[Delay in the diagnosis of McArdle's disease].

We describe the case of a 14-year-old patient with fatigability and muscular cramps beginning in the first infancy and started by physical exercise. The clinical record, the increase of creatinine-kinase without clinical or biological signs of inflammation and the electromyographic normality, suggested the presence of this disease, which was confirmed through microscopic studies (optic and electronic) and histochemistry. We review the clinical and biological characteristics, the diagnostic methods for this type of glycogenosis and its differential diagnosis, because we think that the low suspi

Delay in diagnosis of thoracolumbar fractures.

The time interval between the date of trauma and the diagnosis of vertebral column fractures hinders management and increases liability. We have examined the features and implications of this delay. 585 consecutive thoracolumbar fractures (2005-2016), were considered; 382 (65.30%) were males and 203 (34.70%) females. Mean age was 51 yr. Fall from a height (187; 31.97%), simple fall (147; 25.13%) and road accidents (111; 18.97%) were the most frequent causes of trauma. Physical exertion caused 8.38% (N=49). 142 patients (24.27%) were not diagnosed on the injury day (mean = 3.2 days). Delay was

Cryptococcosis-the impact of delay to diagnosis.

Cryptococcosis is a mycotic disease caused by the yeast Cryptococcus sp. It is associated with significant mortality and morbidity, including long-term neurological sequelae. It is not known whether the high morbidity observed is related to a delay in diagnosis. A retrospective chart review of all cryptococcal infections that had been diagnosed in the region between 1997 and 2015 was performed. Twenty-nine cases were identified. Overall mortality rate was 10.3%, with an attributable mortality rate of 6.9%. Forty-five per cent of patients with central nervous system involvement developed long-t

Delays to the diagnosis of cervical dystonia.

The diagnosis of cervical dystonia (CD) is based on physical examination and is therefore reliant on clinician experience. Due to variability of presenting symptoms it may be misdiagnosed, thus delaying the provision of effective treatment. We sought to determine the average time taken to make a diagnosis of CD in our clinical cohort and explore contributing factors to diagnostic delay. Forty-nine patients with a diagnosis of CD attending a movement disorder specialist for treatment completed a questionnaire regarding symptoms and clinical interactions at onset and diagnosis. The mean time fro

Delay in diagnosis of breast cancer.

OBJECTIVES: To assess the consequences of physician delay in the diagnosis of breast cancer by comparing stage, treatment, and outcome of patients with and without delay, and to identify patient characteristics that may make diagnosis more difficult. SUMMARY BACKGROUND INFORMATION: Delay in diagnosis of breast cancer is the most common clinical scenario resulting in malpractice litigation. METHODS: The records of 1014 patients were reviewed and the events preceding the diagnosis were reconstructed. Accurate assessment of the physician delay in diagnosis could be made for 606 patients, 51 (8%)

Delayed diagnosis of a glenohumeral joint dislocation in a child with developmental delay.

Glenohumeral joint dislocations rarely occur in children. Those that have been reported have all been reduced in a closed fashion. With the exception of one, there have not been any comorbidities in the children that would have led to a delay in diagnosis. To the best of our knowledge, we report the first case of a child with a delayed diagnosis of an anterior dislocation of the glenohumeral joint that required an open reduction. In a child with other medical problems, especially developmental delay, who may have difficulty expressing his symptoms, early detection and intervention are crucial …

Delayed Diagnosis of Primary Sinonasal Mucosal Melanoma.

Primary sinonasal mucosal melanoma (SNMM) is a rare and highly aggressive malignancy that is frequently diagnosed at advanced stages because of its nonspecific presentation. We report the case of a 70-year-old Peruvian woman who presented with a six-month history of recurrent unilateral epistaxis refractory to direct compression, chemical cauterization, and nasal packing, associated with progressive nasal obstruction, facial pain, foul-smelling nasal discharge, and significant weight loss. Nasofibrolaryngoscopy revealed a friable pigmented mass arising from the left nasal cavity. Computed tomo

Delayed Diagnosis of Subungual Acral Lentiginous Melanoma.

We report a 62-year-old, Hispanic male who presented to a rural medical provider who noted dark nail discoloration to the left thumb. Several weeks later, he presented to a rural emergency department with left thumb subungual hematoma and paronychia attributed to recent trauma. After evacuation and oral antibiotics, the provider discussed the prospect of melanoma given presence of longitudinal melanonychia and positive Hutchinson's sign. After 2.5 years, the patient re-presented to the same provider who noted increased pain and scarring of the left thumbnail. Referral to an orthopedic hand sur

Delayed diagnosis of Swyer-James-MacLeod syndrome.

Swyer-James-MacLeod Syndrome is a rare obliterative lung disease typically caused by childhood infection resulting in arrested pulmonary development. Imaging findings include unilateral hyperlucency on chest x-ray, and hyperlucency, hypovascularity and expiratory gas trapping on computed tomography. Recognition of abnormal imaging can lead to earlier diagnosis and institution of appropriate management.

A delayed diagnosis of high-grade spondylolisthesis.

Spondylolysis, or pars defect, occurs in nearly half of children with back pain. Despite the marked prevalence, diagnosis of spondylolysis with spondylolisthesis often is delayed or missed secondary to referred pain and uncharacteristic presentation. This article describes an 8-year-old patient with 15 months of right heel pain who was initially treated by her primary care provider for presumed Sever disease before being referred to orthopedics. After orthopedic consultation, she was diagnosed with a high-grade spondylolisthesis with L5 nerve root compression. Although spondylolysis is an infr

Delayed diagnosis of atraumatic urinary bladder rupture.

Spontaneous rupture of the urinary bladder (SRUB) is a very rare and often missed diagnosis. While the clinical presentation is often non-specific, SRUB is associated with a high mortality rate and therefore warrants swift diagnosis in order to avoid delay in management. Herein, we present a case of SRUB with multiple etiological factors and temporal association with phosphate enema administration.

Delayed Diagnosis of Neck Pain: Eagle Syndrome.

Eagle syndrome is a rare condition characterized by an abnormally elongated styloid process with or without abnormal direction and/or ossification of the styloid ligament. Clinically, it consists of throat and neck pain radiating to the ear. Here, we present the case of a 34-year-old female with the complaint of left-sided neck pain below the ear for the past year. The patient had tried different analgesics after seeing different doctors, but the pain did not resolve. After conducting radiological investigations at the hospital, a diagnosis of Eagle syndrome was made. The patient was treated w

[Reversible Paraplegia - Favorable Outcome After Delayed Diagnosis].

Reversible Paraplegia - Favorable Outcome After Delayed Diagnosis Abstract. A 74-year-old woman was referred for progressive gait disturbances. On presentation, she had a complete paraplegia (wheelchair-bound for 19 months) and bladder sphincter dyssynergia with sensory sacral sparing. Magnetic resonance imaging studies revealed a 24 × 13 × 17 mm intradural mass with compression of the spinal cord and myelomalacia between C6 and Th1. We performed unilateral laminectomies of C6-Th1 and microsurgical resection of a meningioma. Under intensive rehabilitation, the patient regained independent walk…
