Vaginal disinfection with chlorhexidine during childbirth.

The purpose of this study was to determine whether chlorhexidine vaginal douching, applied by a squeeze bottle intra partum, reduced mother-to-child transmission of vaginal microorganisms including Streptococcus agalactiae (streptococcus serogroup B = GBS) and hence infectious morbidity in both mother and child. A prospective controlled study was conducted on pairs of mothers and their offspring. During the first 4 months (reference phase), the vaginal flora of women in labour was recorded and the newborns monitored. During the next 5 months (intervention phase), a trial of randomized, blinded…

Severe anaphylaxis after a chlorhexidine bath.

Anaphylaxis to chlorhexidine is rare. We report a patient who experienced anaphylaxis during a whole body bath with chlorhexidine. Early signs of a type I allergy may have been masked because of previous concomitant treatment with corticosteroids and PUVA.

[Anaphylactoid shock caused by chlorhexidine gluconate].

We report two cases of anaphylactoid shock caused by chlorhexidine gluconate. Both patients had skin flare, severe hypotension and increased airway pressure during cannulation of an antibacterial IVH catheter containing chlorhexidine gluconate after skin sterilization with chlorhexidine gluconate. In case 1, we did not identify the mechanism and causative drugs. In case 2, an intradermal test to chlorhexidine gluconate was positive 2 months later. Then we confirmed that the anaphylactoid shock was caused by chlorhexidine gluconate. We should bear in mind the risk of anaphylactoid shock when we…

Chlorhexidine-induced degeneration of adrenergic nerves.

Possible toxic effects of chlorhexidine (CHX) on the sympathetic adrenergic ground plexus were studied in whole mounts of albino rat irides using Falck-Hillarp fluorescence histochemistry. CHX dissolved in an isotone , buffered sodium-acetate solution or in 70% alcohol was injected into the anterior chamber of eye. CHX caused a marked and dose-dependent degeneration of adrenergic nerves. Two days after the lowest dose, 0,25 micrograms (5 microliters of a 0.05% CHX solution), approximately 30% of the nerves had disappeared. Almost complete degeneration was observed after the same time with high…

Allergic contact dermatitis to chlorhexidine in children.

BACKGROUND: Hypersensitivity to chlorhexidine is rare in children. The usefulness of different diagnostic tests for allergic contact dermatitis (ACD) is unknown. OBJECTIVE: We aim to describe a case series of children with hypersensitivity to chlorhexidine and to compare the results of different diagnostic tests. PATIENTS: Nine children with hypersensitivity to chlorhexidine were included. Only six of these children and their parents consented to a comprehensive allergy assessment, which included the prick test, intradermal test, patch test, and repeated open application test (ROAT). RESULT…

Harmful effects of chlorhexidine on hepatic metabolism.

Chlorhexidine (CHX) is an over-the-counter antiseptic amply used by the population. There are reports that CHX acts in mitochondria as an uncoupler and inhibitor. The purpose of this study was to investigate the short-term effects of CHX on hepatic metabolic pathways linked to energy metabolism in the perfused rat liver. The compound inhibited both glucose synthesis and the urea cycle. Oxygen consumption was raised at low concentrations (up to 10 μM) and diminished at higher ones. A pronounced diminution in the cellular ATP content was observed. Conversely, CHX stimulated glycolysis and enhanc…

Virucidal efficacy of chlorhexidine: a systematic review.

This study aimed to systematically review the literature about the virucidal efficacy of CHX in comparison to other substances used in the oral cavity. Electronic searches were performed in four databases (PubMed, Scopus, Embase, and Web of Science). Only studies that presented the following characteristics were included: (1) verified virucidal efficacy of CHX against Herpes Simplex Type-1 (HSV-1), any Influenza, or any human coronavirus (HcoV); and (2) compared the virucidal efficacy of CHX with essential oils (Listerine®), quaternary ammonium compounds, povidone-iodine, hydrogen peroxide, ne

Prevention of Chlorhexidine Ototoxicity With Poloxamer in Rats.

OBJECTIVE: Skin preparations, like chlorhexidine, are toxic to the inner ear, preventing their use as a preoperative skin disinfectant in the presence of a nonintact eardrum (e.g., perforation or tube). We aimed to determine if a poloxamer gel applied over perforated eardrums could prevent chlorhexidine ototoxicity. STUDY DESIGN: Controlled, in vivo. SETTING: Academic research laboratory. SUBJECTS AND METHODS: Bilateral myringotomies were performed on 20 Sprague-Dawley rats. Half had poloxamer placed over both eardrums before treatment. All rats had chlorhexidine instilled in one ear and sa…

Clinical efficacy of a chlorhexidine-delivering toothbrush.

OBJECTIVES: Evaluate the efficacy and safety of an experimental toothbrush with a slow-release system of chlorhexidine (CHX) and determine its ability to inhibit plaque, bleeding, staining and oral tissue abnormalities during 6 weeks of use. MATERIAL AND METHODS: One hundred and fifty healthy volunteers were randomly assigned to one of three groups: the Test Brush group with a template slow-delivery system of CHX (Ttb), the Control Brush group without CHX (Ctb) and the Control Brush group without CHX but rinsing post-brushing with a 0.2% CHX mouthrinse (Ctb+R). At baseline as well as at 3 and…

The local delivery of chlorhexidine in a bioresorbable matrix.

The use of local antimicrobial agents, such as the chlorhexidine chip, should be used cautiously and not without thorough debridement or scaling and root planing of the area in question. Because these chips are basically nonsurgical, patients become enthusiastically excited and relieved to learn they don't need surgery. But patients must be strongly advised that surgical intervention may be a next step in the correction of the periodontal pocket if significant change is not attained with local antimicrobials.

Prevention and control of neonatal pyoderma with chlorhexidine.

The extent of staphylococcal colonisation and neonatal pyoderma was studied in 5 220 infants in two large Swedish hospitals in 1979-1982. A method for routine cord care with 4% chlorhexidine detergent solution was evaluated in one hospital. Nursery colonisation with Staph. aureus and streptococci decreased significantly. The method proved to be clinically efficient for prevention of pyoderma in the nursery as well as for controlling staphylococcal infection after discharge from hospital. The combined use of a quantitative sampling method and phage-typing showed that most post-discharge lesions…

Chlorhexidine in cosmetic products - a market survey.

BACKGROUND: Chlorhexidine may cause type I and type IV allergy. Some chlorhexidine-allergic individuals have been exposed in the healthcare setting as patients or healthcare workers, but for others the source of sensitization is unknown. Chlorhexidine may be used as a preservative or an antimicrobial agent in cosmetic products at a concentration up to 0.3%, as set by the European Cosmetics Directive (now Regulations). OBJECTIVES: To identify cosmetic product types containing chlorhexidine, and to measure the concentration of chlorhexidine in selected products. METHODS: Between February 2013 …

Neonatal skin and chlorhexidine: a burning experience.

PURPOSE: Despite the lack of safety data, chlorhexidine gluconate (CHG) is an antiseptic with broadspectrum coverage often used in neonatal intensive care units (NICUs). Adverse skin reactions, most commonly burns, have been reported after the use of CHG. Preserving skin integrity in preterm infants is vital in the prevention of sepsis, excessive water loss, hypothermia, and renal failure. DESIGN: This is a case report of two incidents of significant skin burning in extremely low birth weight (ELBW) infants who were treated with CHG for the purposes of umbilical cord sterilization prior to um…

The influence of chlorhexidine on the remineralization of demineralized dentine.

OBJECTIVES: To examine the differences in the amounts of bound chlorhexidine (CHX) on demineralized dentine blocks and to investigate the different aspects of remineralization of demineralized dentine according to different concentrations of CHX. METHODS: Dentine blocks (2 mm × 7 mm × 0.9 m) were demineralized in 0.2 M formic acid solution. Amount of bound CHX on the dentine blocks was measured on a spectrophotometer after the dentine block was soaked in 0.02%, 0.2%, or 2% CHX solutions for 1 min. The change in elastic modulus of dentine block stored in simulated body fluids was measured at 0…

[Subgingival application of chlorhexidine in the treatment of periodontitis].

The clinician is frequently faced with poor treatment outcome of scaling and root planing predominantly in deep pockets and furcations. In an attempt to optimize clinical effects at these weakly responding sites chemo-mechanical treatment strategies have been developed and studied, among those using chlorhexidine as an antimicrobial agent. In this overview the clinical effects of subgingival chlorhexidine irrigation, gel, chip and varnish administration as an adjunct to scaling and root planing in the treatment of periodontitis are discussed. There is little to no evidence indicating treatment…

Chlorhexidine gluconate and acetate in patch testing.

Patch testing to chlorhexidine is usually performed with chlorhexidine gluconate aq. We report the results of 297 patients, almost all with leg ulcers, concomitantly tested with chlorhexidine acetate 1% aq. and chlorhexidine gluconate 1% aq. 39 patients had positive reactions to one of these compounds or to both. 36 positive reactions to the acetate were found, in contrast to 18 reactions to the gluconate. The reactions were considered relevant in 22 of 39 patients, since these patients had developed an eczema in an area where a chlorhexidine compound was used, and discontinuing the chlorhexid…
