Tuberculosis in Far North Queensland, Australia: a retrospective clinical audit.

BACKGROUND: Compared with global numbers, Australia has enjoyed relatively good tuberculosis control over the past 30 years, with an annual incidence of 5.7 per 100 000 population. Thanks to its unique geography and proximity to high-burden countries, such as Papua New Guinea (PNG), Far North Queensland (FNQ) has previously been shown to have higher rates of tuberculosis compared with both the state and national average. AIMS: To document tuberculosis epidemiology in FNQ compared to two previous audits of the region. METHODS: Retrospective clinical audit of all cases of tuberculosis reported…

Data quality audit of the arthroplasty clinical outcomes registry NSW.

BACKGROUND: The Arthroplasty Clinical Outcomes Registry NSW (ACORN) was initiated in 2012. ACORN is a registry piloting within NSW, Australia with several participating hospitals; it aims to monitor patient-centred outcomes and post-surgical complications after total hip and knee arthroplasty. Using retrospective audit methodology, we aimed to investigate the completeness and accuracy of data in ACORN. METHODS: We undertook a reabstracting audit of 100 clinical records of patients who underwent surgery in 2012/2013 (50 each from hospitals A and B). These records represented 27% (100/367) of p

Postoperative infection after excisional toenail matrixectomy: a retrospective clinical audit.

BACKGROUND: Excisional toenail matrixectomies are performed on the area of the foot that has been reported to have the highest concentration of resident microorganisms. A retrospective infection audit was performed to identify whether this unique area of the foot was more susceptible to postoperative infection. METHODS: A retrospective audit reviewing the postoperative infection rate over a 6-year period after excisional nail matrixectomy in 111 patients was undertaken. RESULTS: The postoperative infection rate was found to be high (18.9%) relative to that of clean orthopedic foot and ankle

Diagnostic Terminology and Clinical Practice in Interstitial Ectopic Pregnancy: A Five-Year Clinical Audit.

Background Interstitial ectopic pregnancy is rare and carries risks of rupture and severe haemorrhage. Accurate terminology is important because interstitial and cornual pregnancy are often used interchangeably despite representing distinct implantation sites. The primary objective of this five-year audit was to assess compliance with anatomically precise diagnostic terminology. Secondary objectives evaluated ultrasound assessment, escalation of diagnostically uncertain findings, and documented individualisation of management. Methods We retrospectively audited confirmed cases diagnosed betwee

Using ambient AI in clinical consultations: reframing policy around clinical audit and patient safety.

Recent work has mapped what is legally permitted, commercially practiced, and technically coming for ambient AI in clinical care, yet it has not addressed the prior question of what the recording is for. This Perspective offers a policy argument, not an empirical evaluation. Documentation, workflow efficiency, patient recall, medico-legal transparency, and quality improvement are all legitimate purposes of consultation recording; we propose that one further, high-value purpose has been under-recognized: clinical audit. The high-fidelity transcript makes communicated clinical reasoning observab…

Ask-Audit-Apply: A Framework for Clinical Reasoning with Artificial Intelligence.

Artificial intelligence (AI)-enabled clinical decision support (CDS) tools are increasingly embedded in clinical workflows, generating and prioritizing clinical information, including diagnostic suggestions, evidence summaries, and management recommendations. Yet guidance on how clinicians should reason with AI at the point of care remains limited, creating risks of over-reliance, inconsistent verification, and diffusion of responsibility. We propose Ask-Audit-Apply (AAA) as a clinician-facing framework for AI-supported clinical reasoning that specifies decision-level behavioral practices. AAA

Testing for Mycoplasma genitalium in pelvic inflammatory disease: A clinical audit.

The records of women attending Gold Coast health hospital sites were retrospectively analysed to determine if women diagnosed with pelvic inflammatory disease (PID) were being tested for Mycoplasma genitalium (MG). Only 11.4% of 299 women were tested for MG despite 74.2% being tested for Chlamydia trichomonas (CT) and Neisseria gonorrhoeae (NG). Only 9% of the women were treated with antibiotics which would treat macrolide-sensitive MG infection. Increasing education and awareness of MG and utilising reflex macrolide testing for MG will help direct effective antibiotic therapy and prevent the

INTRODUCTION: Phlebotomy is taking a venous blood sample for a medical biology analysis .If the taking of a sample is poorly executed, the results for this sample may be inaccurate and mislead the clinician, or the inconvenience of the patient having to undergo a new levy. The three main problems associated with errors in the collection are: hemolysis, contamination and mislabelling. We conduct clinical audit to evaluate compliance of activities in relation to the recommendations. Our objective was to determine the compliance rate of the different steps of the phlebotomy procedure and propose …

Clinical audit and quality improvement: rivals, partners, or one and the same?

Clinical audit and quality improvement are both central to improving healthcare quality; however, there is often a lack of conceptual clarity about their similarities and differences. They are frequently misunderstood, conflated, or applied in ways that do not match their primary purpose. In the United Kingdom, this lack of conceptual clarity is evident across clinical practice, healthcare improvement literature, and professional curricula. The misapplication of methods may contribute to inefficient use of resources and missed opportunities to improve experiences and outcomes for patients, fam…

Management of clinical fractures of the scaphoid: results of an audit and literature review.

OBJECTIVES: Two to 5% of scaphoid fractures are missed on initial presentation. The failure of early recognition and treatment are considered to contribute to delayed union and non-union. Despite advances in diagnostic imaging, a dogmatic approach has persisted in the management of patients with clinical suspicion but no radiographic evidence of scaphoid fracture. A critical analysis of the current treatment protocol of indiscriminate cast immobilization and serial clinical and radiographic follow-up is presented. METHODS: A prospective study involving 90 patients with clinical signs suggesti

[Medical records audit in primary care clinics of the Israel Navy].

The primary care clinics of the Israeli Defense Forces are similar to those of the civilian health system, yet some characteristics are unique: Soldiers are a young, healthy population; their physicians are either serving in the reserves for short periods, or are primary care physicians of the regular army; and during military service the soldier is usually treated in several different primary care clinics. A detailed medical record facilitates communication between the various primary care physicians. As part of a 2-year quality assurance project all naval clinics in Israel were surveyed at 6
