Endocardial-Epicardial Phase Mapping of Prolonged Persistent Atrial Fibrillation Recordings: High Prevalence of Dissociated Activation Patterns.

BACKGROUND: Endocardial-epicardial dissociation and focal breakthroughs in humans with atrial fibrillation (AF) have been recently demonstrated using activation mapping of short 10-second AF segments. In the current study, we used simultaneous endo-epi phase mapping to characterize endo-epi activation patterns on long segments of human persistent AF. METHODS: Simultaneous intraoperative mapping of endo- and epicardial lateral right atrium wall was performed in patients with persistent AF using 2 high-density grid catheters (16 electrodes, 3 mm spacing). Filtered unipolar and bipolar electrogr

Identification of an epicardial slow conduction channel using ripple mapping in ablation of postinfarct ventricular tachycardia.

An 82-year-old man underwent redo catheter ablation of ventricular tachycardia (VT) after anterior infarction. A ripple mapping conducting channel (RMCC) was identified within the anterior scar in the left ventricular epicardium during sinus rhythm. Along the RMCC, delayed potentials during sinus rhythm, a good pace map with a long stimulus to the QRS interval, and mid-diastolic potentials during VT were recorded, and epicardial ablation at this site eliminated the VT. These findings suggested that the RMCC in the epicardial scar served as a critical isthmus of the postinfarct VT, and ablation

Epicardial Ventricular Tachycardia Ablation Guided by a Novel High-Resolution Contact Mapping System: A Multicenter Study.

Background Mapping using a multipolar catheter with small and closely spaced electrodes has been shown to improve the validity of electrograms to identify endocardial critical sites of reentry isthmus and foci of earliest activation. However, the feasibility, safety, and clinical outcome of using such technology to guide epicardial ventricular tachycardia (VT) ablation has not been reported. Methods and Results Thirty-three consecutive patients from 5 high-volume centers were studied. These patients had 43 epicardial maps using a novel 64-pole mini-basket catheter to guide VT ablation. Activat

Epicardial Intraoperative Three-Dimensional Mapping of Wolff-Parkinson-White Syndrome in a Child With Ebstein's Anomaly.

Catheter ablation of arrhythmias in small patients has inherent significant risk because of the patient, catheter, and lesion size. Heart block, perforation, and coronary damage may occur. This report shows adaptation of a three-dimensional electroanatomic mapping system for intraoperative use in a small patient with Ebstein's anomaly who was undergoing a cone procedure and a bidirectional Glenn procedure. Use of this technique gave clear identification of the epicardial insertion points of two accessory pathways causing Wolff-Parkinson-White syndrome and resistant episodes of supraventricular

High-density epicardial activation mapping to optimize the site for video-thoracoscopic left ventricular lead implant.

BACKGROUND: The left ventricular (LV) lead local electrogram (EGM) delay from the beginning of the QRS complex (QLV) is considered a strong predictor of response to cardiac resynchronization therapy. We have developed a method for fast epicardial QLV mapping during video-thoracoscopic surgery to guide LV lead placement. METHODS: A three-port, video-thoracoscopic approach was used for LV free wall epicardial mapping and lead implantation. A decapolar electrophysiological catheter was introduced through one port and systematically attached to multiple accessible LV sites. The pacing lead was ta…

Transatrial pericardial insufflation of carbon dioxide to facilitate epicardial mapping and ablation of atrial and ventricular arrhythmias.

BACKGROUND: Epicardial access (EA) is an essential tool for ablation of certain arrhythmias, but conventional techniques carry inherent risks. Carbon dioxide (CO2) insufflation in the pericardium through the coronary sinus aims to facilitate EA but is time-consuming and not widely used. Transatrial pericardial insufflation of CO2 (TAPI-CO2) offers a simplified alternative, underexplored in electrophysiology procedures. OBJECTIVE: This study aimed to evaluate the efficacy and safety of TAPI-CO2 for EA in electrophysiology procedures. METHODS: This retrospective study included all patients und

Direct Epicardial Mapping and Ablation Revealing Three-Dimensional Roof-Dependent Macroreentrant Atrial Tachycardia Breaking Into Bachmann's Bundle.

Catheter ablation for macroreentrant atrial tachycardia (AT) that recurs after atrial fibrillation (AF) ablation is occasionally difficult because of epicardial connections. Here, we report the case of a patient who underwent ablation for recurrent AT after AF ablation. Endocardial mapping of the left atrium (LA) showed a centrifugal pattern from the base of the appendage, and the LA roof showed discontinuous activation from the posterior LA. Direct epicardial mapping was performed, revealing the roof-dependent macroreentrant AT using the epicardial connection from the LA posterior to the Bach…

Epicardial mapping and ablation of ventricular tachycardia from the coronary venous system in post-coronary bypass patients.

BACKGROUND: Ventricular tachycardia (VT) ablation of mid- or epicardial substrate is difficult and requires a creative approach in patients with a history of coronary bypass that precludes percutaneous epicardial catheter manipulation. The coronary venous system (CVS) provides limited access to the epicardial surface of the heart. The objective of this study is to assess the feasibility, safety, and efficacy of epicardial mapping and ablation of VT substrates from the CVS in patients with history of coronary bypass. METHODS: Patients undergoing VT ablation at our institution were retrospectiv…

Simultaneous Non-Invasive Epicardial and Endocardial Mapping in Patients With Brugada Syndrome: New Insights Into Arrhythmia Mechanisms.

BACKGROUND: The underlying mechanisms of Brugada syndrome (BrS) are not completely understood. Recent studies provided evidence that the electrophysiological substrate, leading to electrocardiogram abnormalities and/or ventricular arrhythmias, is located in the right ventricular outflow tract (RVOT). The purpose of this study was to examine abnormalities of epicardial and endocardial local unipolar electrograms by simultaneous noninvasive mapping in patients with BrS. METHODS AND RESULTS: Local epicardial and endocardial unipolar electrograms were analyzed using a novel noninvasive epi- and e

Larger low voltage zone in endocardial unipolar map compared with that in epicardial bipolar map indicates difficulty in eliminating ventricular tachycardia by catheter ablation.

Patients with ischemic and non-ischemic cardiomyopathy often have substrate for ventricular tachycardia (VT) in the endocardium (ENDO), epicardium (EPI), and/or intramural. Although it has been reported that the ENDO unipolar (UNI) voltage map is useful in detecting EPI substrate, its feasibility to detect intramural scarring and its usefulness in radiofrequency catheter ablation (RFCA) remain unclear. To assess the relationship between the left ventricle (LV) ENDO UNI voltage map and the LV EPI bipolar (BIP) voltage map, and to determine the usefulness of the ENDO UNI voltage map to guide RFC

The effect of electrode density on the interpretation of atrial activation patterns in epicardial mapping of human persistent atrial fibrillation.

BACKGROUND: Mechanisms sustaining human persistent atrial fibrillation (AF) remain debated, with significant differences between high-density epicardial and global endocardial mapping studies. A key difference is the density of recording electrodes. OBJECTIVE: We aimed to determine the differences in the prevalence of different atrial activation patterns, and specifically in the prevalence of rotational activations, with varying densities of bipolar electrodes. METHODS: Epicardial mapping was performed in 10 patients undergoing cardiac surgery, with bipolar electrograms recorded using a tria…

Epicardial mapping during induction of nonsustained polymorphic ventricular tachycardia in a 7-day-old canine myocardial infarction model.

Programmed electrical stimulation and epicardial mapping were performed to study electrophysiologic mechanisms for induction of nonsustained polymorphic ventricular tachycardia in 29 dogs with 7-day-old myocardial infarction. Nine dogs had 15 different episodes of reproducible monomorphic ventricular tachycardia (11 sustained and 4 nonsustained) (group A). Seven episodes of polymorphic ventricular tachycardia (group B) were induced in six dogs, three of which degenerated into ventricular fibrillation. Epicardial activation time during premature stimulation at the time of tachycardia induction …

Noninvasive 3D Mapping and Ablation of Epicardial Premature Ventricular Contractions From the Endocardial Aspect of the Left Atrial Appendage.

Ablation is an established treatment for ectopy originating from the left ventricle (LV). We report on a case of noninvasive 3-dimensional mapping locating the origin precisely in the epicardial LV summit area. However, after failed attempts from LV and epicardially, ablation via the left atrial appendage was finally successful. (Level of Difficulty: Advanced.).
