[Dietary advice for treatment of inborn errors of metabolism in adult neurology: principes and limitations].

Special diets can be an efficient treatment for certain inborn errors of metabolism. Regimens applicable in adult neurology include low protein diet (phenylketonuria, homocystinuria, urea cycle disorders, organic acidurias), low fatty acid diets (fatty acid B oxidation defects, adrenomyeloneuropathy, Refsum's disease) and ketogenic diet (pyruvate dehydrogenase deficiency, glucose transporter (GLUT1) deficiency, refractory epilepsy). Although, these regimens can be very efficient in some instances, withdrawal and nutrient deficiencies are major problems encountered.

Thiamin, riboflavin and pyridoxine status in chronic renal insufficiency.

The restricted diets of patients with chronic renal insufficiency (CRI) can lead to inadequate intake of some vitamins. We studied thiamin, riboflavin and pyridoxine nutritional status of CRI subjects on unrestricted or low-protein diet. Results show that riboflavin status is inadequate even before the beginning of the follow up and worsens with time for both groups, while thiamin and pyridoxine status becomes inadequate in some patients after a long period on the low-protein diet. Water-soluble vitamin supplements could improve the nutritional status of CRI patients on protein-restricted diet

Protein and calorie malnutrition in infant cebus monkeys: growth and behavioral development during deprivation and rehabilitation.

The growth and development of 32 cebus monkeys were studied during a period of insult in nutritional or rearing conditions and after rehabilitation. Eight experimental groups of four animals each were subjected to one of four diets--control, protein restricted, calorie restricted, and protein-calorie restricted, and one of two rearing conditions-partial isolation or a comparatively enriched condition-in a 4 x 2 factorial design. The period of insult from 2 to 6 months of age was followed by 6 months of rehabilitation in both diet and rearing conditions. It was found that only diet affected phy

Serum Metabolomics Reveals Impaired Fatty Acid Oxidation and Elevated One-Carbon Demand in Cats Fed a Low-Protein Diet Compared with a Low-Fat or Low-Carbohydrate Diet.

BACKGROUND: Feline obesity remains a major metabolic health concern, yet the mechanistic effects of controlled macronutrient substitution on feline metabolism are poorly defined. Cats, as obligate carnivores, have high amino acid oxidation and limited carbohydrate metabolic capabilities that may result in metabolic shifts when dietary protein is substituted for dietary fat or for dietary carbohydrate, as nitrogen-free extract (NFE). OBJECTIVES: The aim of our study was to apply targeted metabolomics to assess fasted metabolic consequences of an isoenergetic macronutrient substitution diet des

Introducing a granule based protein substitute to the diet of a child with phenylketonuria to address reluctance to ingest phenylalanine-free protein substitute: A case report.

Phenylalanine (Phe)-free protein substitutes are used within the management of phenylketonuria (PKU). However, adherence to the Phe-restricted diet is often challenging. A child (age 4.5 years) with PKU rejected the Phe-free protein substitutes used within her therapeutic diet, causing stress for herself and family at mealtimes. Switching to a new Phe-free protein substitute that can be mixed into other foods [PKU GOLIKE® (3-16)] provided an alternative strategy that was acceptable to the child. Good control of blood Phe was maintained. Newer Phe-free protein substitutes may provide a strategy

Growth and reproductive adaptation in male rats with chronic protein deficiency.

In previous studies, male rats fed a low-protein diet beginning at weaning were found to have impaired sexual development through age 11 weeks when compared to food-restricted, weight-matched controls fed a diet with normal protein content. To determine whether male rats show long-term adaptation of the reproductive axis to low-protein feeding, we assessed sexual maturation and growth in rats fed a low protein (9%) diet from weaning until sacrifice at various points in time between ages 79 and 185 days. After age 80 days, there was no difference in reproductive organ weights (prostate, seminal

High-protein vs. standard-protein diets in overweight and obese patients with heart failure and diabetes mellitus: findings of the Pro-HEART trial.

AIMS: The intermediate-term effects of dietary protein on cardiometabolic risk factors in overweight and obese patients with heart failure and diabetes mellitus are unknown. We compared the effect of two calorie-restricted diets on cardiometabolic risk factors in this population. METHODS AND RESULTS: In this randomized controlled study, 76 overweight and obese (mean weight, 107.8 ± 20.8 kg) patients aged 57.7 ± 9.7 years, 72.4% male, were randomized to a high-protein (30% protein, 40% carbohydrates, and 30% fat) or standard-protein diet (15% protein, 55% carbohydrates, and 30% fat) for 3 mont

Improved dietary treatment with low protein and phosphorus restriction in uremic rats.

The independent effect of low-protein or low-phosphorus diets in chronic renal failure were studied. The following four isocaloric diets, which contained various amounts of protein (24%, 6%) and phosphorus (0.5%, 0.12%) were fed to chronically uremic rats: high protein and no phosphorus restriction (HPrNP), high protein and phosphorus restriction (HPrRP), low protein and no phosphorus restriction (LPrNP), and low protein and phosphorus restriction (LPrRP). Adult male rats were made chronically uremic by electrocoagulation of one kidney and contralateral nephrectomy. Two grades of renal damage

Dietary protein and the renin-angiotensin system in chronic renal allograft rejection.

We examined the effects of dietary protein restriction in fourteen patients with chronic kidney rejection. The patients were randomly assigned, using a crossover design to two 11-day periods, one on a low-protein diet (0.55 g/kg/day) and the other on a high-protein diet (2 g/kg/day). The low protein diet was associated with a significant improvement in glomerular permselectivity without any change in blood pressure, glomerular filtration rate, or renal plasma flow. The low protein diet was also associated with a significant reduction in plasma renin activity. Acute converting enzyme inhibition

Responses to dietary macronutrient distribution of overweight rats under restricted feeding.

Changes in protein and amino acid balance after energy-restricted feeding have scarcely been studied, although it has been suggested that protein utilization may depend on the macronutrient composition of the restricted diet. The aim of this study is to investigate the influence of dietary fat quality and content, during an energy-restricted regime, on body composition and protein metabolism. Therefore, diet-induced overweight rats were divided into three dietary groups: one group was fed on a control diet ad libitum (control) and the other two groups were fed energy-restricted diets (about 60

Effects of Long-Term Dietary Protein Restriction on Intestinal Morphology, Digestive Enzymes, Gut Hormones, and Colonic Microbiota in Pigs.

Using protein-restriction diets becomes a potential strategy to save the dietary protein resources. However, the mechanism of low-protein diets influencing pigs' growth performance is still controversial. This study aimed to investigate the effect of protein-restriction diets on gastrointestinal physiology and gut microbiota in pigs. Eighteen weaned piglets were randomly allocated to three groups with different dietary protein levels. After a 16-week trial, the results showed that feeding a low-protein diet to pigs impaired the epithelial morphology of duodenum and jejunum (p < 0.05) and reduc

[Nutritional management of patients suffering of chronic renal failure].

Nutritional support constitutes a fundamental approach to favour the management of chronic renal failure and to postpone the need of kidney dialysis. The specific goals of the nutrition intervention are: control of protein intake, control of phosphate and of calcium intake, control of potassium intake, control of energy intake, control of lipid intake with clear identification of the polyunsaturated vs monounsaturated vs saturated fatty acid rate, control of vitamin intake, prevention of malnutrition and intervention with oral supplements or artificial nutrition (even if for short time) if mal

Effects of a restricted diet on the submaxillary gland of intact and castrated male rats.

Effects of diet restriction (50% of normal) during 4 weeks were studied in intact (controls) and castrated rats. A restricted diet led to cessation of animal growth. No differences were observed between controls and castrated rats. However, underfeeding produced a reduction of testicular weight in intact rats. Histochemical observations showed a reduction of tryptophan (a marker of the presence of testosterone-dependent protein) in the submaxillary gland from animals fed a restricted-diet. In animals fed a normal diet, castration reduced significantly the gland weight, as well as the diameter

Effectiveness of a carbohydrate restricted diet to treat non-alcoholic fatty liver disease in adolescents with obesity: Trial design and methodology.

BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is the most common liver disorder among children in the developed world and can progress to cirrhosis, hepatocellular carcinoma, and liver failure. No evidence-based dietary guidelines exist on the most effective diet prescription to treat NAFLD. OBJECTIVE: To compare the effect of a carbohydrate (CHO)-restricted diet vs fat-restricted diet, the current standard of care, on changes in hepatic fat infiltration, body composition, and metabolic health over an 8-week period among overweight and obese children diagnosed with NAFLD. METHODS: In

Biochemical characteristics of different forms of protein-energy malnutrition: an experimental model using young rats.

I. In three separate experiments, four groups of five to eight young male rats were fed either (i) a high-protein diet, for which the net dietary protein:total metabolizable energy ratio (NDp:E) was 0-1 (HP diet); or (ii) a low-protein diet, for which NDp:E was 0-04 (LP diet). In both these groups, food intake was ad lib. In group (iii) the HP diet was given in an amount approximately equal to that taken by the LP group fed ad lib. (HP-restricted). In group (iv) rats were fasted for 48 h after receiving the HP diet (HP-fasted). Each experiment lasted 4 weeks. 2. In the LP and HP-restricted gro

Refeeding syndrome in a young woman with argininosuccinate lyase deficiency.

A severely chronically protein and calorie restricted young woman with argininosuccinate lyase deficiency developed transient refeeding syndrome (RFS) and hyperammonemia after modest diet liberalization following initiation of glycerol phenylbutyrate (GPB). The patient required IV supportive care and supplementation with potassium, magnesium and calcium. She is now doing well on GPB and an appropriate maintenance diet. Susceptibility to RFS should be considered in chronically nutritionally restricted patients with metabolic disorders after liberalization of diet.

Clinical improvements after treatment with a low-valine and low-fat diet in a pediatric patient with enoyl-CoA hydratase, short chain 1 (ECHS1) deficiency.

BACKGROUND: Enoyl-CoA hydratase short-chain 1 (ECHS1) is a key mitochondrial enzyme that is involved in valine catabolism and fatty acid beta-oxidation. Mutations in the ECHS1 gene lead to enzymatic deficiency, resulting in the accumulation of certain intermediates from the valine catabolism pathway. This disrupts the pyruvate dehydrogenase complex and the mitochondrial respiratory chain, with consequent cellular damage. Patients present with a variable age of onset and a wide spectrum of clinical features. The Leigh syndrome phenotype is the most frequently reported form of the disease. Herei

Metabolic and development changes in growing rats born to dams restricted in protein and/or energy intake.

During gestation and lactation, the food intake of female rats was restricted to 50% of the ad libitum stock diet intake of controls (C). One restricted group (M1) was fed stock diet while another restricted group (M2) was fed casein-supplemented stock diet containing 66% protein. Observations were directed to their progeny. After weaning at 21 days, all progeny were fed the stock diet ad libitum for 2 weeks. Mean birth weight of M2 was lower than M1 which was lower than C. Response of fetal and newborn piglets to maternal protein restriction during early or late pregnancy.

Dietary protein restriction during pregnancy has an adverse effect on progeny development, although the importance of the time at which the nutritional insult occurs is unclear. The objective in our study was to test the hypothesis that severe protein restriction during the first trimester of swine pregnancy has a greater detrimental effect on fetal development than restriction in late pregnancy. On the day of mating, primiparous swine were assigned to control (C, 13% protein) or protein-restricted (PR, 0.5% protein) diets and fed in 4 regimens: 1) C diet throughout pregnancy (114 +/- 2 days)
