A double blind multicentre study comparing sodium cromoglycate (600 mg/100 ml) by enema with prednisolone (20 mg/100 ml) by enema is reported. The study was conducted over a nine week period in the treatment of 70 patients with ulcerative colitis. A controlled trial has been carried out in order to compare the efficacy of enemas containing a high dosage of 5-ASA (4 g) versus enemas containing hydrocortisone 100 mg. The trial was conducted on 86 patients, 44 of whom received 5-ASA and 42 received hydrocortisone. The results were favorable in terms of clinical, sigmoidoscopic, and histologic criteria for 5-ASA treatment. Other aspects have been investigated, such as retrograde spread of enemas which have been shown to reach the left colon. No nephrotoxicity was detected. The long term experience confirmed the preliminary positive results.

Rectal enema used for diagnostic and therapeutic purposes infrequently causes colitis. In medical practice, enemas are known to incidentally bring about colitis by mechanical, thermal, or direct chemical injuries. Coffee enema is told to ameliorate the constipation in alternative medicine. We hereby report a case of acute colitis resulting from coffee enema, which was presented with severe abdominal pain and hematochezia.

The previously reported Malone antegrade continence enema (MACE) was used in 21 children for intractable fecal incontinence or constipation. Twelve are completely clean, and three are much improved (71% success rate). However, only four of the 21 have not had some type of complication, either minor or major. Five of the 21 now have a colostomy, and one has abandoned the procedure.

The injuries incident to enemas may vary from oedema of the mucosa to gangrene of the large bowel. The burn of the rectum is very rare, and in mild cases it has an identical course to that of the caustic oesophagitis. The place of corticosteroids, administered by intravenous route or topically, in the therapy of te burn of the rectum is unproven. Healing of the lesion occurs by fibrosis and may result in structure formation, which usually need a treatment only when obstructive symptoms develops.

Radiolabelled enema solution was administered to eight healthy subjects, both in fasted and fed states. Enema spreading was monitored over a 4-h period using gamma scintigraphy and colonic motility was recorded simultaneously using a pressure sensitive radiotelemetry capsule. The rate and extent of enema dispersion were unaffected by eating. Spreading could be correlated with colonic motility and was inhibited by aboral propulsion of the colonic contents.

Manganese chloride, iron glycerophosphate, and cellulose additive were assessed as base materials for use in a T1-shortening single contrast enema for magnetic resonance (MR) colonography. Contrast-to-noise ratios (CNRs) were compared to those with the standard 10 mmol/L gadolinium-based enema. On T1-weighted three-dimensional gradient-recalled-echo images, CNRs with the iron glycerophosphate enema exceeded those with the manganese- and gadolinium-based enemas. The purpose of this article is to describe our findings on barium enema in patients who have had incomplete colonoscopy. For patients undergoing colonoscopy and barium enema within 6-months of each other between 1991 and 1997, barium enemas were reevaluated for redundant anatomy as well as lesions. Seventy-seven women and 30 men underwent 108 barium enemas. The most common finding was a redundant colon in 68% (55/81) of study patients, but only 33% (9/27) of control (complete colonoscopy) patients.
