OBJECTIVE: Identify variables associated with paediatric access to cochlear implants (CIs).
DESIGN: Part 1. Trends over time for age at CI surgery (N = 802) and age at hearing aid (HA) fitting (n = 487) were examined with regard to periods before, during, and after newborn hearing screening (NHS). Part 2. Demographic factors were explored for 417 children implanted under 3 years of age. Part 3. Pre-implant steps for the first 20 children to receive CIs under 12 months were examined.
RESULTS: Part 1. Age at HA fitting and CI surgery reduced over time, and were associated with NHS implementati

IMPORTANCE: Surgeons should understand the potential long-term complications in patients who undergo cochlear implantation (CI) and should be able to facilitate counseling.
OBJECTIVES: To review the delayed complications after CI and some of the management plans used to treat these complications.
EVIDENCE ACQUISITION: The PubMed and OVID databases were searched for articles published from January 1, 2003, through December 31, 2013, using the search terms cochlear implant and complications. Seven hundred sixty-six articles were identified and searched for reports of delayed complications (>3 …

IMPORTANCE: Though the effects of cochlear implantation (CI) on patients with hearing loss has been well described, its effect on the auditory and vestibular symptoms in patients with Ménière's disease (MD) has not been firmly established.
OBJECTIVES: To determine whether CI in patients with MD improves hearing performance as measured by standard post-CI audiometric testing and to describe the impact of CI on subjective auditory and vestibular quality-of-life measures.
DESIGN, SETTING, AND PARTICIPANTS: Retrospective review of all patients with MD 18 years or older who met diagnostic criteri…

OBJECTIVES: To report patient outcomes after transcanal endoscopic cochlear implantation for sensorineural hearing loss.
PATIENTS: Males and female subjects more than 7 years old with sensorineural hearing loss (confirmed with audiologic studies) were selected. CT imaging was used to rule out any anatomic anomalies of the temporal bone. Smaller pediatric patients whose canal might still be growing and who might have a higher incidence of otitis media and otitis externa were excluded. Obese adults whose canal skin was too thick to allow good access down the ear canal were also excluded.

OBJECTIVES: The objective was to describe a case of bilateral cochlear implantation in a 59-year-old man with hearing and visual impairment due to Refsum's disease.
METHOD: A retrospective case review was performed.
RESULTS: After cochlear implantation, the patient demonstrated much improved audiometric performance and reported improved sound localization.
CONCLUSIONS: Bilateral cochlear implantation is an important strategy in the improvement of hearing and quality of life in individuals with Refsum's disease.

INTRODUCTION: Cochlear implantation is a safe surgery for restoration of hearing in profoundly deaf candidates. Profound deafness may at times, manifest as a part of a syndrome associated with cardiac anomalies. Cardiac co-morbidities may influence cochlear implantation in a spectrum of ways from minor intra operative issues to major life threatening complications. Issues related to pre operative, intra operative and post operative care needs to be addressed by an efficient in house cardiologist.
OBJECTIVES: Our retrospective study was aimed at analyzing the various cardiac co-morbidities enc…

OBJECTIVES: To analyse complications and outcome of cochlear implant (CI) treatment in seniors receiving CIs during a 10-year period.
METHODS: A total of 28 patients, 79 years or older (mean age 81.6 years), were evaluated and compared with a younger group of 76 patients, 20-60 years old (mean age 48.9 years). A retrospective study of the patients' records was performed. Data on per- and post-operative complications, pre- and post-operative speech perception, estimated cognitive skills, and social situation was extracted. A subjective score was assessed and correlated with post-operative perf

OBJECTIVES/HYPOTHESIS: Compared to immunocompetent patients, organ transplant recipients receiving immunosuppressant medications may experience higher rates of postoperative complications. This study was designed to retrospectively review the outcomes of cochlear implantation among organ transplant patients.
STUDY DESIGN: Retrospective case series.
METHODS: Five organ transplant patients received seven cochlear implantations at the University of Miami Ear Institute from January 1, 1992, until August 31, 2005. Inpatient and outpatient records were analyzed to identify healing problems, wound …

Refsum's disease is a rare disorder characterized by accumulation of phytanic acid in plasma and tissues. The main clinical features are retinitis pigmentosa, cerebellar ataxia and chronic polyneuropathy. Sensorineural hearing loss of varying severity can be an additional feature. This study reports our experience with two patients with Refsum's disease who underwent successful cochlear implantation. Both patients had good audiological outcomes after implantation and were overall very satisfied. Bilateral implantation should be considered with patients with severe dual sensory loss. One patien…

The otosclerotic process commonly involves the otic capsule and may cause quite widespread demineralisation which leads to a progressive and often profound bilateral sensorineural hearing loss. In this situation cochlear implantation may be the only effective treatment. This chapter considers the pathology of this hearing loss, the effects of cochlear obliteration on implantation, and the effects of demineralisation of the otic capsule on placement of the electrode and nonauditory stimulation. A study is reported from 4 cochlear implant centres in the UK and the Netherlands of 53 patients with…

OBJECTIVES: With the application of universal newborn hearing screening programs, a large pool of newly identified deaf infants has been identified. The benefits of early intervention with cochlear implants (CI) is being explored. Mounting evidence suggests that age at implantation is a strong predictor of language outcomes. However, new behavioral procedures are needed to measure speech and language skills during infancy. Also, procedures are needed to analyze the speech input to young CI recipients.
STUDY DESIGN: Cohort-sequential.
METHODS: Thirteen infants with profound hearing loss who w

OBJECTIVE: To evaluate the symptoms leading to diagnosis and the quality of rehabilitation after cochlear implantation in Usher syndrome.
STUDY DESIGN: Retrospective cohort study.
SETTING: ENT department of a tertiary referral hospital.
PATIENTS: Among 210 patients given an implantation in the Ear, Nose, and Throat department, 185 were congenitally deaf and 13 had Usher syndrome (7.0%). Five had a family history of Usher, and eight were sporadic cases. Eleven cases were Usher type I, one was Usher type III, and one was not classified. The age at implantation ranged from 18 months to 44 year…

Profoundly deaf patients with chronic suppurative otitis media have been contraindicated for cochlear implantation in the past. Complications such as infection of the radical cavity, fat necrosis, skin flap problems, change in electrode position and cholesteatoma have occurred in subjects with radical cavities. The aim is to create a dry, self-cleansing, infection-free cavity. This is essential in patients about to receive a cochlear implant, as infection may be introduced into the cochlea at the implantation site and destroy any remaining neural elements. The aim of this paper was to show tha…

In four cases implantation was performed concomitantly with subtotal petrosectomy, while the remaining case required a two-stage procedure. No complications occurred. Superficial siderosis is a rare central nervous system disorder characterized by deafness, ataxia, and pyramidal signs. The hearing loss is believed to be predominantly neural and is usually progressive and bilateral. Careful assessment is therefore necessary to determine the best approach to hearing rehabilitation. A case is presented of superficial siderosis in a young woman who has benefitted significantly from cochlear implantation using the Nucleus device.
