular interventions to evaluate the clinical course of these dissections. METHODS: Cases from a large tertiary center performing a large number of neurovascular procedures were collected retrospectively. The patients' medical records and imaging studies were reviewed, with particular attention given to the cause of the dissection, the development of ischemic events resulting from the di…

Transient global amnesia after cerebral angiography with iohexol.

We describe a patient without a previous history of migraine or epilepsy and with no known vascular risk factors, who suffered subarachnoid haemorrhage. During vertebral angiography using nonionic contrast medium (iohexol), spasm of the basilar artery was seen. The patient suffered transient global amnesia. Angiography 3 months later with the same contrast medium was normal and produced no further deficit. This case lends support to the supposed ischaemic aetiology of transient global amnesia; in patients without other evidence of cerebrovascular disease, arterial spasm may be responsible.

Sustained tumour opacification in infants during cerebral angiography.

Total body opacification in infants following the intravenous injection of a large dose of water soluble contrast medium has long been recognised, but no example of increased radiodensity of abdominal or intracranial tumours has been reported using this technique. This paper reports the sustained opacification of intracranial tumours in three infants during cerebral angiography. Two of the tumours were papillomas of the choroid plexus of the lateral ventricle and the third a medulloblastoma of the fourth ventricle. Three possible explanations of this phenomenon are briefly considered: 1. That …

[Computer tomography angiography of the cerebral and cervical vessels].

Due to the high temporal and spatial resolution in multislice computed tomography it is possible to perform high-quality computer tomography angiography (CTA) of the cerebral and cervical vessels. Compared to digital subtraction angiography (DSA), a method that is still considered to be the golden standard, CTA is faster and can be performed using a minimal invasive technique. In this paper we outline the clinical indications for performing CTA (aneurysms, stroke, vasospasms, arterio-venous malformations, dissection). Examples of scan protocols are given both for angiography of the cerebral ve

[Usefulness of selective cerebral angiography by transradial approach].

Transradial angiography has recently emerged as an alternative to the transfemoral or transbrachial approach, especially for coronary procedures. However, there have been few studies on cerebral angiography using the transradial approach. The purpose of this study was to assess the outcomes, complications, and limitations of selective cerebral angiography via the transradial approach. Selective cerebral angiography by the right transradial approach using 100-cm-long 4-F catheters was performed in 83 patients. Using five types of catheters, the success rates of selective catheterization to the …

We report the usefulness of computed cerebral angiotomography (CT angiography) for demonstrating cerebral aneurysm and the clinical signific … by CT angiography. Of these 36 aneurysms suspected by CT angiography 32 aneurysms were confirmed by cerebral angiography. The detection rate of CT angiography in this report was 89%, higher than those of previous reports. Thirteen aneurysms were located at internal carotid-posterior communicating artery (ICPC) junction. 11 at anterior communicating artery (Acom), 7 at …

Angiography of nonhemorrhagic cerebral infarction in young adults.

The radiographic examinations and hospital records of 93 young adult patients (15-45 years of age) with nonhemorrhagic cerebral infarction evaluated at our institution during the past 9 years were reviewed. The angiographic examinations were abnormal in 76% of patients. The most common abnormalities were embolic disease and atherothrombotic disease. Forty-seven patients underwent angiography within 7 days of their event. There were no major neurologic or systemic complications related to early angiography. We believe that angiography performed early in the course of the illness is a high-yield

Carotid back pressures in conjunction with cerebral angiography.

A method for obtaining carotid back pressures at the time of cerebral angiography is described. These preoperative measurements were compared to intraoperative measurements in 18 patients. Preoperative mean carotid back pressures (usually common carotid pressures) were obtained by occluding the artery with a double-lumen balloon catheter and recording the pressure distal to the balloon. Although the mean arterial blood pressure and the arterial carbon dioxide tension proved to be variables, the correlation of the pressures measured at angiography and at surgery was high (p less than 0.001).

Magnetic resonance angiography in reversible cerebral vasoconstriction syndromes.

OBJECTIVE: To investigate the evolution and clinical significance of vasoconstriction on magnetic resonance angiography (MRA) in patients with reversible cerebral vasoconstriction syndromes (RCVS). METHODS: Patients with RCVS were recruited and followed up with MRA examinations until normalization of vasoconstriction or for 6 months. The vasoconstriction severity of the major cerebral arterial segments (M1, M2, A1, A2, P1, P2, and basilar artery) was scored on a 5-point scale: 0 (0-<10%), 1 (10-<25%), 2 (25-<50%), 3 (50-<75%), and 4 (> or =75%). Subjects with at least 1 segment with a vasocon
