Early decompressive hemicraniectomy in fulminant herpes simplex encephalitis.

Herpes encephalitis can be a life-threatening condition, despite early instauration of acyclovir treatment. In particular patients may succumb to rapidly progressive cerebral oedema. We report a 66-year patient with a Glasgow Coma Score (GCS) of 6 and incipient uncus herniation of the right temporal lobe on the third day. Decompressive hemicraniectomy was immediately performed. The long-term outcome was satisfactory with unassisted gait and a Barthel Index score of 70 after 9 months.

[Cerebral haematoma in the course of herpes simplex encephalitis].

INTRODUCTION: Herpes simplex encephalitis (HSE) is the most common cause of infectious encephalitis at all ages. The usual presentation includes a high temperature, headache and confusion associated with convulsions and signs of a focal neurological deficiency. The typical findings shown by magnetic resonance imaging (MRI) consist of hyperintense lesions, in T2 and FLAIR sequences, especially in the medial and inferior temporal lobe. AIMS. The aim of this paper is to report a case of HSE that associated a cerebral haematoma (CH) in its clinical course. CASE REPORT: A 69 year old female patien

The mimicking of mania in "benign" herpes simplex encephalitis.

This paper reports on a relative rarity, the case of a 41-year-old male who, after receiving an original admission diagnosis of clear-cut acute mania, actually turned out to be suffering from herpes simplex encephalitis. Interestingly enough, his illness was unusually "benign" in the sense that at no time during massive cycling bipolar mood disturbances did any clinically apparent neurological signs and symptoms or other serious somatic complications or any psychopathology pointing to an acute organic brain syndrome appear. Karl Bonhoeffer's often forgotten but still relevant clinical insights

Neuroretinitis associated with herpes simplex encephalitis in an adult.

A 48-year-old man who died of herpes simplex encephalitis had a bilateral papillitis at autopsy. Intranuclear inclusion bodies were especially numerous within the ganglion cells and inner nuclear layer of the macula and consisted of typical virions by electron microscopy. Contiguous spread from the brain to the eyes may have occurred via the optic nerves. Clinically, the disk and retinal changes were misinterpreted as being caused by papilledema. Papillitis should be included in the differential diagnosis of disk swelling in adults with suspected Herpesvirus hominis infection of the central ne

Analysis of herpes simplex virus type 1 glycoprotein D nucleotide sequence in human herpes simplex encephalitis.

Viral factors responsible for HSV neurovirulence in humans are still unknown. The aim of this work was to investigate the hypothesis that viral variants might contribute to the specific neurovirulence of some HSV strains. HSV-1 DNA was recovered from cerebrospinal fluid (CSF) in ten patients with HSV encephalitis (HSE) and the regions of HSV-1 gD gene corresponding to known antigenic sites were analyzed by direct sequencing of PCR products. Twenty-two mutations were found among a total of 6580 bp analyzed over a portion of 1000 bp of gD gene, of which 20 were silent whereas two conferred amino

Unraveling Herpes Simplex Virus Encephalitis: Diagnosis and Management.

Herpes simplex virus (HSV) encephalitis is a rare but critical neurological condition characterized by acute brain inflammation, primarily affecting the temporal lobes. It is caused by HSVs belonging to either type 1 or type 2. Patients typically present with fever, altered mental status, seizures, and focal neurological deficits. Prompt diagnosis through neuroimaging and cerebrospinal fluid (CSF) analysis is essential for initiating antiviral therapy and improving patient outcomes. This report describes a 39-year-old male who presented with high-grade fever, neck rigidity, headache, vomiting,

Epileptic negative myoclonus in herpes simplex virus encephalitis.

Epileptic negative myoclonus (ENM) is a rarely reported clinical seizure semiology. It is defined as a brief interruption of tonic muscle activity in association with an epileptiform discharge on the EEG (spike or sharp wave) without preceding myoclonia. The diagnosis of ENM requires a polygraphic recording with EEG and surface EMG electrodes covering the affected body part(s), while the muscles have tonic activity. Historically, ENM has been reported in a variety of chronic epilepsy syndromes such as benign epilepsy with centrotemporal spikes, progressive myoclonic epilepsy syndromes, symptom

Recurrent Herpes Simplex Virus Encephalitis After Neurologic Surgery.

BACKGROUND: The herpes simplex virus (HSV) is the most common cause of sporadic encephalitis worldwide. Even with proper treatment, this infection is associated with a mortality rate of 19%-30% and with potential neurologic sequelae. Recurrences of encephalitis are rare and limited to a few cases in the literature. Although the mechanism of reactivation has not yet been clarified, in our patient, the surgery might have acted as a precipitating factor. CASE DESCRIPTION: The case involved a female 10-year-old patient with a history of type 1 HSV encephalitis since 24 months of age. Secondarily,

[Neurosyphilis: important differential diagnosis of herpes simplex encephalitis].

Diagnosis of herpes simplex encephalitis in the acute stage is based on clinical symptoms (nonspecific prodromi, neuropsychological deficits, epileptic seizures) in combination with typical CSF abnormalities (lymphomonozytic pleocytosis) and MR imaging abnormalities assumed to be typical for herpes simplex encephalitis (increased fluid-attenuated inversion recovery and T2 hyperintensities in the mesiotemporal lobe region). Definite diagnosis of herpes simplex encephalitis is based on positive polymerase chain reaction in the CSF, usually available some days after hospital admission. Suspected

Anterior opercular syndrome, caused by herpes simplex encephalitis.

Bilateral frontal and parietal opercular lesions cause a syndrome characterized by paralysis of the masticatory, facial, pharyngeal, and tongue muscles (the anterior opercular syndrome). The anterior opercular syndrome can occur in patients with herpes simplex encephalitis (HSE), but in most of these patients the diagnosis of HSE was not confirmed. We describe the anterior opercular syndrome in four patients with HSE. In two of these patients, the anterior opercular syndrome dominated the clinical picture, but in the other two patients it was overshadowed by other manifestations of HSE. The di

Diffusion MRI in Rasmussen's encephalitis, herpes simplex encephalitis, and bacterial meningoencephalitis.

Three patients with Rasmussen's encephalitis, herpes simplex type 1 encephalitis, and bacterial meningoencephalitis are included in this study. Echo-planar diffusion MRI was acquired with the trace protocol at 1.5 T. b= 1000 s/mm(2) images, and apparent diffusion coefficient (ADC) maps were studied with respect to lesion identification. ADC values were also studied, and compared to those of 25 normals. In Rasmussen's encephalitis b= 1000 s/mm(2) images were uninformative while ADC maps had superior information. In herpes simplex type 1 encephalitis both b= 1000 s/mm(2) images, and ADC maps had…

Evaluation of cholinergic functions in patients with Japanese encephalitis and Herpes simplex encephalitis.

Cognitive and memory impairment are related to cholinergic dysfunction and are important complications of viral encephalitis, In view of paucity of studies on cholinergic dysfunction in encephalitis, this study has been undertaken. We report acetyl choline esterase (AChE) and muscurinic 2 (M2) receptor levels in herpes simplex encephalitis (HSE) and Japanese encephalitis (JE) patients, and correlate these with cognitive functions and MRI findings. Patients with JE and HSE were evaluated for consciousness, neurological and MRI findings, plasma AChE and M2 receptor levels on admission and after
