[Diastrophic dwarfism (author's transl)].

On the basis of their own patient the authors discuss clinical and radiological features of diastrophic dwarfism. In this rare disorder the infant is dwarfed, the limbs are shortened. It is associated with marked talipes equinovarus, limited movements and contractures of other joints. Dislocation in the hip or knee and development of kyphoscoliosis leads to furhter deformity. As aetiopathogenesis the authors consider the role of infection in the mother during her early pregnancy or teratogenic aspects of some drugs (tetracyclines, septrim).

Mesomelic dwarfism in pseudoachondroplasia.

Pseudoachondroplasia (PSACH) is associated with mutations in the cartilage oligomeric matrix protein (COMP) gene and the clinical characteristics include short stature, deformities of the extremities involving the epiphyses and metaphyses, early onset arthritis, and ligament laxity. PSACH has been considered a rhizomelic form of dwarfism. So far no previous report has described mesomelic shortening of the limbs in PSACH. We reviewed nine patients with a diagnosis of PSACH based on clinical and radiographic examination and mutation analysis of the COMP gene. The mean height in the adults was 11…

[Limb lengthening in dwarfism].

Limb lengthening in dwarfism has become a standardised procedure with a good prognosis. In most cases external fixation is used. Gain of leg length up to 15 cm and more is possible in the lower leg and the femur and 8.5 cm in the humerus. Limb lengthening is useful in many cases of dwarfism due to skeletal dysplasia. There are a number of risks and possible complications involved and the procedure also requires considerable time. We report the results of 48 patients with dwarfism operated on in the Orthopädische Kinderklinik Aschau (Orthopaedic Hospital for Children). It must not be recommende…

Achondroplastic and hypochondroplastic dwarfism.

To ensure proper management, prognosis and genetic counseling among patients with chondrodystrophy a specific diagnosis is critical. Achondroplasia is the most common intrinsic bone dysplasia which results in short-limbed dwarfism. Because of its frequency, it is the most completely documented and understood of these conditions. Emphasis has been given to the various characteristics which allow an exact diagnosis to be made. Hypochondroplasia can be misdiagnosed unless attention is paid to the many differences which distinguish it as a separated disorder. Some ten other conditions must be cons…

Orthopedic complications of dwarfism.

Of 248 dwarfs with Achondroplasia, Pseudoachondroplastic dysplasia, Metaphyseal chondrodysplasia. Diastrophic dwarfism, Morquio syndrome, and Spondylo-epiphyseal dysplasia Congenita, 85 were managed by orthopedic methods. Experience with bracing is generally unsatisfactory. Surgical lengthening of the lower extremities is not recommended. A number of other surgical procedures are advisable in selected cases.

Microcephalic, osteodysplastic, primordial dwarfism.

A case of microcephalic, osteodysplastic, primordial dwarfism (cephaloskeletal dysplasia of Taybi and Linder) is reported. This rare disease is characterised by unique clinical appearances and diagnostic radiographic findings. It is also associated with distinctive brain abnormalities. The latter include micrencephaly, lissencephaly, corpus callosum aplasia/agenesis and unusual histological brain abnormalities.

[Thanatophoric dwarfism (author's transl)].

Two cases of thanatophoric dwarfism, one associated with cloverleaf skull diagnosed in the uterus and complete with histological findings are reported. Differential diagnosis with respect to similar forms of dwarfism and the genetic problem are discussed.

[Growth hormone receptor and dwarfism].

Laron syndrome is a rare autosomal recessive disease characterized by extreme resistance to growth hormone (GH). A GH receptor defect, which was hypothesized as the cause of the disease, was demonstrated after the cloning of the GH receptor cDNA. Several abnormalities of the GH receptor gene have been identified in the patients, demonstrating the genetic heterogeneity of the disease. Most of the identified mutations belong to the extracellular domain of the receptor, in the hormone-binding region, and this finding is consistent with the absence of GH binding activity found in the patients' pla

[Prenatal diagnosis of thanatophoric dwarfism].

Thanatophoric dwarfism is a rare malformation, occurring in less than 1:10,000 pregnancies. It can be discovered by standard ultrasound examination, but other skeletal dysplasias such as achondroplasia, achondrogenesis and polydactyly syndromes must be taken into consideration. Sonographic findings are polyhydramnia, narrow chest, symmetric tetramicromelia and macrocephaly. Macrocephaly might cause a problem for vaginal delivery. The narrow chest with secondary lung hypoplasia determines the infaust prognosis.

[Differential diagnosis of psychosocial dwarfism].

2 case reports regarding patients with a psychosocial dwarfism. Both children showed clinical and endocrinological signs of "idiopathic" hypopituitarism. Both patients came from economically well situated families of the upper middle-class. Disturbances in eating and sleeping behaviour are important hints that there might be a psychosocial etiology.
