This study investigates the pathophysiological process of healed perforated corneal ulcers (HPCUs) in humans. All subjects underwent keratoplasty due to opacities or leakage from HPCUs. Half of each specimen was fixed with 4% glutaraldehyde for transmission electron microscope (TEM) examination. The other half was fixed in 10% formaldehyde for immunofluorescence (IF) examination. TEM identified layered structures with two cell types (polygonal and elongated) connected by gap or adherent junctions during early stage of healing. Both apoptotic and mitotic changes were found in both types of cell

The current treatments for the management of corneal and scleral perforations include sutures and adhesives. While sutures are invasive, induce astigmatism and carry a risk of infection, cyanoacrylate glues are toxic, proinflammatory and form an opaque and rough surface that precludes vision. Consequently, the clinical need for a fast curing and strong tissue adhesive with minimised cytotoxicity and host inflammation remains unmet. In this paper, we engineer a gelatine methacryloyl (GelMA) adhesive that can be crosslinked in situ within 2 min using UV or visible light and a riboflavin (RF)/sod

A 10-year-old boy presented with a corneal perforation of the right eye caused by a fish hook injury while watching a friend fish. Surgical removal of the fish hook was performed. Under general anesthesia, the shank of the fish hook was cut and the hook was advanced and released. The observation of fly fishing is accompanied by risk similar to participation. Management is guided by the peculiarities of the injury.

166 patients of corneal perforation from HSK, suppurative corneal ulcer and chemical or thermal burn were treated by penetrating keratoplasty. In the group of HSK, 69.8% of the grafts remained transparent and vision was restored to 0.05-1.0 in 81 cases (69.8); in the group of suppurative corneal ulcer, 40% of the grafts were transparent and vision was restored to 0.05 or better in 15 cases (37.5%); while in the group of burns, the grafts were clear only in 20% and vision was 0.05-0.6 in 3 cases (30%). The complications and causes for failure were similar to those in patients without perforatio…

A 10-year-old boy with Cockayne's syndrome had an aseptic perforation centered in a corneal opacity of the right eye. Exposure keratitis, which was caused by anhidrosis, loss of blinking, and lack of Bell's phenomenon, was considered responsible for this condition. A therapeutic soft contact lens was applied to heal the corneal wound.

Three cases of corneal perforation which were treated with tissue adhesive (Histoacryl) are reported. The first case showed free fistulation after perforating keratoplasty, whilst the second had a trophic ulcer and the third had a traumatic corneal laceration with loss of corneal tissue. Treatment was successful in all three cases. The surgical technique, results and possible complications are mentioned.

The current indications for and method of application of tissue adhesive are described. A method for an expanded utilisation of tissue adhesive is discussed. To study the effect of the introduction of tissue adhesive to The Wilmer Institute in 1974, the records of 104 consecutive non-traumatic corneal perforations or descemetoceles admitted to The Wilmer Institute from 1960 to 1980 were assessed retrospectively with follow-up being obtained from records or from referral physicians for 87 of these perforations. Forty-nine percent of the cases were caused by bacterial corneal ulcers, 13% by expo…

PURPOSE: Corneal perforation is a rare, vision-threatening complication of ocular graft-versus-host disease (GVHD) and is not well understood. Our objective was to examine the clinical disease course and histopathologic correlation in patients who progressed to this outcome.
METHODS: This study is a retrospective case series from four academic centers in the United States. All patients received a hematopoietic stem cell transplant (HSCT) prior to developing ocular GVHD. Variables of interest included patient demographics, time interval between HSCT and ocular events, visual acuity throughout …

INTRODUCTION: Ocular rosacea is a multifactorial disease. Its pathophysiology remains unclear. The ocular manifestations of rosacea are not specific and can range from simple blepharoconjunctivitis to sight-threatening such as corneal perforation.
CASE REPORT: We report the case of a 10-year-old child who presented with a red painful right eye. Based on the clinical findings, we concluded that she had a corneal perforation on ocular rosacea. She benefited from an ipsilateral lamellar autokeratoplasty by lamellar autograft. The evolution was marked by a good healing and a good visual recovery …

PURPOSE: To describe the outcomes of tuck-in tenon patch graft (TPG) in the management of corneal perforation up to 5-mm size.
METHODS: Thirty-one cases of sterile corneal perforation (3-5 mm) underwent autologous TPG. The technique included, freshening of the edges, measuring the size of defect, creating a 360-degree stromal pocket around the perforation margin, harvesting the tenon graft followed by tucking into the stromal pocket, and application of cyanoacrylate glue or suturing the graft using 10-0 monofilament nylon suture. The main outcome measure was rate of healing (percentage of cas…

INTRODUCTION: Lamellar Ichthyosis is an autosomal recessive, inherited skin disorder characterized by thickening, fissuring and scaling of the skin.
OBJECTIVE: To report a case of lamellar icthyosis and highlight the importance of monitoring corneal health in these patients.
CASE: We report a rare case of bilateral spontaneous corneal perforation in a patient with lamellar ichthyosis .The patient presented with complaints of diminution of vision, foreign body sensation, watering and discharge in both of his eyes for the last three months followed by pain and redness for one week. Visual acui…

PURPOSE: Although ocular manifestations are common among patients with Behçet's disease, corneal perforation has not been reported in the literature. We report an unusual case of Behçet's disease with corneal perforation as the main ocular involvement.
METHODS/RESULTS: An elderly Chinese patient was referred for eye examination because of clinical suspicion of Behçet's disease. Eye examination showed evidence of long-standing autoimmune disease of the eye, mild iritis and a corneal perforation in the left eye. Despite evidence of chronicity and corneal perforation, the patient was asymptomati…

The authors have recently treated a case of Parinaud's oculoglandular syndrome due to Chlamydia trachomatis serotype L2, a causative agent of lymphogranuloma venereum (LGV). The ocular manifestations included a mixed papillary-follicular conjunctivitis with fleshy superior limbal lesions in both eyes. A superior marginal corneal perforation requiring a therapeutic corneal graft was present in the right eye. The patient had vaginitis, inguinal lymphadenopathy, a history of Sjögren's syndrome, and seropositivity to human immunodeficiency virus (HIV). The ocular disease resolved completely after …
