Reality confusion in spontaneous confabulation.

A woman produced spontaneous confabulations after rupture of an anterior communicating artery aneurysm. She confused currently irrelevant with currently relevant information in implicit memory; confabulations about people concerned only new acquaintances; false reality could be induced by an intensive 5-minute discussion; and in a recognition task, she confused false repetitions in another modality with real item repetitions. The findings support the theory that the defect causing spontaneous confabulation precedes conscious memory processing.

[Confusion: more than just dementia].

Dementia is a frequent cause of mental or cognitive impairment in elderly patients. Sometimes elderly patients are unjustly diagnosed as being demented. Two such cases are reported in this article. Both patients were referred to a geriatric outpatient clinic with a diagnosis of dementia. The first patient had auditory hallucinations as a feature of a presumed paraneoplastic syndrome. The other patient experienced auditory agnosia as a result of an as yet unrecognized stroke. To recognize supposed dementia a thorough anamnesis and clinical evaluation are a prerequisite and often indicative of t

Human monkeypox: confusion with chickenpox.

Human monkeypox is a zoonosis occurring sporadically in the tropical rain forest of western and central Africa. The exact incidence and geographical distribution are unknown, since many cases are not recognized. Special surveillance was established in three regions in Zaire in 1981 that led to a substantial increase in reported cases. The question arose as to the possibility that clinical diagnostic errors cause some cases of monkeypox to be misdiagnosed as other eruptive diseases. This paper presents the results of a study assessing the extent of and reasons for these clinical diagnostic erro

Is markedness a confused concept?

It is well known that, across languages, certain phonological features are more frequent than others. But whether these facts reflect abstract universal markedness constraints or functional pressures (auditory and articulatory difficulties and lexical frequency) is unknown. Romani, Galuzzi, Guariglia, and Goslin (2017) report that the putative markedness of phonological features captures their order of acquisition and their propensity to elicit errors in patients with an apraxia of speech (but not in phonological aphasia). The authors believe these results challenge the existence of abstract m

Dealing with confusion in the elderly.

Elderly patients with established brain failure present the family doctor with problems of longterm management for which there are, at best, limited pharmacologic solutions. An organized plan of management developed jointly with the significant other(s) will help achieve treatment objectives. By assessing the patient's physical and mental function, it is possible to work out appropriate changes in the living environment and behavioral interventions which will help retard deterioration, maintain and conserve function and reduce the confusion and distress felt by all parties. Though initially it…

Crusted, Chronic, and Confusing.

Chronic ulcerative lesions initially treated as bacterial cellulitis were ultimately diagnosed as cutaneous leishmaniasis. This case highlights diagnostic pitfalls in persistent ulcers, the importance of considering endemic exposures, and the role of species identification in guiding therapy.

A Case of Confusion and Bradycardia.

Editor's note: This is the next installment in a series on electrocardiogram (ECG) interpretation. Nurses in all settings should know the basics, as medications and physiological changes can cause cardiac arrhythmias. Each article will start with a brief case scenario and an ECG strip and then take you step by step through analyzing the heart rhythm.

HRI-confusion: A multimodal dataset for modelling and detecting user confusion in situated human-robot interaction.

The dataset was collected from 28 participants (17 female, 9 male, and 1 non-binary) for a study aimed at modelling and detecting user social behaviours with different confusion states in task-oriented situated human-robot interaction (HRI). The dataset consists of user facial body video recordings synchronised with user speech across three designed experiment scenarios (Tasks 1 - 3). Each experiment lasted approximately one hour per participant. The videos are segmented into individual clips corresponding to specific experimental conversations under predefined conditions: general confusion an…
