Human inherited complete STAT2 deficiency underlies inflammatory viral diseases.

STAT2 is a transcription factor activated by type I and III IFNs. We report 23 patients with loss-of-function variants causing autosomal recessive (AR) complete STAT2 deficiency. Both cells transfected with mutant STAT2 alleles and the patients' cells displayed impaired expression of IFN-stimulated genes and impaired control of in vitro viral infections. Clinical manifestations from early childhood onward included severe adverse reaction to live attenuated viral vaccines (LAV) and severe viral infections, particularly critical influenza pneumonia, critical COVID-19 pneumonia, and herpes simple

Iron deficiency, depression, and fatigue in inflammatory bowel diseases.

BACKGROUND: Iron deficiency and anemia are common findings in IBD. Treatment of anemia improves quality of life. Neurological symptoms like depression or anxiety are also common in IBD; however, their relationship with ID has not been studied in detail. METHODS: Prospective, single center, non-interventional trial in an IBD cohort (n = 98), which is generally at risk for ID. Quality of sleep (using the Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale, and Insomnia Severity Index) and the presence of fatigue (Piper fatigue scale), depression (Self-rating Depression Scale [SDS]) or anxi

Humoral deficiency in three paediatric patients with genetic diseases.

BACKGROUND: Primary immunodeficiencies (PID) represent a heterogeneous group of genetic disorders characterised by poor or absent function in one or more components of the immune system. Humoral or antibody immunodeficiencies are the most common form of PID, of which common variable immunodeficiency (CVID) is the most frequent symptomatic form. CVID is usually characterised by hypogammaglobulinaemia with poor antibody specificity, and an increased susceptibility to infections, autoimmunity and lymphoproliferation. Fewer than 10% of CVID patients have a known monogenic basis. Several chromosoma

Human mitochondrial diseases associated with tRNA wobble modification deficiency.

BACKGROUND: The precise mechanisms involved in the pathogenesis of hypersensitivity pneumonitis (HP) have not been identified. HP is characterized by inflammatory lymphocytic alveolitis and a remarkable increase in T-lymphocytes detected in bronchoalveolar lavage fluid (BALF). It is suggested that both CD4+ and CD8+ T cells may contribute to the pathogenesis of HP. Experiments on animal models suggest that cell mediated immunity (CMI) is more important for the pathogenesis of HP than complex-mediated immunity, but the relationship between the subsets of BALF lymphocytes and humoral or cell-med

Focal cytochrome c oxidase deficiency in various neuromuscular diseases.

To determine whether focal cytochrome c oxidase (CCO) deficiency characterized by scattered fibers with absent CCO activity among normal fibers was a specific finding for mitochondrial myopathies, we studied 389 muscle biopsies from various neuromuscular diseases other than mitochondrial myopathies. Focal CCO deficiency was found in 14 biopsies: 5 of 26 patients with myotonic dystrophy, 3 of 19 with nemaline myopathy, 1 of 7 with distal myopathy with rimmed vacuole formation, 3 of 22 with limb-girdle muscular dystrophy, 1 of 9 with amyotrophic lateral sclerosis, one of 79 with Duchenne muscula

Endocrinological studies in ataxiatelangiectasia and other immunological deficiency diseases.

Endocrinological and immunological studies were performed in five patients with ataxia-telangiectasia, two patients with sex-linked non-lymphopenic hypogammaglobulinaemia, one patient with sex-linked lymphopenic hypogammaglobulinaemia, and two patients with hypopituitary dwarfism. One of five patients with ataxia-telangiectasia had deficient growth hormone responses to insulin-induced hypoglycaemia but normal responses to arginine infusion. One pubertal ataxia-telangiectasia patient had increased gonadotropin secretion. Patients with hypopituitary dwarfism failed to respond to insulin-induced

Global Burden of Diseases Associated With Iron Deficiency: GBD 2021.

BACKGROUND: Iron deficiency is a major global public health concern associated with various adverse outcomes. METHODS: This study utilized the Global Burden of Disease Study 2021 (GBD 2021) to analyze the contemporary burden of iron deficiency-associated diseases. We conducted an epidemiological analysis using Bayesian age-period-cohort methods for forecasting, decomposition analysis to assess the impact of aging, population growth, and epidemiological shifts, and slope/concentration indices to assess health inequalities. RESULTS: Between 1990 and 2021, disability-adjusted life years (DALYs)

[Vitamin D deficiency as a risk factor for cerebrovascular diseases].

Vitamin D is a lipid-soluble steroid hormone whose role has been well-known in calcium and bone metabolism for a long time. In the last decades, vitamin D deficiency, which affects approximately 24% to 40% of the population, has been linked to the pathogenesis of cardiovascular disorders including cerebrovascular diseases, one of the leading causes of death worldwide. According to epidemiological studies, vitamin D deficiency is associated with an increased risk and more severe outcome of ischemic stroke, vascular cognitive impairment, and cerebral cavernous malformations. Although several con

Anemia and iron deficiency in pregnant women with rheumatic diseases.

INTRODUCTION: Anemia and iron deficiency are the most common pathologies in pregnancy and associated with adverse pregnancy outcome. As patients with rheumatic diseases are also at high risk for anemia, we aimed to investigate the frequency of anemia and iron deficiency during pregnancy in this group and whether anemia is a risk factor for adverse maternal or child outcome. METHODS: We analyzed 368 pregnancies from a German registry for pregnancies in patients with rheumatic diseases (TURIRE) from 2014-2022. Anemia and iron deficiency were defined according to the World Health Organization. M

Wernicke-Korsakoff syndrome and other diseases associated with thyamine deficiency.

Wernicke-Korsakoff syndrome is the best known consequence of thiamine deficiency, frequently associated with patients with chronic and excessive alcohol consumption, but it can be produced by any cause that produces thiamine deficiency. The disease is underdiagnosed so it is essential to have a high clinical suspicion, mainly in patients who do not have alcohol consumption as a risk factor. For this, the diagnosis continues to be eminently clinical, with the difficulty of high clinical variability. Complementary tests are used to support the diagnosis and rule out other causes that can produce

Human RIPK1 deficiency causes combined immunodeficiency and inflammatory bowel diseases.

Receptor-interacting serine/threonine-protein kinase 1 (RIPK1) is a critical regulator of cell death and inflammation, but its relevance for human disease pathogenesis remains elusive. Studies of monogenic disorders might provide critical insights into disease mechanisms and therapeutic targeting of RIPK1 for common diseases. Here, we report on eight patients from six unrelated pedigrees with biallelic loss-of-function mutations in RIPK1 presenting with primary immunodeficiency and/or intestinal inflammation. Mutations in RIPK1 were associated with reduced NF-κB activity, defective differentia

Alpha-1-antitrypsin deficiency in Serbian adults with lung diseases.

AIM: Alpha-1-antitrypsin (A1AT) is the main inhibitor of neutrophil elastase, and severe alpha-1-antitrypsin deficiency (A1ATD) is a genetic risk factor for early-onset emphysema. Despite the relatively high prevalence of A1ATD, this condition is frequently underdiagnosed. Our aim was to determine the distribution of the A1ATD phenotypes/alleles in patients with lung diseases as well as in the Serbian population. METHODS: The study included the adults with chronic obstructive pulmonary disease (COPD) (n=348), asthma (n=71), and bronchiectasis (n=35); the control was 1435 healthy blood donors.

[Hygienic aspects of iodine deficiency diseases in the Tomsk Region].

The impact of the content of iodine and other trace elements in the environmental objects of the Tomsk Region on the development of iodine-deficiency diseases was studied in children. Slightly sour soils and low soil humus levels in the Tomsk Region were ascertained to be conducive to low iodine content in soils and plants. As compared with the clarke of iodine in the lithosphere, the soils in the Tomsk Region were marked by low iodine levels, permitting it to be, in terms of endemic goiter, referred to as a biogeochemical province. The soils of rural areas in the Tomsk Region were characteriz…
