BACKGROUND AND OBJECTIVE: To present a new method for the management of toxoplasmic retinochoroiditis (TRC).
METHODS: The patients were females ranging in age from 10 to 61 years (average 26.5). Four eyes of 4 patients were treated with intravitreal injections of 1.0 mg clindamycin in 0.1 mL and 1.0 mg of dexamethasone in 0.1 mL. The injections were given under general or peribulbar anesthesia. Three patients continued one systemic drug. Follow-up ranged from 11 to 26 months (mean 17.5).

Toxoplasma polymyositis was diagnosed in a dog given repeated injections of glucocorticoids for a skin condition. The dog developed generalized lower motor neuron paralysis, with generalized hyperesthesia, myoglobinuria, and high serum muscle enzyme activities. Diagnosis of toxoplasmosis was confirmed by muscle biopsy and seroconversion. Despite supportive care, the dog's neurologic status deteriorated progressively until parenteral treatment with clindamycin was instituted. The dog made a gradual recovery, with complete return of locomotor function and muscle mass within 2 months.

Bacterial infections of the upper respiratory airways are common. Some of these infections are caused by anaerobic bacteria and Staphylococcus aureus and may constitute considerable therapeutical problems. Chronic suppurative otitis media may sometimes give rise to serious osteitis of surrounding bone structures, sigmoid sinus thrombosis and intracranial abscesses. The causative microorganisms are mostly anaerobes. Many paranasal sinus infections are also due to anaerobic bacteria. These infections can rapidly cause irreversible damage of the sinus mucosa and eventually spread intracranially.

Eight patients with active toxoplasmic retinochoroiditis were treated with clindamycin and a triple sulphonamide combination for a period of up to 6 weeks. In addition systemic prednisone was administered when the optic nerve or the macula was involved (6 of the 8 cases). In all cases we observed clinical improvement within 2 weeks. Visual acuity improved in all cases. Side effects of the treatment were observed in 2 cases. The follow-up period ranged from 8 to 22 months.

Seventeen patients with active toxoplasmic retinochoroiditis were treated for four weeks, seven with clindamycin alone and ten with a combination of clindamycin and sulfadiazine. Clindamycin is a recently developed, semisynthetic antibiotic whose spectrum of activity suggests a potential for the treatment of common ocular infections. Eighty patients with moderate acne vulgaris were enrolled from out-patient department for the comparative evaluation of clindamycin phosphate 1% and clindamycin phosphate 1% with nicotinamide gel 4%. In group I forty patients were given clindamycin phosphate 1% alone.ln group II forty patients were given clindamycin phosphate 1% and nicotinamide gel 4% in combination. We have recently observed four patients with bacteremic infections due to group A streptococci that were highly resistant to clindamycin and only moderately susceptible to erythromycin. Two of the infections developed while the patients were receiving clindamycin. The organisms were not beta-hemolytic under standard conditions nor were they M or T typable. We urge that all streptococci isolated from patients with serious infections be grouped serologically and be tested for antibiotic susceptibilities, especially if therapy with antibiotics other than penicillin is to be considered.

A 25-year-old black female heroin addict presented with Staphylococcus aureus endocarditis. Because of a history of penicillin allergy, therapy was gegun with cephalothin (Keflin); lack of clinical response led to the use of clindamycin. Response was excellent. After six weeks of treatment she was discharged well, only to return six days later with recurrent endocarditis.
