Preservation of the anterior ciliary vessels during extraocular muscle surgery.

Conventional, full-tendon, rectus muscle surgery disrupts the anterior ciliary vessels. This may lead to anterior segment ischemia when the number of interrupted vessels is sufficient to significantly compromise anterior segment blood flow. Clinical observations and histologic studies provide new anatomic information concerning the course of the anterior ciliary vessels in the sub-Tenon's region. These studies demonstrate the feasibility of dissection and preservation of the anterior ciliary vessels during rectus muscle surgery. Dissection and preservation of 35 clinically apparent anterior ci

Impact of the Elephant Trunk on Distal Remodelling After Surgery for Acute Type I Aortic Dissection.

OBJECTIVES: For surgical repair of acute type I aortic dissection, total arch replacement (TAR) with a frozen elephant trunk (FET) has been known to result in better long-term remodelling of a residual false lumen. This study was designed to investigate the impact of the elephant trunk by comparing long-term remodelling features among different extents and strategies of aortic replacement. METHODS: We conducted a single-centre retrospective analysis of patients who underwent surgical repair for acute type I aortic dissection from January 2004 to June 2022. Patients were categorized based on t

Tetrafurcate graft Eversion suturing combined with single-branched stent technique in surgical repair of acute type A aortic dissection.

BACKGROUND AND OBJECTIVE: Acute type A aortic dissection (ATAAD) is a critical and life-threatening condition. To date, surgical intervention remains the primary life-saving treatment. However, this surgical approach is often accompanied by a range of postoperative complications, and the associated mortality rate remains high. In this study, we introduced an innovative technique that utilized tetrafurcate graft eversion for suturing and reinforcing the aortic root, in conjunction with a single-branched stent technique, for the management of ATAAD. METHOD: A total of eighty-nine ATAAD patients

Functional polymorphisms in matrix metalloproteinases -1, -3, -9 and -12 in relation to cervical artery dissection.

BACKGROUND: Cervical artery dissection is a leading cause of cerebral ischemia in young adults. Morphological investigations have shown alterations in the extracellular matrix (ECM) of affected vessel walls. As matrix metalloproteinases (MMP) play a central role in the regulation of the ECM, an increased expression of these enzymes might lead to the endothelial damage in spontaneous cervical artery dissection (sCAD). Five different DNA polymorphisms in MMP-1, -3, -9 and -12 were tested for their frequency in patients with sCAD and compared with those of a control population. METHODS: Blood wa

A modified root reinforcement technique for acute aortic dissection with a weakened aortic root: a modified Florida sleeve technique and two cases report.

Despite marvelous advances in repair for acute type A aortic dissection over past decades, it remains challenging to repair the aortic root when aortic dissection extended to the sinuses causes the fragile root because of its thinner layers, which are susceptible to suture trauma. Here, we describe a modified Florida sleeve technique to strengthen the weakened aortic root. After mobilization of the aortic root and the coronary arteries, a designed Dacron tube graft was wrapped outside the sinuses as neo-adventitia to reinforce the dissected weakened wall. During surgery for aortic dissection,

[Carotid dissection associated with Ehlers-Danlos syndrome. Report of one case].

In the vascular type of Ehlers-Danlos syndrome there is a defect in the synthesis of collagen type III, important constituent of the blood vessel wall, which may cause arterial rupture and dissection. We report a 15-year-old girl with a history of Ehlers-Danlos syndrome, who suffered an ischemic stroke secondary to dissection of the right internal carotid artery. She was managed conservatively with antiplatelet agents with a good clinical response.

Quantitative evaluation for intravascular structures of vertebral artery dissection with a novel zoomed high-resolution black-blood MR imaging.

BACKGROUND: Although non-stroke vertebral artery dissection (VAD) is diagnosed using MRI, detecting the subtle intravascular structure remains challenging. This study aimed to evaluate the validity of quantitative intravascular scanning based on novel zoomed high-resolution black blood (Z-HB) MRI for distinguishing VAD from other vessel pathologies. METHODS: Twenty-one patients with non-stroke VAD and 18 with symptomatic atherosclerotic plaques in their vertebral artery underwent Z-HB MRI and subsequent profile curve processing. Axial Z-HB imaging was obtained from dissected and normal segmen

Collet-Sicard syndrome due to cervical artery dissection disclosed by high-resolution magnetic resonance imaging.

BACKGROUND AND PURPOSE: Cervical artery dissection (CAD) represents a leading cause of unilateral lower cranial nerve IX-XII palsy, known as Collet-Sicard syndrome (CSS). High-resolution magnetic resonance imaging (HR-MRI) is widely used in the evaluation of patients with CAD, providing information regarding vessel wall abnormalities and intraluminal thrombus. METHODS: We present a patient with palsy of multiple lower cranial nerves in the context of CSS, attributed to unilateral spontaneous internal carotid artery dissection. RESULTS: We describe a 68-year-old man with unremarkable previous

Total arch repair for acute type A aortic dissection with 2 modified techniques: open single-branched stent graft placement and reinforcement of the dissected arch vessel stump with stent graft.

BACKGROUND: In total arch replacement for patients with acute type A aortic dissection, anastomoses of the graft to the left subclavian artery and descending aorta are often difficult, and the arch vessel anastomosis is frequently performed at the site of dissection. To make this procedure easier and safer, we developed 2 modified techniques: open single-branched stent graft placement into the left subclavian artery and the descending aorta and reinforcement of the dissected arch vessel stump with a stent graft neointima. The feasibility and initial clinical results of these 2 new techniques a

Intimal flap detected by three-dimensional curved multiplanar reconstruction image in isolated posterior inferior cerebellar artery dissection: a report of two cases.

BACKGROUND: Spontaneous isolated posterior inferior cerebellar artery (PICA) dissection has been reported more frequently since high-resolution vessel wall magnetic resonance imaging (HR vw-MRI) was introduced to the field. The intimal flap or double lumen, which is commonly reported to be a direct sign of the dissection, is not easily detectable on HR vw-MRI because the size of the PICA is very small and tortuous. CASE PRESENTATION: Two patients with posterior circulation ischemic stroke due to spontaneous isolated PICA dissection underwent HR vw-MRI. The curved multiplanar reconstruction im

[Morphological signs of mitochondrial cytopathy in skeletal muscles and micro-vessel walls in a patient with cerebral artery dissection associated with MELAS syndrome].

Skin and muscles biopsy specimens of a patient harboring A3243G mutation in mitochondrial DNA, with dissection of internal carotid and vertebral arteries, associated with MELAS were studied using histochemical and electron-microscopy techniques. Ragged red fibers, regional variability of SDH histochemical reaction, two types of morphologically atypical mitochondria and their aggregation were found in muscle. There was correlation between SDH histochemical staining and number of mitochondria revealed by electron microscopy in muscle tissue. Similar mitochondrial abnormality, their distribution

Subarachnoid Hemorrhage Due to Flow-Related Dissection of the Posterior-Inferior Cerebellar Artery Associated with a Distal Arteriovenous Malformation.

BACKGROUND: Cerebral arteriovenous malformations (CAVMs) are characterized by altered hemodynamics and associated with angioanatomic changes, such as aneurysms. We encountered a patient with a CAVM-associated dissection of the medial trunk of the posterior inferior cerebellar artery (PICA) instead of an aneurysm. CASE DESCRIPTION: We report the case of a 56-year-old male patient with spontaneous subarachnoid hemorrhage within the cisterna magna and fourth ventricle. Digital subtraction angiography revealed a cerebellar arteriovenous malformation located within the inferior semilunar lobule th

Thoraflex Hybrid Prosthesis (THP): the profile.

INTRODUCTION: With the incidence of thoracic aortic disease on the rise, total arch replacement (TAR) with frozen elephant trunk (FET) remains the gold-standard management strategy due to optimal results. Several FET devices exist commercially on the global market. However, the mainstay and most commonly used and reported device is the Thoraflex Hybrid Prosthesis (THP), with several recent reports suggesting its superiority. AREAS COVERED: This review aims to collate and summarize the evidence in the literature on the clinical outcomes of TAR with FET using THP, with a focus on mortality, neu

[Arterial Anastomosis( Aorta, Middle Artery, Dissected Vessels, and Artificial Vessels)].

Ascending aortic replacement is a basic procedure in aortic surgery. The crucial factor is the anastomosis method, including the anastomosis site formation. The anastomosis method is usually completely different between true aneurysms and acute aortic dissection because of the difference in the fragility of the aorta. It is necessary to prepare a surgical strategy well preoperatively, such as whether to perform the anastomosis under aortic clamping or by the open distal method, the order of anastomosis, the type of prosthesis, and whether to use a one-piece or two-piece prosthesis. In recent y
