Integration of parts in the facial skeleton and cervical vertebrae.

INTRODUCTION: The purpose of this study was to undertake an exploratory analysis of the relationship among parts in the facial skeleton and cervical vertebrae and their integration as 2-dimensional shapes by determining their individual variations and covariations. The study was motivated by considerations applicable to clinical orthodontics and maxillofacial surgery, in which such relationships bear directly on pretreatment analysis and assessment of posttreatment outcome. METHODS: Lateral radiographs of 61 adolescents of both sexes without major malocclusions were digitized and marked up by…

Giant cell tumour of the seventh cervical vertebra.

A 43-year-old man presented with bubbly expansion and rarefaction of the body of the seventh cervical vertebra. The lesion involved the right side of the vertebral arch and had a soft-tissue component extending mainly anterior to the vertebra. Histopathologic examination of the specimen obtained by open biopsy revealed a giant cell tumour. Giant cell tumours of the spine, excluding the sacrum, are rare. Radiographically, they may be confused with metastatic carcinoma, plasmacytoma, lymphoma, chordoma and even benign lesions, particularly aneurysmal bone cyst and brown tumour of hyperparathyroi

Transverse process fractures of the cervical vertebrae: are they insignificant?

Transverse process fractures of the cervical vertebrae have been considered rare and insignificant. In a retrospective study of 216 patients with cervical fractures evaluated by plain films and computed tomography, we found that transverse process fractures were common. Transverse process fractures were present in 24% of patients with cervical fractures and accounted for 13.2% of all cervical fractures. Cervical radiculopathy and brachial plexus palsy were present in 10% of patients with transverse process fractures. In 78% of transverse process fractures, CT scanning showed that the fracture …

Cervical butterfly-block vertebra. A case report.

A new type of block vertebral malformation is described. A 42-year-old woman presented after an akinetic seizure. Routine cervical spine films, obtained because of a recent history of abnormal head posture, disclosed a vertebral anomaly. Computed tomography and conventional polytomography revealed that the lesion was composed of a butterfly vertebra as the upper portion of a pair of nonsegmented vertebrae. Vertebral fusion and segmentation anomalies are reviewed and compared with the specific features of this case.

[Fractures of the dens of the 2d cervical vertebra and their treatment].

Fractures of the dens axis make 10-15 per cent to all injuries of the cervical portion of the vertebral column. The potential danger of these injuries and difficulties of their treatment are conditioned by anatomo-functional features of this portion of the spine. The mechanism of fractures of the dens axis is not clear yet. Clinical manifestations of dens axis fractures are determined by the character and extent of its desplacement. Its diagnosis is somewhat difficult sometimes. Conservative therapy is not always a success. The existing methods of surgical treatment of dens axis fractures make

[Errors in diagnosis and treatment of injuries of cervical vertebrae].

An analysis of diagnostic and therapeutic errors is presented based on observations of 264 patients with injuries to the cervical spine and their sequelae. Errors in interpretation of x-ray films could arise through improper knowledge of the roentgenological picture of cervical vertebrae, especially of the first and second. In clinics of general surgery skeletal extension is employed only in limited cases, whereas in applying the Glisson loop axial deformities of the spine could be eliminated but not always to a full extent. Errors in estimating the capacity for work result in development of p…

Optimal entry points and trajectories for cervical pedicle screw placement into subaxial cervical vertebrae.

The present study was performed to determine the optimal entry points and trajectories for cervical pedicle screw insertion into C3-7. The study involved 40 patients (M:F = 20:20) with various cervical diseases. A surgical simulation program was used to construct three-dimensional spine models from cervical spine axial CT images. Axial, sagittal, and coronal plane data were simultaneously processed to determine the ideal pedicle trajectory (a line passing through the center of the pedicle on coronal, sagittal, and transverse CT images). The optimal entry points on the lateral masses were then

Quantitative morphometric study of the subaxial cervical vertebrae end plate.

BACKGROUND CONTEXT: Cervical disc arthroplasty has been gradually adopted as an alternative for the treatment of cervical degenerative disease. However, there is a large discrepancy between footprints of currently available cervical disc prostheses and anatomic dimensions of cervical end plates. PURPOSE: This study aimed to accurately and comprehensively quantify the three-dimensional (3D) anatomic morphology of the cervical vertebral end plate and provide a theoretical basis for designing appropriate disc prostheses. Moreover, we introduced a novel geometric and mechanical model for 3D recon

Extreme selection in humans against homeotic transformations of cervical vertebrae.

Why do all mammals, except for sloths and manatees, have exactly seven cervical vertebrae? In other vertebrates and other regions, the vertebral number varies considerably. We investigated whether natural selection constrains the number of cervical vertebrae in humans. To this end, we determined the incidence of cervical ribs and other homeotic vertebral changes in radiographs of deceased human fetuses and infants, and analyzed several existing datasets on the incidence in infants and adults. Our data show that homeotic transformations that change the number of cervical vertebrae are extremely…

[Non-cerebrovascular complication in chirotherapy manipulation of the cervical vertebrae].

PURPOSE: Chirotherapy is a popular and successful management option for reversible functional disorders of the cervical spine. Though rarely observed, complications do occur, mainly involving the cerebrovascular system. By means of the here described case and a literature survey, we aim to highlight non-cerebrovascular complications of chirotherapeutic cervical spine manipulation. RESULTS: A 43-year-old male initially consulted an ENT specialist, suffering from tinnitus aurium and loss of hearing ability. His hearing significantly increased after intravenous drug therapy, but the tinnitus rem

[Central retinal artery occlusion in surgery of the cervical vertebrae].

Retinal artery occlusion related to ocular compression is an uncommon but severe complication of general anaesthesia, most of the time results in blindness. We report a case of central retinal artery occlusion following spinal surgery. We discuss the mechanism of blindness and review the circumstances that may lead to this complication. Proper positioning of the head and avoidance of ocular compression will prevent the occurrence of retinal occlusion during general anaesthesia.

[Immediate and delayed luxation of lower cervical vertebrae (author's transl)].

The secondary occurence of traumatic cervical spinal luxation is discussed with reference to three cases where there were no radiologic signs of dislocation immediately following the accident. Pathomechanics, diagnostic possibilities and treatment are discussed. Surgical realignment via the anterior access route, with intercorporeal spondylodesis, has proved advantageous. The name "immediate and delayed cervical spinal luxation" is suggested for cases of vertebral dislocation occurring some time after the accident.
