Improving the selection of recommended prophylactic antibiotics using an electronic medical record.

OBJECTIVE: To estimate the effect of reference text in an electronic medical record (EMR) on the recommended use of preoperative antibiotics for cesarean deliveries. METHODS: A retrospective cohort study of all cesarean deliveries at Stony Brook Hospital between May 2009 and June 2011, was performed. Reference text was added to the EMR in May 2010. The cesarean deliveries performed before (group 1) and after (group 2) the addition of the reference text were compared for the type and dose of antibiotic ordered preoperatively, as well as the patient's body mass index (BMI), drug allergies, and

Validating pathophysiological models of aging using clinical electronic medical records.

Bioinformatics methods that leverage the vast amounts of clinical data promises to provide insights into underlying molecular mechanisms that help explain human physiological processes. One of these processes is adolescent development. The utility of predictive aging models generated from cross-sectional cohorts and their applicability to separate populations, including the clinical population, has yet to be completely explored. In order to address this, we built regression models predictive of adolescent chronological age from 2001 to 2002 National Health and Nutrition Examination Survey (NHA

Automated electronic medical record abstraction algorithm for radical prostatectomy outcomes.

OBJECTIVE: Variation in outcomes following radical prostatectomy and inclusion of prostate cancer surgery metrics in hospital ratings signal need for procedure-specific quality improvement (QI) efforts. We hypothesized that a novel electronic medical record (EMR)-based, automated algorithm-driven algorithm for surgical outcomes and quality metrics following robot-assisted laparoscopic radical prostatectomy (RALP) would demonstrate >90% sensitivity and specificity and significant inter-rater reliability (IRR) with National Surgical Quality Improvement Program (NSQIP) abstraction. STUDY DESIGN:

Electronic Monitoring of Medication Adherence to Direct Oral Anticoagulants: A Systematic Review.

Strict medication adherence, which reflects the process by which patients take their medication as prescribed, is crucial for the use of direct oral anticoagulants (DOACs). Therefore, technological devices may serve as promising tools for assessing adherence. We aimed to systematically review the literature focusing on electronically monitored adherence (EMA) to DOACs. All studies indexed in EMBASE, Cochrane Library, MEDLINE, Scopus, and Web of Science from inception until September 1, 2023, were searched. Original studies targeting the query topics were included, findings were categorized and

Quantitative Analysis of Diagnostic Reasoning Using Initial Electronic Medical Records.

Background/Objectives: Diagnostic reasoning is essential in clinical practice and medical education, yet it often becomes an automated process, making its cognitive mechanisms less visible. Despite the widespread use of electronic medical records, few studies have quantitatively evaluated how clinicians' reasoning is documented in real-world electronic medical records. This study aimed to investigate whether initial electronic medical records contain valuable information for diagnostic reasoning and assess the feasibility of using text analysis and logistic regression to make this reasoning pr

From Needles to Numbers: Leveraging Electronic Medical Records in Acupuncture Studies.

Acupuncture, a cornerstone of traditional Chinese medicine, is recognized for its therapeutic benefits in chronic pain management. However, its integration into Western medicine is limited. Current research often features small sample sizes and single-center studies, lacking the statistical power for broader acceptance by those unfamiliar with non-Western medicine. This perspective advocates for utilizing electronic medical records (EMRs) to enhance acupuncture research. EMRs compile extensive clinical and demographic data, enabling comprehensive analyses. We present a preliminary review of th

Pediatric Lipid Screening Prevalence Using Nationwide Electronic Medical Records.

IMPORTANCE: Universal screening to identify unfavorable lipid levels is recommended for US children aged 9 to 11 years and adolescents aged 17 to 21 years (hereafter, young adults); however, screening benefits in these individuals have been questioned. Current use of lipid screening and prevalence of elevated lipid measurements among US youths is not well understood. OBJECTIVE: To investigate the prevalence of ambulatory pediatric lipid screening and elevated or abnormal lipid measurements among US screened youths by patient characteristic and test type. DESIGN, SETTING, AND PARTICIPANTS: Th

An artificially intelligent (or algorithm-enhanced) electronic medical record in orofacial pain.

This review examines how a highly structured data collection system could be used to create data-driven diagnostic classification algorithms. Some preliminary data using this process is provided. The data collection system described is applicable to any clinical domain where the diagnoses being explored are based predominately on clinical history (subjective) and physical examination (objective) information. The system has been piloted and refined using patient encounters collected in a clinic specializing in Orofacial Pain treatment. In summary, whether you believe a branching hybrid check-bo

An objective study of the impact of the electronic medical record on outcomes in trauma patients.

It is commonly believed that the electronic medical record (EMR) will improve patient outcomes. However, there is scant published literature to support this claim and no studies in any surgical population. Our hypothesis was that the EMR would not improve objective outcome measures in patients with traumatic injury. Prospectively collected data from our university-based Level I trauma center was retrospectively reviewed. Demographic, injury severity as well as outcomes and complications data were compared for all patients admitted over a 20-month period before introduction of the EMR and a 20-

Using linked electronic medical record-pharmacy data to examine lupus medication adherence: A retrospective cohort study.

INTRODUCTION: Medication nonadherence is common in systemic lupus erythematosus (SLE) and associated with morbidity and mortality. We explored the reliability of pharmacy data within the electronic medical record (EMR) to examine factors associated with nonadherence to SLE medications. METHODS: We included patients with SLE who were prescribed ≥1 SLE medication for ≥90 days. We compared two datasets of pharmacy fill data, one within the EMR and another from the vendor who obtained this information from pharmacies and prescription benefit managers. Adherence was defined by medication possessio

Pharmacological Prophylaxis for Venous Thromboembolism Among Hospitalized Patients With Acute Medical Illness: An Electronic Medical Records Study.

Patients hospitalized with acute medical illness have an elevated risk of venous thromboembolism (VTE). American College of Chest Physicians guidelines list various chronic illnesses, sepsis, advanced age, history of VTE, and immobility as risk factors and recommend prophylactic anticoagulation using fondaparinux, low-molecular weight heparin, or low-dose unfractionated heparin. The objectives of this study were to examine pharmacological prophylaxis against VTE among hospitalized medically ill patients and to assess demographic and clinical correlates related to VTE prophylaxis. A retrospecti

Are patient-reported medical histories enough for safe dental care? A comparison with electronic medical records in dental practice.

OBJECTIVES: Accurate medical information is essential for safe dental treatment planning and for tailoring dental care to the patient's systemic condition. The patient-reported medical history questionnaire is a well-established and recommended practice in dental clinics, guiding the dentist in deciding whether to obtain additional information from the patient's treating physician. We aimed to evaluate the reliability of this approach through comparison with corresponding medical records. METHODS: We retrospectively compared 276 self-reported medical questionnaires with corresponding electron

Using electronic medical records in hospital simulation for infection control intervention assessment.

BACKGROUND: Clinical trials for assessing the effects of infection prevention and control (IPC) interventions are expensive and have shown mixed results. Mathematical models can be relatively inexpensive tools for evaluating the potential of interventions. However, capturing nuances between institutions and in patient populations have adversely affected the power of computational models of nosocomial transmission. METHODS: In this study, we present an agent-based model of ICUs in a tertiary care hospital, which directly uses data from the electronic medical records (EMR) to simulate pathogen

Medical and genetic correlates of long-term buprenorphine treatment in the electronic health records.

Despite the benefits associated with longer buprenorphine treatment duration (i.e., >180 days) (BTD) for opioid use disorder (OUD), retention remains poor. Research on the impact of co-occurring psychiatric issues on BTD has yielded mixed results. It is also unknown whether the genetic risk in the form of polygenic scores (PGS) for OUD and other comorbid conditions, including problematic alcohol use (PAU) are associated with BTD. We tested the association between somatic and psychiatric comorbidities and long BTD and determined whether PGS for OUD-related conditions was associated with BTD. Th

Development and Validation of an Emergency Department Electronic Medical Record Gout Flare Alert.

OBJECTIVE: Patients with acute gout are frequently treated in the emergency department (ED) and represent a typically underresourced and understudied population. A key limitation for gout research in the ED is the timely ability to identify acute gout patients. Our goal was to refine a multicriteria, electronic medical record alert for gout flares and to determine its diagnostic characteristics in the ED. METHODS: The gout flare alert used electronic medical record data from ED nursing notes and was triggered by the term 'gout' preceding past medical history in the chief complaint, the term '
