Assessment of diaphragm weakness.

Thirty patients with breathlessness and diaphragm weakness were studied by measuring transdiaphragmatic pressures during maximal inspirations to total lung capacity, maximal static inspiratory efforts from residual volume, and maximal sniffs from functional residual capacity. Maximal static respiratory mouth pressures were also recorded, and rib cage and abdominal movements were monitored with pairs of magnetometers. Sniff transdiaphragmatic pressure was abnormally low in all patients and was correlated with transdiaphragmatic pressure during other maneuvers, and with maximal static inspirator…

Management of Diaphragm Tumors.

Diaphragm tumors are very rare, with secondary tumors being more common than primary tumors. The most common benign primary tumors include lipomas and cysts, and malignant primary tumors include rhabdomyosarcoma and leiomyosarcoma. Endometriosis is the most common benign secondary tumor, followed by malignant tumors with localized spread of disease. In addition, widely metastatic disease has been described. Benign lipomas and cysts can be managed conservatively, but more complex or concerning disease typically requires resection. The diaphragm can often be repaired primarily, though any large …

Thoracoscopic Radial Diaphragm Plication.

In appropriately selected patients diaphragm plication improves quality of life by alleviating dyspnea and allowing patients to return to their routine activities. Many plication techniques exist, but the optimal surgical approach remains unclear. We report our experience with a minimally invasive radial diaphragm plication technique. It offers 2 distinct advantages: (1) suture placement avoids the phrenic nerve fibers, allowing for potential nerve recovery, and (2) the interrupted radial sutures improve the distribution of tension along the flaccid muscle and may achieve a more durable repair…

A demand diaphragm pacemaker.

A PACO2 feedback control system (demand diaphragm pacemaker) was developed in this laboratory to maintain homeostasis during diaphragm pacing, thus avoiding hypo- or hyperventilation. Application of this system to the research animal made it possible to maintain blood gases and pH within normal limits for up to 15 hrs. Oscillation between maximal tidal volume and apnea that occurred with the amplitude feedback control was eliminated by using a rate feedback control. A possible additional benefit of a feedback control system is the reduction of current applied to the nerve as compared to that i…

Imaging of the Diaphragm: A Primer.

The diaphragm is a critical musculotendinous structure that contributes to respiratory function. Disorders of the diaphragm are rare and diagnostically challenging. Herein, the author reviews the radiologic options for the assessment of the diaphragm.

[Surgical Principals of the Diaphragm].

Due to its importance as a central respiratory muscle as well as a separating layer between the thorax and abdomen, the functional integrity of the diaphragm is crucial in any case of surgical intervention. High demands are placed on surgical interventions regarding the functional integrity of the diaphragm. The aim of surgery may even be an improvement in the functional status of the patient. The surgery of the diaphragm is performed according to the same basic principles, regardless of the indication and the extent of the surgery. One of the most important principles concerns the suturing of…

Ageing of the Diaphragm Muscle.

The diaphragm muscle is the most important contractile district used for breathing. Like other muscles in the human body, it is subject to ageing and sarcopenia. Sarcopenia can be classified as primary (or age-related) when there are no local or systemic pathologies that cause a functional and morphological detriment of skeletal musculature. Secondary sarcopenia occurs when there is a cause or more pathological causes (illness, malnutrition, immobility) related or unrelated to ageing. In the elderly population, transdiaphragmatic pressure (Pdi) decreases by 20-41%, with a decline in the overal…

Diaphragm disease of the jejunum.

Non-steroidal anti-inflammatory drugs (NSAIDs) are associated with the development of diaphragm disease. We report a 73-year-old male patient with this condition who had used NSAIDs for 2 years. He presented with general weakness, syncopal attacks, and a short history of melena. At laparotomy, multiple areas of constricted bowel were found in the resected proximal jejunum. Gross and microscopic examination confirmed diaphragm disease. The relevant literature is reviewed.

Acquired paralysis of the diaphragm.

Acquired DP in adults is multifactorial. Symptoms largely depend on whether or not the paresis is unilateral or bilateral and on the etiology, onset, progression, and presence of underlying pulmonary disease. UDP is a well-tolerated condition with a favorable prognosis. Diaphragmatic plication is indicated in carefully selected patients with UDP and can be done by minimally invasive techniques. Patients with BDP are usually severely symptomatic and require some form of mechanical ventilatory support. Diaphragmatic pacing is an option for a select group of patients with BDP in centers with spec…

Eventration of the diaphragm and associations.

Eventration of the diaphragm, most often an isolated entity and detected incidentally, has been known to be associated with several genetic syndromes. Authors report their experience of seeing diaphragmatic eventration in association with Poland syndrome and wandering spleen syndrome and briefly discuss the literature.

Pleomorphic rhabdomyosarcoma of the diaphragm.

Pleomorphic rhabdomyosarcoma of the diaphragm represents fewer than 0.0001% of all cancers. One case is reported, and a total of eight cases to date are reviewed. It is an adult disease of both sexes, the lowest reported age being 14 years. The tumor presents at a late stage, and the average symptom-death interval is only a few months. Early diagnosis and wide excision are essential along with radiotherapy and chemotherapy before any survival--much less worthwhile survival--can be hoped for. There is evidence of host immune response against rhabdomyosarcomas. Immunotherapy, therefore, should b…

Diaphragm disease of the ileum.

Diaphragm disease is a condition wherein the bowel lumen is divided into a series of short compartments by circular membranes of mucosa and submucosa, leading to a pinhole lumen and subsequent obstruction. It is a very rare condition commonly attributed to non-steroidal anti-inflammatory drugs (NSAIDs) usage. We present two cases of obstruction secondary to diaphragm disease of the ileum diagnosed on histology of the resected specimen. One patient had no history of chronic NSAID intake.

The diaphragm and respiratory muscles.

This article describes the functional anatomy of the muscles that contribute to the generation of ventilatory pressures. The physiologic principles that guide their function are analyzed, and their composition and metabolism are described, inasmuch as this affects their adaptation and capacity to face acute and chronic loads. Special emphasis is placed on the clinical and laboratory evaluation of their function. In addition, the concept of respiratory muscle fatigue is developed so the reader can place all these concepts in the context of their clinical applications.

Imaging evaluation of the diaphragm.

The diaphragm performs most of the physiologic work of inspiration, and forms an anatomic barrier between the thoracic and abdominal cavities. Disorders of the diaphragm can be related to impairment of either of these functions, and most have radiologic manifestations. Both intrathoracic and intra-abdominal disease processes can alter the normal radiologic appearance of the diaphragm. Abnormalities are usually first detected on chest radiographs, often incidentally in asymptomatic patients, and many require further characterization by other imaging studies for definitive diagnosis. Fluoroscopy…

Effect of hyperinflation on the diaphragm.

Acute hyperinflation causes the inspiratory muscles to operate at shorter than normal lengths. The ability of these muscles, in particular the diaphragm, to lower intrathoracic pressure is therefore reduced. Skeletal muscles, however, adapt to chronic shortening, and animals models of emphysema have shown that with chronic hyperinflation, the diaphragmatic muscle fibres lose sacromeres. As a result, the force-generating ability of these fibres is relatively preserved. In patients with hyperinflation due to chronic obstructive pulmonary disease, the ability of the diaphragm to generate pressure…
