This article reports a case of colovesical fistula presenting with epididymitis. A 75-year-old man with a recent conservatively managed localised diverticular perforation presented to hospital with acute pain and swelling of his left testicle and epididymis. On further questioning, the patient reported passing air in his urine. Urine cultures grew Enterococcus faecalis. Epididymitis and orchitis are commonly seen in the outpatient setting. Men between 14 and 35 years of age are most often affected, and Chlamydia trachomatis and Neisseria gonorrhoeae are the most common pathogens in this age group. In other age groups, coliform bacteria are the primary pathogens. A 45-year-old man with spinal injury and diabetes mellitus who complained high fever and progressive enlargement of left intrascrotal mass visited to our hospital. PURPOSE: Amiodarone is an antiarrythmic agent, which is often successfully used when all other antiarrythmics have failed. We present a case of scrotal vasculitis, of difficult diagnosis despite the patient's history, where it was necessary to resort to surgery in order to establish definitely its diagnostic differentiation with a specific epididymitis. Taking advantage of this uncommon form of presentation of scrotal vasculitis, in this case an evolved Schonlein-Henoch syndrome in a 35-year-old patient, we analyse the different forms of gonad, epididymis and scrotal affectation recognised in cases of vasculitis, with special reference to the forms of acute presentation, simulating testicle torsion.

Complications which prolong hospitalization occur after 10% of prostatectomies, depending on the type of operation. Epididymitis develops after prostatectomy especially in postoperative urinary tract infection, and in patients with risk factors (retention of urine, recurrent residual urine, infected urine, and after adenomectomy). In comparison with the time before specific antibiotic therapy, postoperative epididymitis has fallen by 50% with preliminary antibiotic treatment. Postoperative epididymitis very rarely occurs under treatment with gentamicin.

A case of tuberculous epididymitis which presented as an acute hydrocele is reported and the relevant literature reviewed. The patient had undergone contralateral orchidectomy 20 years previously for the same condition associated with pulmonary tuberculosis. The most recent episode followed unrelated illness; the diagnosis was confirmed after surgical intervention and cure was achieved with adjuvant chemotherapy. Although rare, genital tuberculosis should still be considered in cases of scrotal swelling when there is a history of previous tuberculous disease or exposure.

In 21 men suffering from acute or chronic epididymitis who underwent scrotal surgical exploration, diagnostic microbiological studies consisting in cultures of the epididymal tissues and mid-stream urine specimens, as well as examination of the urethral swabs--before and after prostatic massage--were performed. Epididymitis is an uncommonly recognized complication of systemic necrotizing vasculitis. We report 2 cases in which epididymitis heralded the onset of more severe visceral organ involvement. Early biopsy of atypical epididymitis in patients with hypertension or constitutional symptoms is important if effective therapy is to be instituted before irreversible organ damage occurs.
