In sixth nerve palsy the role of a muscle-transposing procedure, as compared with a recession-resection procedure, is discussed, and reference is made to the special problems when the palsy is the result of hydrocephalus with frequently an associated A phenomenon. In Duane's retraction syndrome, surgical treatment should be limited as a general rule to a restoration of binocular function in the primary position with elimination of the compensatory head posture, because of an understanding of the underlying anomaly of innervation.

PURPOSE: Although the high incidence of skeletal, neural, and other ocular problems has been identified in previous reports of large cohorts of patients with Duane syndrome, there has not been any documentation of anomalies in close relatives. Patients with Duane syndrome have a greater than 50% incidence of associated abnormalities that include deafness, other ocular anomalies, cranial nerve palsies, and skeletal abnormalities. This study was undertaken to review the experience of our strabismus service with patients who have Duane retraction syndrome (DRS) and to delineate the associated ano…

OBJECTIVE: To summarize the clinical features of 201 cases with Duane's retraction syndrome (DRS) and discuss its differential diagnosis.
METHODS: The 201 cases in 1979 similar 1996 were retrospectively summarized. The clinical features including chief complaints, sex distribution, age at first visit, laterality, type of presentation, ocular deviation in the primary position, refractive errors, amblyopia, globe retraction, change of the palpebral fissure, upshoot and downshoot in adduction, binocular single vision, and its associated ocular and non-ocular anomalies were analyzed.
RESULTS: Th

Duane's retraction syndrome is associated with various ocular and systemic abnormalities. We report a case with bilateral Duane's retraction syndrome type I accompanied by renal agenesis, vesico-ureteral reflux, patent ductus arteriosus, and external ear malpositioning. Because of disabling consequences, we recommend a thorough physical examination and screening for renal and cardiac abnormalities in all patients presenting with Duane's reaction syndrome.

A 7-year-old boy with a history of Duane retraction syndrome was found to have an anomalous orbital band arising from the right lateral rectus muscle. Surgical removal of this band to correct exotropia resulted in postoperative optic neuropathy and hypoesthesia in the distribution of the superior branch of the trigeminal nerve. This case demonstrates a rare complication of orbital surgery in patients with Duane retraction syndrome. It serves as a reminder to evaluate patients with congenital cranial dysinnervation disorders for additional structural defects and consider orbital imaging in pati…

Duane retraction syndrome (DRS) is complicated to treat due to its wide spectrum of clinical presentations and the treatment of choice varies among surgeons. To provide insight into this challenging condition, we evaluated the long-term surgical outcomes of esotropic DRS type 1. The surgical motor success, defined as a horizontal deviation of 8 prism diopters (PD) or less, was found in 77.3% of patients after an average follow-up of 5.6 ± 3.4 years. After surgery, a gradual shift towards exotropia was observed at an average rate of 1.3 ± 3.7 PD exodrift per year. In this study, the long-term s…

We report a rare case of restrictive strabismus in a pediatric patient who was initially misdiagnosed with Duane retraction syndrome (DRS) because she had large-angle exotropia, restricted adduction, globe retraction, narrowing of the palpebral fissure, and downshoot on attempted adduction. Notably, the parents first observed strabismus at 12 months of age, with photographs taken before 6 months of age revealing no ocular misalignment. Despite the absence of typical inflammatory signs, the temporal discrepancy, characterized by delayed onset inconsistent with congenital DRS, prompted further i…

Duane retraction syndrome (DRS) is a congenital cranial dysinnervation disorder that is characterized by paradoxical lateral rectus muscle innervation of the affected eye by axons meant to innervate the ipsilateral medial rectus muscle, with resultant varying degrees of co-contraction. It is characterized by severe abduction deficiency, variable limitation of adduction, globe retraction with narrowing of the palpebral fissure, and oblique elevation or depression on adduction. A total of 16 patients with unilateral DRS were identified. The mean age was 13 ± 8 years (range: 6 to 28 years). There…

BACKGROUND: To evaluate possible monogenic and chromosomal anomalies in a patient with unilateral Duane retraction syndrome, modest dysmorphism, cerebral white matter abnormalities, and normal cognitive function.
MATERIALS AND METHODS: Performing high-resolution array comparative genomic hybridization (array CGH) and sequencing of HOXA1, KIF21A, SALL4, and CHN1 genes.
RESULTS: The proband had unilateral Duane retraction syndrome (DRS) type III on the right with low-set ears, prominent forehead, clinodactyly, and a history of frequent infections during early childhood. Motor development and c…

Duane retraction syndrome is characterized by globe retraction and palpebral fissure narrowing on adduction, with restriction of abduction, adduction, or both. Usher syndrome type 2 consists of congenital bilateral sensorineural hearing loss and retinitis pigmentosa. The authors present a case with a yet unreported association between Duane retraction syndrome type 1 and Usher syndrome type 2.

Purpose: We report a case of an unusual association between Duane retraction syndrome and Marcus Gunn jaw-winking syndrome in the eye of a patient without other anomalies. Methods: Clinical case. A four-year-old boy presented mild blepharoptosis of the left upper eyelid. The infant momentarily elevated the upper eyelid to a higher level by opening the mouth and thrusting the jaw to the right side. While attempting to abduct the left eye, there was no movement with the widening of the left palpebral fissure. Still, the normal movement of the right eye was consistent with the left rectus paralysi…

OBJECTIVE: To analyze whether the presence of the abducens nerve might depend upon the type of Duane's retraction syndrome (DRS).
DESIGN: Observational case series.
PARTICIPANTS: Sixteen patients with type 1 DRS, 2 patients with type 2 DRS, and 5 patients with type 3 DRS.
METHODS: Ophthalmologic examination and thin-sectioned magnetic resonance imaging (MRI) at the brain stem level were performed in 23 DRS patients. To confirm the accuracy of the procedure, we compared the results obtained with those of a control group of 30 individuals using the same technique.
MAIN OUTCOME MEASURES: Exam

We retrospectively studied 101 consecutive patients with Duane's retraction syndrome. Eighty-five percent were unilaterally affected and 15% were bilaterally affected. More patients had straight eyes in primary position than esotropia or exotropia. On lateral version reflex testing the amount of underaction of the lateral and medial rectus muscles could be determined, and could be used to predict whether the patients' eyes were esotropic, exotropic, or straight in primary position. Almost all the patients showed an esotropia when looking to one side and either an exotropia or no deviation to t

Introduction: Cysticercosis is a systemic parasitic disease caused by the larvae of the tapeworm Taenia and has varied clinical manifestations. Ocular involvement can be intraocular or extraocular and can mimic various etiologies and patterns of strabismus. This study reports 3 cases of orbital myocysticercosis mimicking clinical presentation of inverse Duane's retraction syndrome and discuss the clues to the clinical diagnosis. Methods: Retrospective review of medical records of three patients with medial rectus muscle cysticercosis with clinical findings mimicking acquired inverse Duane's re

Given our patients' diagnosis of Duane retraction syndrome (DRS) type 1 with normal head posture, a management plan of observation was recommended.
