Radiographic examination of dentigerous cysts in the transitional dentition.

OBJECTIVES: To examine radiographically the relationship between the deciduous tooth and dentigerous cyst of the permanent successor during the transitional dentition. METHODS: From a retrospective review of all patients who visited our institution from April 1988 to August 2001, 70 patients under 16 years of age who had histologically confirmed dentigerous cysts that had developed from the central incisor to the second premolar were identified. These 70 patients were investigated using panoramic and periapical radiographs. RESULTS: In most cases (54 cases; 77.1%) the cyst was in the premola…

Dentigerous cysts of inflammatory origin. A clinicopathologic study.

The exact histogenesis of dentigerous cysts remains unknown, but most authors favor a developmental origin from the tooth follicle. The aim of this article is to report a series of 15 dentigerous cysts that we believe to be of inflammatory origin. These inflammatory dentigerous cysts occurred in the first and early part of the second decades of life. Males were affected more frequently, and there did not appear to be any racial predilection. All of the cases involved permanent teeth: premolars in nine cases, canines in four cases, and second molars in two cases. The mandible was affected twice…

Immunohistochemical study of polycystin-1 in dentigerous cysts.

BACKGROUND: The alterations involved in step-wise transformation of a dental follicle to dentigerous cyst (DC) is not clearly known. Primary cilium and its protein have been hypothesized to be associated with DC. Mutation of a ciliary protein, polycystin-1 (PC1) is associated with autosomal dominant polycystic kidney disease. This study was performed to assess the immunohistochemical expression of PC1 between DC and postfunctional follicular tissue (PFFT). MATERIALS AND METHODS: Thirty-one consecutive PFFT and 15 DC formed the study group. The PFFT and DC tissues were stained with antibody ag…

Actinomyces-associated calcifications in a dentigerous cyst of the mandible.

Actinomyces-associated lesions in the jaw, such as radicular cyst and osteomyelitis, have been reported by many authors. The lesions are caused by infection from peripheral sites and can be seen to contain Actinomyces druses on pathologic examination. To our knowledge, no previous reports have described Actinomyces-associated calcification in the jaw, although the lesions in the jaw often include druses. We report here a rare case of Actinomyces-associated calcifications in a dentigerous cyst of the mandible.

Clinicopathologic features of dentigerous cysts in the maxillary sinus.

Dentigerous cyst (DC) in the maxillary sinus is extremely rare. This study evaluated the clinical features, etiologic factors, imaging, treatment, and signs and symptoms of this type of DC. Clinical symptoms, radiographic findings, surgical methods, and histopathologic findings of 4 patients with DCs in the maxillary sinus were evaluated. In addition, we reviewed the literature reports of DCs in the maxillary sinus over the past 40 years. Our 4 patients with DC in the maxillary sinus showed the following results: (1) all the patients were males at first 3 decades of age, (2) swelling and/or ye…

Dentigerous cyst associated with a mesiodens: a case report.

Dentigerous cysts are the second most common odontogenic cysts after radicular cysts and are most commonly seen in association with third molars and maxillary canines. Only 5% of dentigerous cysts involve supernumerary teeth, of which mesiodens is the most frequent type. This paper presents a case of dentigerous cyst associated with a mesiodens that caused a painless swelling in the upper lip of an 18-year-old female. The patient was treated surgically by enucleation of total cyst and surgical extraction of mesiodens under local anesthesia.

[Histomorphometry of dentigerous cysts in children, associated or not with formocresol].

Literature reports appeared in the last years suggested that some type of dentigerous cyst (DC) in children would initiate by teh action of other factors than developmental and at different stages of dental sac growth. Periodontitis and pulp therapy with formocresol (FC) have been suggested as aeteological factors, both promoting DC in children by irritation of the underlaying dental sacs of premolar. The purpose of this study is to analyse the pathological changes in DC of patients aged 0 to 15 years, and to compare the findings according to the anatomical site of DC and the existence of prev…

Unusual Imaging Features of Dentigerous Cyst: A Case Report.

Dentigerous cysts (DC) are cystic lesions radiographically represented by a well-defined unilocular radiolucent area involving an impacted tooth crown. We present an unusual radiographic feature of dentigerous cyst related to the impacted mandibular right second molar, in a 16-year-old patient, which suggested an ameloblastoma or odontogenic keratocyst (OKC) because of its multilocular appearance seen on the panoramic radiography. A multi-slice computed tomography (MSCT), however, revealed a unilocular lesion without septations, with an attenuation coefficient from 3.9 to 22.9 HU suggesting a …

Fascin Expression in Ameloblastoma, Odontogenic Keratocyst and Dentigerous Cyst.

OBJECTIVE: The purpose of this study was to assess and compare fascin expression in 4 lesions which differ in aggressiveness: odontogenic keratocyst, dentigerous cyst and two types of ameloblastoma (solid and unicystic), and to find out whether fascin expression is associated with aggressiveness of these lesions or not. MATERIAL AND METHOD: Nine solid ameloblastomas , 12 unicystic ameloblastomas, 13 odontogenic keratocyst and 12 dentigerous cyst were assessed in this study. The slides were examined at x400 magnification. Finally the lesions were divided into two groups based on microscopic ex…

Dentigerous cyst associated with ectopically impacted maxillary second premolar.

Dentigerous cysts are benign odontogenic cysts that are accompanying with the crown of permanent teeth. They are typically single in incidence and are situated in the mandible. They are usually associated with impacted mandibular third molar and maxillary canines. Here we report a very rare presentation of dentigerous cyst in anterior maxilla accompanying with ectopically impacted maxillary second premolar. The cyst was enucleated along with extraction of premolar.

Appearance can be deceptive: Dentigerous cyst crossing the midline.

Dentigerous cyst is a developmental odontogenic cyst, which develops by accumulation of fluid between reduced enamel epithelium and the tooth crown of an unerupted tooth. Dentigerous cysts are usually solitary, slow growing, asymptomatic lesions that are incidentally found during routine radiographs They most frequently involve the mandibular third molar followed in order of frequency by the maxillary canine, mandibular second pre-molar and maxillary third molar. Occasionally, these cysts become painful when infected causing swelling and erythema. The cyst is usually small, however, when large…

Extensive dentigerous cyst associated with a mesiodens: CT findings.

The most common of the supernumerary teeth in humans are mesiodentes, which arise in the midline of the maxilla between the central incisors. The most common pathologic findings associated with a mesiodens are retention of the adjacent incisors, malposition, and diastema. The development of a dentigerous cyst in association with an impacted mesiodens is relatively uncommon. We report the case of a 35-year-old man with an extensive dentigerous cyst associated with a mesiodens who presented with a painful swelling in the left nasolabial area. We discuss the imaging findings in this case, particu…

Laser-assisted treatment of a dentigerous cyst: case report.

Dentigerous cysts are benign maxillary odontogenic cysts associated with the crown of an unerupted tooth. They can expand the cortical bone to such an extent that they cause displacement of teeth and root resorption in the adjacent teeth. The purpose of this report was to describe the clinical case of a dentigerous cyst in a mandibular permanent molar of a 6-year-old child. The cyst caused the displacement of the bud of the permanent second molar and root resorption of the primary second molar. The cyst was treated by: (1) mucous fenestration using an erbium laser: (2) drainage of the fluid co…

Dentigerous Cyst associated with Horizontally Impacted Mandibular Second Premolar.

Dentigerous Cyst/developmental cyst of benign odontogenic origin are ones that surround the crown of impacted, embedded, unerupted or developing teeth. Dentigerous cyst is second most common cyst of the oral cavity after radicular cyst. They are usually solitary in occurrence and mostly associated with the mandibular third molars. Dentigerous cysts involving impacted second premolars are rarely reported in the literatures. We present a rare case of dentigerous cyst in a 12-year-old female patient associated with an impacted mandibular second premolar. How to cite this article: Mishra R, Tripat…

Descending necrotizing mediastinitis associated with an infected dentigerous cyst.

Descending necrotizing mediastinitis (DNM) is a rare condition in which an infection from the head and neck propagates into the mediastinum. The most common cause of DNM is odontogenic infection. DNM is spread by the fascial planes from the neck into the mediastinum and requires an aggressive surgical drainage through cervical and thoracic approaches. We report on a 67-year-old male patient, who had acute mediastinitis related to an infected dentigerous cyst in the left parasymphyseal region. A multidisciplinary team approach was used to treat the patient. The team consisted of thoracic surgeo…

Conservative treatment of dentigerous cyst associated with primary teeth.

Dentigerous cyst is the most common odontogenic cyst. It is characterized by a unilocular radiolucent lesion that encloses permanent tooth buds or, under certain circumstances, displaced tooth buds. Buccal bony expansion is the most common clinical feature. Several treatment modalities have been mentioned in the literature for management of dentigerous cysts. The purpose of this article was to report an extensive right mandibular dentigerous cyst on a 10-year-old boy. Marsupialization was chosen to preserve the permanent tooth bud and a denturelike obturator was then provided for space mainten…
