16-days old rats were operated with either uni- or bilateral ligation of ductuli efferents and separation of testis and epididymis to the level of the inferior epididymal artery (non-union operation), induction of cryptorchidism or bilateral sham operation. A 72-year-old man presented with left scrotal swelling. The patient was diagnosed with left testicular hydrocele and underwent the hydrocele aspiration. However, it recurred within a short period. Magnetic resonance imaging (MRI) revealed a tumor in the epididymis and the patient then underwent left high orchiectomy. The histopathological examination revealed an adenocarcinoma of the epididymis. Ten months after the surgery, the patient has been free of disease.

Tumors of the epididymis, both primary and secondary, and whether benign or malignant, are very rare. Adenomatoid tumors and leiomyoma are the most frequently diagnosed benign tumors of the epididymis. In a review of the American and European literature, leiomyoma was the second most common neoplasm of the epididymis, representing 6% of primary epididymal tumors. A case is presented of Wegener's granulomatosis limited to the epididymis in a 32-year-old man. Tests for antineutrophils cytoplasmic antibodies were negative throughout. He showed excellent response to treatment with prednisolone, azathioprine and cotrimoxazole, following recurrence of his disease, after local complete excision. He remains free of disease 40 months after the discontinuation of all his treatment.

A 17-year-old man visited our hospital with the chief complaint of painless swelling of the left scrotal content. An elastic hard mass was palpable at the upper pole of the left testis.
