Delirium superimposed on dementia versus delirium in the absence of dementia: phenomenological differences.

OBJECTIVE: To examine differences in the phenomenological characteristics of delirium superimposed on dementia compared to those observed in delirious patients without dementia, based on the rating items of the Memorial Delirium Assessment Scale (MDAS). METHODS: We conducted an analysis of a prospectively collected clinical delirium database utilized to record and monitor the care of delirious patients treated at Memorial Sloan-Kettering Cancer Center (MSKCC). Sociodemographic, medical variables, and MDAS total score and individual item ratings were analyzed in respect to differences between …

AI-delirium guard: Predictive modeling of postoperative delirium in elderly surgical patients.

INTRODUCTION: In older patients, postoperative delirium (POD) is a major complication that can result in greater morbidity, longer hospital stays, and higher healthcare expenses. Accurate prediction models for POD can enhance patient outcomes by guiding preventative strategies. This study utilizes advanced machine learning techniques to develop a predictive model for POD using comprehensive perioperative data. METHODS: We examined information from the National Surgical Quality Improvement Program (NSQIP), which included 17,000 patients who were over 65 and undergoing different types of surger…

[Multicomponent programs in delirium prevention and delirium management in older adults : Evidence, guideline recommendations and perspectives on delirium-sensitive care].

Delirium is among the most frequent neuropsychiatric complications in older people. It is associated with functional decline, increased morbidity and mortality, loss of independence, cognitive deterioration and an elevated risk of dementia. Contemporary concepts view delirium as the result of an interplay between predisposing vulnerability factors and acute physiological stressors. Current approaches to delirium prevention and management focus on nonpharmacological multicomponent interventions that combine early mobilization, cognitive reorientation, sleep hygiene, pain management, optimizatio…

Differences in long-term outcomes between ICU patients with persistent delirium, non-persistent delirium and no delirium: A longitudinal cohort study.

PURPOSE: Determine differences in physical, mental and cognitive outcomes 1-year post-ICU between patients with persistent delirium (PD), non-persistent delirium (NPD) and no delirium (ND). MATERIALS AND METHODS: A longitudinal cohort study was performed in adult ICU patients of two hospitals admitted between July 2016-February 2020. Questionnaires on physical, mental and cognitive health, frailty and QoL were completed regarding patients' pre-ICU health status and 1-year post-ICU. Delirium data were from patients' total hospital stay. Patients were divided in PD (≥14 days delirium), NPD (<14…

[Polypharmacy and Delirium in the Elderly].

Polypharmacy and Delirium in the Elderly Abstract: Delirium often occurs in elderly hospitalized patients. Multimorbidity and associated polypharmacy are known risk factors for developing delirium. Moreover, delirium itself often leads to the prescription of additional drugs. This article aims to enlighten the interrelation of delirium and polypharmacy in the context of recent evidence. It also tries to show possibilities of deprescribing.

Delirium secondary to anticholinergics.

We present a case of a young man who developed sudden deterioration in his physical and mental state whilst being treated as an inpatient for substance-induced psychosis. This deterioration was manifested by sudden disorientation, change in behaviour and visual hallucinations. It was only after excluding other potential causes that this presentation was attributed to the regular administration of procyclidine that was being used to counteract the extrapyramidal side effects from antipsychotics. The patient showed a dramatic improvement on stopping procyclidine. This case highlights the importa

Delirium in Palliative Care.

Delirium, a widespread neuropsychiatric disorder in patients with terminal diseases, is associated with increased morbidity and mortality, profoundly impacting patients, their families, and caregivers. Although frequently missed, the effective recognition of delirium demands attention and commitment. Reversibility is frequently not achievable. Non-pharmacological and pharmacological interventions are commonly used but largely unproven. Palliative sedation, although controversial, should be considered for refractory delirium. Psychological assistance should be available to patients and their fa…

Delirium in hospitalised children.

Delirium is a syndrome characterised by an acute and fluctuating alteration in cognition and awareness. It occurs frequently in children with serious medical illness, and is associated with adverse outcomes such as increased length of hospital stay, duration of mechanical ventilation, hospital costs, and mortality. Delirium-especially the hypoactive subtype-is often overlooked by paediatric practitioners, but can be reduced by mitigating risks and effectively managed if detected early. Non-modifiable risk factors of delirium include young age (age <2 years), cognitive or neurological disabilit…

[Etiology and Treatment of Delirium].

Delirium is a potentially life-threating acute disease accompanied by a mortality up to 30 % of all affected patients. It occurs more frequently in older patients ≥ 65 years. Delirium is defined as acute onset of disturbances in attention, awareness and cognition that develops within a short period of time and fluctuates in severity. It can be subdivided in hyperactive, hypoactive and a mixed type. The hypoactive type occurs more frequently in geriatric patients and has the highest mortality of all types. Differential diagnosis of delirium include stroke, central nervous system infection, meta…
