Mount Everest and Makalu cold injury amputation: 40 years on.

Freezing cold injuries (frostbite) of the extremities are a common injury among alpinists participating in high altitude expeditions, particularly during inclement weather conditions. Anecdotally, a digit that has suffered frostbite may be at greater risk to future cold injuries. In this case study, we profile a 62-year-old elite alpinist who suffered multiple digit amputations on both his hands and foot after historic summit attempts on Makalu (8481 m) and Mt. Everest (8848 m) in 1974-1979. We describe the clinical treatment he received at that time, and follow up his case 40 years after the

The hepatoprotective effect of sodium nitrite on cold ischemia-reperfusion injury.

Liver ischemia-reperfusion injury is a major cause of primary graft non-function or initial function failure post-transplantation. In this study, we examined the effects of sodium nitrite supplementation on liver IRI in either Lactated Ringer's (LR) solution or University of Wisconsin (UW) solution. The syngeneic recipients of liver grafts were also treated with or without nitrite by intra-peritoneal injection. Liver AST and LDH release were significantly reduced in both nitrite-supplemented LR and UW preservation solutions compared to their controls. The protective effect of nitrite was more

Heat and cold: what does the environment do to marathon injury?

The medical work load seems to increase both with heat and humidity, and with cold and rainy conditions. Heat tolerance during exercise is variable and heat intolerance may contribute to collapse and increase medical encounters. Exposure to cold, wet conditions results in increasing incidence of hypothermia in exhausted marathon runners. Finish-line encounters and course dropouts increase as conditions cool and warm away from the most advantageous conditions in the 4.4-15 degrees C (40-59 degrees F) wet bulb globe temperature (WBGT) range. The risk of requiring medical attention and not finish

Long-term follow-up evaluation of cold sensitivity following nerve injury.

This article reports the long-term follow-up evaluation (mean, 10 years) findings of 50 patients with upper-extremity peripheral nerve injuries. The most common persistent symptom was cold sensitivity (n = 38), and 33 of 38 patients rated its intensity as moderate or severe. Cold sensitivity typically developed within months after initial injury and resolved in only 2 patients. No significant difference in the incidence of cold sensitivity was found between patients with subjectively normal hand sensation (7 of 11) and those who reported abnormal hand sensibility (31 of 37). A cold sensitivity

Preventability of neonatal cold injury and its contribution to neonatal mortality.

When the body temperature of a small neonate falls below 35 degrees C, lassitude can be noted; severe derangements of cardiovascular, renal, hepatic, immunological, and hematological systems may also occur depending in part on the duration and severity of hypothermia. Diagnosis requires a low-reading thermometer, lacking which the diagnosis can be suspected, but most often is missed. Fatal cases of diagnosed cold injury commonly have terminal pneumonia or sepsis. Prevention involves identification and home visits to high-risk infants; intensive care of those with the diagnosis at Soroka Hospit…

Cell release during perfusion reflects cold ischemic injury in rat livers.

The global shortage of donor organs has made it crucial to deeply understand and better predict donor liver viability. However, biomarkers that effectively assess viability of marginal grafts for organ transplantation are currently lacking. Here, we showed that hepatocytes, sinusoidal endothelial, stellate, and liver-specific immune cells were released into perfusates from Lewis rat livers as a result of cold ischemia and machine perfusion. Perfusate comparison analysis of fresh livers and cold ischemic livers showed that the released cell profiles were significantly altered by the duration of

Nonfreezing Cold Injuries Among Long-Distance Polar Rowers.

INTRODUCTION: Nonfreezing cold injury (NFCI) is a peripheral cold injury that occurs when the extremities are exposed to cold temperatures, at or near the freezing point, for sustained periods of time (48-96 h at temperatures of usually around 0 to 6°C with associated wind chill). Although NFCI often goes unreported and may be underdiagnosed, it is a cause of significant morbidity in those working in cold conditions, particularly those in the military. Thus, further research into the prevention, recognition, and treatment of NFCI is warranted. METHODS: The height, body weight, and body compos…

Epidemiology of U.S. Army cold weather injuries, 1980-1999.

BACKGROUND: Cold weather injuries (CWI) are of great military concern due to their wide-ranging impact on military readiness. Previous short-term studies have identified CWI to be more prevalent in African-Americans, infantrymen, and lower-ranking soldiers. The purpose of this cross-sectional study was to determine the occurrence of CWI hospitalizations in the U.S. Army from 1980 to 1999, and to identify possible trends, high-risk groups and/or activities. METHODS: The U.S. Army Research Institute of Environmental Medicine Total Army Injury and Health Outcomes Database was searched for hospit

The pathogenesis of non-freezing cold nerve injury. Observations in the rat.

Non-freezing cold nerve injury is uncommon in civilian practice, but may reach epidemic proportions in war zones. Studied since the time of Hippocrates, its aetiology has remained elusive. We sought to replicate experimentally, a peripheral nerve cold temperature gradient, since this has been emphasized in clinical descriptions. Our observations, in the rat, of the vasa nervorum show that cold-induced intravascular aggregation is followed by a 'no-reflow' phenomenon which culminates in endothelial damage and delayed thrombotic occlusion.

Frostbite and other cold injuries in the heroic age of Antarctic exploration.

Frostbite and other cold injuries on the early polar expeditions were common. This paper explains how frostbite was described, prevented, and treated on the Antarctic expeditions of the heroic age, comparing them with modern recommendations. Nonfreezing cold injury probably also occurred but was not differentiated from frostbite, and chilblains were also described.

Skin microcirculation in patients with sequelae from local cold injuries.

The microcirculation in the skin was assessed in 31 patients with sequelae from local cold injuries (LCI) in the extremities. All patients reported cold intolerance 3-4 years after the primary cold injury, which they sustained during military service. The used methods were laser Doppler fluxmetry, transcutaneous oxygen tension (TCpO2) and vital capillaroscopy. Neurovascular reflexes were stimulated by deep inspiration, digital cuff occlusion of venous and arterial circulation, neck cooling with an ice bag and water immersion at 5 and 15 degrees C. Unaffected lower or upper extremities were als…

Pharmacologic investigation of the mechanism underlying cold allodynia using a new cold plate procedure in rats with chronic constriction injuries.

Cold allodynia is a frequent clinical symptom of patients with neuropathic pain. Despite numerous studies of cold allodynia, using animal models of neuropathic pain, little is known about its underlying mechanisms. This study was performed to establish a method for the pharmacologic evaluation of cold allodynia using several analgesics in a chronic constriction injury (CCI) rat model of neuropathic pain. Compared with the results obtained before the CCI operation, the CCI rats placed on a cork plate at 20 degrees C exhibited a slight change in the paw withdrawal latency because of the mechanic

Warm ischemic injury is reflected in the release of injury markers during cold preservation of the human liver.

BACKGROUND: Liver transplantation plays a pivotal role in the treatment of patients with end-stage liver disease. Despite excellent outcomes, the field is strained by a severe shortage of viable liver grafts. To meet high demands, attempts are made to increase the use of suboptimal livers by both pretransplant recovery and assessment of donor livers. Here we aim to assess hepatic injury in the measurement of routine markers in the post-ischemic flush effluent of discarded human liver with a wide warm ischemic range. METHODS: Six human livers discarded for transplantation with variable warm an

Early Cold Stored Platelet Transfusion Following Severe Injury: A Randomized Clinical Trial.

OBJECTIVE: To determine the feasibility, efficacy, and safety of early cold stored platelet transfusion compared with standard care resuscitation in patients with hemorrhagic shock. BACKGROUND: Data demonstrating the safety and efficacy of early cold stored platelet transfusion are lacking following severe injury. METHODS: A phase 2, multicenter, randomized, open label, clinical trial was performed at 5 US trauma centers. Injured patients at risk of large volume blood transfusion and the need for hemorrhage control procedures were enrolled and randomized. The intervention was the early trans

Cold intolerance and neuropathic pain after peripheral nerve injury in upper extremity.

Cold intolerance and pain can be a substantial problem in patients with peripheral nerve injury. We aimed at investigating the relationships among sensory recovery, cold intolerance, and neuropathic pain in patients affected by upper limb peripheral nerve injury (Sunderland type V) treated with microsurgical repair, followed by early sensory re-education. In a cross-sectional clinical study, 100 patients (male/female 81/19; age 40.5 ± 14.8 years and follow-up 17 ± 5 months, mean ± SD), with microsurgical nerve repair and reconstruction in the upper extremity and subsequent early sensory re-edu
