Chest pain, palpitations and panic.

Chest pain and palpitations are commonly described in the general population and are frequent presenting symptoms in all medical settings. Although often transient, they can be persistent and are associated with considerable disability, distress, and concern about heart disease and use of medical resources. Both symptoms are associated with panic but also with other psychiatric disorders. It is most useful to consider etiology as multicausal and interactive; psychological factors affect interpretation of bodily perceptions and have deleterious effects on quality of life and use of medical reso…

Chest Pain, Cough, and Hemoptysis.

An 18-year-old male presented with a 2-day history of left-side anterior chest pain. The pain was intermittent, crampy, and worsened with exertion. Associated symptoms were cough with sputum, night sweats, fever, decreased appetite, and 4.5-kg weight loss; cavitary lesions were seen on chest radiograph. The patient also had significant dental caries. Nasal biopsy revealed findings consistent with Wegener's granulomatosis. Combined treatment with cyclophosphamide and prednisone significantly improved his symptoms over the long term.

Recurrent chest pain in children.

Chest pain in children is relatively common and is often benign and self-limited, but it has received scant attention in the pediatric literature. The author offers rational guidelines for diagnosing the causes of chest pain and comments on the management and follow-up of the patient.

Chest Pain-Specific Legal Risk.

Chest pain is a common chief complaint in the emergency department. When looking at patients with chest pain cases of acute coronary syndrome, pulmonary embolism and aortic dissection account for the majority of cases that involve an allegation of malpractice. While it is likely impossible to catch all these cases having a structured approach to these patients may improve outcomes for both patients and clinicians.

Chest Pain: Common Side Effect.

For chest pain, standards of care are based on emerging evidence-based practice. Angina (chest pain) is the feeling of pressure, fullness, burning, squeezing, or tightness in the chest, usually provoked by exertion and relieved by rest. The discomfort can occur in the shoulders, arms, neck, jaw, or back, and radiates to one or both arms. Women who experience acute myocardial infarction often present with atypical chest pain and other symptoms, such as dyspnea, weakness, and fatigue.

Neurological Causes of Chest Pain.

PURPOSE OF REVIEW: Chest pain is a very common presenting complaint among patients in the hospital, a large proportion of whom have non-cardiac chest pain (NCCP). Neurological causes of NCCP have not been previously reviewed although several causes have been identified. RECENT FINDINGS: Chest pain has been reported as a symptom of multiple neurological conditions such as migraine, epilepsy, and multiple sclerosis, with varying clinical presentations. The affected patients are often not formally diagnosed for long periods of time due to difficulties in recognizing the symptoms as part of neuro…

Deciphering chest pain in women.

Chest pain syndromes in pregnancy.

Chest pain syndromes in pregnancy include numerous catastrophic cardiovascular events. Acute myocardial infarction, aortic dissection, pulmonary embolism, and amniotic fluid embolism are the most important causes of nonobstetric mortality and morbidity in pregnancy. Each of these could result in poor maternal and fetal outcomes if not diagnosed and treated in a timely fashion. However, their diagnosis and management is limited by fetal risks of diagnostic procedures, dangers of pharmacotherapy and interventions that have neither been widely studied nor validated. This article reviews the curre…

Atypical chest pain in athletes.

Initial management of chest pain in athletes always should involve assessment for serious, life-threatening causes, such as myocardial infarction. However, atypical chest pain, or chest pain not due to myocardial ischaemia, is a common presentation in the athletic population. This review looks at the possible causes of atypical chest pain in athletes, focusing upon conditions that are more common in athletes than the general population or that have a link to exercise. Causes can be grouped due to the system involved (musculoskeletal, gastrointestinal, respiratory, cardiac) with more common cau…

Chest pain: a clinical assessment.

Chest pain is one of the most common presentations in emergency medicine. The initial evaluation should always consider life-threatening causes such as aortic dissection, pulmonary embolism, pneumothorax, pneumomediastinum, pericarditis, and esophageal perforation. Radiographic imaging is performed in tandem with the initial clinical assessment and stabilization of the patient. Radiologic findings are key to diagnosis and management of this entity.
