Production of 2,3-dihydroxyisovalerate by Enterobacter cloacae.

2,3-Dihydroxyisovalerate is an intermediate of the valine synthesis pathway. However, neither natural microorganisms nor valine producing engineered strains have been reported yet to produce this chemical. Based on the 2,3-butanediol synthesis pathway, a biological route of 2,3-dihydroxyisovalerate production was developed using a budA and ilvD disrupted Klebsiella pneumoniae strain in our previous research. We hypothesised, that other 2,3-butanediol producing bacteria could be used for 2,3-dihydroxyisovalerate production. Here a budA disrupted Enterobacter cloacae was constructed, and this st

Unveiling a Unique Posterior Cloaca Variant: Expanding the Spectrum.

A posterior cloacal variant is a congenital malformation where a urogenital sinus terminates anterior to a normally placed anus. These are rare malformations with highly variable anatomy. We report on three cases of a novel phenotype of posterior cloaca encountered at our institutions between October 2021 and November 2023. Three newborn girls were referred with ambiguous external genitalia and an anorectal malformation. In all cases, a midline sac, which is likely fused labioscrotal folds, replacing the clitoris was noted anterior to the perineal orifices. Two of the three patients demised as

DNA copy number variations in patients with persistent cloaca.

PURPOSE: Persistent cloaca is a devastating female anomaly associated with renal insufficiency/failure, urinary and fecal incontinence and müllerian dysfunction. Genetically engineered murine models of persistent cloaca suggest that this anomaly could have a genetic component in humans. Genomic copy number variations account for previously unexplained genetic diseases by identifying candidate genes in various disorders. We assessed whether novel copy number variations are present in patients with persistent cloaca. MATERIALS AND METHODS: With institutional review board approval we performed a

MRI of persistent cloaca: can it substitute conventional imaging?

PURPOSE: To define the role of MRI in the preoperative assessment of patients with persistent cloaca and whether it can substitute other imaging modalities. METHODS: We prospectively examined eleven patients with persistent cloaca between July 2007 and March 2012. Non contrast MRI examinations were performed on 1.5T magnet using head coil. Multiple pulse sequences (T1WI, T2WI, fat suppression) were obtained in axial, sagittal and coronal planes of the pelvis, abdomen, and spine. The scans were reviewed for the following: the level and type of rectal termination, the developmental state of str

The development of excitatory neurons in the chick cloaca.

INTRODUCTION: The enteric nervous system of the chick embryo hindgut is derived from the vagal and sacral neural tube. Nerve cells from these regions produce various neuronal phenotypes, including inhibitory and excitatory nerve cells, providing intrinsic innervation to the chick embryo cloaca. We hypothesised that the vagal and sacral neural tubes provide the cloaca, with phenotypically similar nerve cells. The aim of our study was to investigate the origin of excitatory neurotransmission in the developing cloaca. MATERIALS AND METHODS: Chicken embryos were incubated until the 10-12 somite s

Bladder function after total urogenital mobilization for persistent cloaca.

PURPOSE: Total urogenital mobilization has achieved widespread use because it avoids surgical complications of vaginal reconstruction. We used urodynamic studies to assess bladder function and urological outcomes after definitive repair by total urogenital mobilization. MATERIALS AND METHODS: We identified 27 girls diagnosed with persistent cloaca between 1991 and 2008. We retrospectively reviewed the records of 11 patients (median age 14 months) who underwent reconstruction of the cloaca by total urogenital mobilization. Urinary continence was assessed in 7 patients older than 4 years. Urody

Vagal neural crest contribution to the chick embryo cloaca.

Intrinsic innervation of the developing chick cloaca is provided by the enteric nervous system, a network of neurons and glia that lies within its walls. The enteric nervous system originates from neural crest cells that migrate from the vagal and sacral regions of the neural tube during the early stages of development. Abnormal cloacal development can cause a number of anorectal anomalies including persistent cloaca. Our study aimed to investigate the contribution of vagal neural crest cells to the total population of enteric neurons and glia within the chick embryo cloaca, using quail-chick

Severe lower limb defects in exstrophy of the cloaca.

We present here a patient with exstrophy of the cloaca associated with severe lower limb defects. The limb malformations include, on the right, a split foot with distal separation of the tibia and fibula, and on the left, attachment of the lower half of the left leg with a two-toed foot at nearly a right angle to the mid left thigh. A review of the literature indicates that 17-26% of patients with cloacal exstrophy also have lower limb defects. We hypothesize that cloacal exstrophy and associated lower limb defects have a related pathogenesis and that both are related to deficiencies of caudal

Gynecologic concerns in the treatment of teenagers with cloaca.

BACKGROUND/PURPOSE: Gynecologic anomalies are common in patients with persistent cloaca, but, except for hydrocolpos, these patients remain asymptomatic in the neonatal period. Anatomic abnormalities may become manifest in puberty when menses occurs. The authors sought to describe the sequelae of these anatomic defects in the teenage cloaca patient and to determine recommendations for prevention and treatment in the neonatal period. METHODS: From a series of 198 patients operated on for persistent cloaca, the authors report a group of 22 patients who have reached puberty. RESULTS: Seven pati

[Operative correction of a urogenital malformation with persistent cloaca].

A case of malformation of the urogenital system with a persistent urogenital sinus and a cloaca is reported. There was unilateral agenesis of the kidney, megaureter and a double ureter with an ureteral orifice into the vagina and the clitoris. There was a double uterus and vagina with an orifice into the terminal gut. The patient had chronic recurrent urinary tract infections which required a colostomy during childhood. The operation largely restored a normal urogenital area and a functional anus.

Efficacy of vesicostomy for refractory metabolic acidosis in persistent cloaca.

Persistent cloaca involves fusion of the bladder, vagina, and rectum into a single duct called the common duct. Although its pathogenesis remains unclear, it has been associated with hyperchloremic metabolic acidosis. Herein, we present the case of a neonatal girl with high-confluence type variant of persistent cloaca treated with vesicostomy (Blocksom) for refractory metabolic acidosis. She was diagnosed with persistent cloaca before birth; colostomy was performed and a urinary catheter was placed in the bladder. Voiding cystourethrography on day 19 after birth showed that most of the contras

Prenatal ultrasound findings of covered bladder exstrophy and persistent cloaca.

Unlike classic exstrophy, covered bladder exstrophy is a rare variant characterized by a cycling bladder and intact abdominal wall. We present a case of covered bladder exstrophy diagnosed prenatally and associated persistent cloaca (PC) noted only after delivery. This case report demonstrates that prenatal diagnosis of covered bladder exstrophy is possible and PC can present without any abdominal cysts, bowel, or renal findings. Covered bladder exstrophy should be considered in the differential of cystic protrusion of the bladder to the abdominal wall.

Repair of a traumatic cloaca after obstetric anal sphincter injury.

INTRODUCTION AND HYPOTHESIS: Unsuccessful primary repair of fourth-degree obstetric trauma can lead to permanent communication between the rectum and the vagina, which, in association with full-thickness anal sphincter defects, is characterized by complete fecal incontinence and severe impairment of quality of life. The aim of this video is to serve as a tutorial for repair. METHODS: A 27-year-old woman who developed a full-thickness recto-vaginal defect extended from the perineum to the upper third of the vagina has been managed through layered surgical repair without flaps. RESULTS: Anatom

Chronic urogenital sinus expansion in reconstruction of high persistent cloaca.

OBJECTIVE: Soft tissue expansion is a technique useful in reconstruction when a shortage of tissue exists. This study presents an investigation on using chronic balloon expansion in the urethral and vaginal reconstruction of high persistent cloaca. METHODS: The common channel was expanded by a balloon, which was progressively inflated over 3-4 weeks at a pressure of 30-50 mmHg until an adequate amount of tissue was obtained. The expanded channel was longitudinally split into anterior and posterior segments: the former was approximated to form a neourethra in continuity with the reconstructed

Long-term urological outcome of patients presenting with persistent cloaca.

PURPOSE: Persistent cloaca is a complex malformation with variable presentation that remains a difficult reconstructive challenge. Previous reviews have concentrated on surgical technique, and data on long-term outcome are scarce. We evaluate long-term outcome and when possible correlate outcome with anatomy at presentation. MATERIALS AND METHODS: The records and radiographs of 64 patients 0.5 to 25 years old with persistent cloaca treated at 1 institution from 1975 to 2000 were retrospectively reviewed to determine the outcome for urinary and fecal continence. Outcome data were available in
