[Dermatomyositis-update].

Dermatomyositis is a rare idiopathic inflammatory myopathy that affects adults and children, mostly female. Hallmarks of the disease are myositis with necrosis, regeneration and perifascicular atrophy accompanied by a typical skin rash with heliotrope erythema, Gottron's sign, Gottron's papules and nail fold changes with splinter hemorrhage. Typical skin symptoms may appear 6 months up to 2 years before muscle involvement (amyopathic dermatomyositis). New myositis-specific antibodies may allow clinicoserologic correlations within a heterogeneous clinical spectrum. Autoantibody profiles, subtyp…

Juvenile dermatomyositis.

Juvenile dermatomyositis (JDM) is a rare, often chronic autoimmune disease with onset during childhood. It is characterized by weakness in proximal muscles and pathognomonic skin rashes. Although the etiology remains unclear, it has been proposed that JDM is caused by a vasculopathy within the muscle tissue and multiple other organ systems of genetically susceptible individuals, possibly in response to environmental triggers. The goals of treatment include control of the underlying inflammatory myositis and prevention and/or treatment of complications (eg, contractures and calcinosis). Delayed…

[Update: dermatomyositis].

Dermatomyositis is an idiopathic inflammatory myopathy and an "orphan disease" (incidence 1:100,000). It comprises a heterogenous clinical spectrum with periorbital heliotrope erythema, acral Gottron papules, and proximal muscle weakness. Muscle-specific antibody profiles correlate with clinical variants. Overlap with other collagen vascular disorders occurs and about one-third of patients have an underlying malignancy (paraneoplastic dermatomyositis). High-dose oral corticosteroids are the mainstay of treatment, given until improvement of muscle symptoms and/or normalization of muscle enzymes…

[Juvenile dermatomyositis].

Juvenile dermatomyositis (JDM) is an inflammatory multi-system disease of unknown etiology with classic involvement of the skin and striated muscles. Following a prodromal period, patients develop a progressive proximal muscle weakness. Typical skin involvement includes heliotrope rash, facial erythema, Gottron's sign and nailfold capillary abnormalities. For the diagnosis of JDM, modified Bohan and Peter criteria are used including clinical skin and muscle signs plus elevated muscle enzymes and typical findings from electromyography, muscle biopsy and - more recently - also on magnetic resona…

Amyopathic dermatomyositis.

UNLABELLED: Amyopathic dermatomyositis (ADM) is a rare condition characterized by skin lesions suggestive of dermatomyositis (DM) without detectable muscle abnormalities after at least 2 years of follow-up. Pulmonary fibrosis is uncommon in patients with ADM. CASE REPORT: A 64-year-old woman presented with a 2 years and 6 months history of nondestructive polyarthritis. She had skin changes suggestive of DM, including a pink rash over the face, neck, and forearms; Gottron's papules over the metacarpophalangeal joints; and heliotrope edema of the eyelids. She reported no muscle symptoms. Findin…

[Amyopathic dermatomyositis].

Dermatomyositis represents a progressive autoimmune disease of the connective tissue with SLE-like dermatitis, with inflammatory myopathy and muscular weakness. It is an infrequent and serious disease affecting not only the skin and striated muscle. Cases without skin affection are classified as polymyositis. Beside autoimmune mechanisms, aetiology includes also genetic, environmental and paraneoplastic factors. Clinical skin symptoms as well as histopathological findings include Gottron's papulae, heliotropic periorbital erythrema, which, together with palpebral oedema, brings about the weepy…

[Amyopathic dermatomyositis].

Dermatomyositis is a systemic autoimmune disease which belongs to the group of idiopathic inflammatory myopathies. The disease is rare with an incidence of 0.1-1/100,000 and a prevalence of 1-6/100,000. Women are affected twice as often as men. In some patients the disease presents with dermatologic changes weeks to months before the myopathy arises. It was observed that in some patients the myositis develops much later and sometimes not at all. Therefore, the term of dermatomyositis sine myositis used in earlier literature has been changed to amyopathic dermatomyositis. The most important que…

[Juvenile dermatomyositis].

Juvenile dermatomyositis is part of the idiopathic inflammatory myositis group of diseases, presenting acute or chronic multisystemic inflammatory injury, that mainly affects muscles and skin. It s incidence is 4/1,000,000, prevailing in women and Caucasians, with symptoms starting between 5 14 years of age. The etiology is unknown, nevertheless is well recognized the role of vasculitis produced by autoantibodies, such is responsible for the damage on skin and muscular microvasculature. The acute or chronic complications and the neuropathic manifestations (cerebral vasculitis and peripheric ne…

Amyopathic dermatomyositis.

Amyopathic dermatomyositis is a variant of dermatomyositis that is characterized by the typical skin rash but without the muscle abnormalities. It has been proposed that the amyopathic and myopathic forms of dermatomyositis exist on a continuum, a concept that is supported by family and genetic studies and the observation that a small proportion of amyopathic patients transform to a frankly myopathic state. The amyopathic state is defined by a lack of muscle weakness and through diagnostic tests, including serum muscle enzymes, electromyogram studies, and muscle biopsies, that are usually norm…

Familial dermatomyositis.

Susceptibility, response to therapy, prognosis, and natural history of connective tissue diseases are variably linked to histocompatibility antigens (HLA), although the precise role that such genes play is still not clear. In juvenile dermatomyositis (JDMS) there are few reports of both concordance and discordance in identical twins and occurrence in more than one family member. To further document the familial occurrence of JDMS, we describe 2 sisters, one with classical JDMS and the other with amyopathic dermatomyositis. The 2 patients were shown to share the HLA-DQA1*0501 allele, known to b…

Juvenile dermatomyositis.

Juvenile dermatomyositis (JDMS) is a chronic inflammatory condition characterized by muscle weakness and a distinctive rash caused by underlying vasculopathy. Long-term complications include subcutaneous and muscular calcification, contractures and in some cases the gradual development of a second connective tissue disease. Early aggressive treatment with systemic immunosuppressants and other agents such as intravenous immunoglobulin (IVIG) reduces mortality and morbidity.

Juvenile dermatomyositis.

Juvenile dermatomyositis is a chronic inflammatory disease of skeletal muscle accompanied by skin involvement. It is a presumed autoimmune disease. Other organs may be involved. Corticosteroids are standard therapy and are associated with decreased mortality. Significant morbidity may occur.

Juvenile dermatomyositis.

Myositis in childhood is characterized by elevated serum levels of muscle-derived enzymes, proximal symmetrical muscle weakness, abnormal EMG findings, and a muscle biopsy, which frequently documents an inflammatory process. In the pediatric age group, JDMS, which has characteristic cutaneous involvement in addition to myositis, is much more common than PM and is more common among female patients. With the use of steroids, mortality has been reduced from 33 per cent to 7 per cent. The development of calcifications can be the most debilitating consequence of JDMS. It is our premise that JDMS is

Juvenile dermatomyositis.

Thirteen cases of dermatomyositis seen in children are described. Records of hospitals serving 250,000 children were reviewed for the 10-year period during which these children were seen and no other cases were found. Eleven of the 13 are living, including seven believed to be in complete remission; the remainder have ongoing disease of varying severity. Literature concerning this unusual childhood disease is reviewed and there is general agreement as to the characteristic nature of the clinical picture. Special features, including rash, muscle findings, deformity, calcinosis and gastrointesti…
