The incidence of epiglottic cysts in a cohort of adults with acute epiglottitis.

OBJECTIVE: To evaluate the effects of a coexisting epiglottic cyst on the clinical course and airway management of acute epiglottitis in adults. A review of the airway risk factors were studied in adults with acute epiglottitis in South Korea from 1997 to 2009. DESIGN: Retrospective chart review. SETTING: Academic tertiary care referral medical center. PARTICIPANTS: The study included 117 adult patients that were diagnosed with acute epiglottitis laryngoscopically over a period of 12 years. Two distinct groups were identified: one with acute epiglottitis associated with an infected epiglott…

Acute epiglottitis: MRI.

We report the MRI findings in an adult with epiglottitis. There was thickening of the epiglottis and left aryepiglottic fold. Management of this life-threatening condition requires imaging only when the diagnosis is uncertain or when an abscess or other complication is suspected.

Adult acute epiglottitis.

Acute epiglotiitis is a disease of rapid onset and progression of symptoms and has been well described in children. The importance of being aware of this diagnosis is to prevent an acute upper airway obstruction that can be potentially fatal. Here we describe two cases of adult acute epiglottitis where the patients had severe symptoms of sorethroat but a normal looking oropharynx.

Pasteurella multocida epiglottitis.

Pasteurella multocida, a small gram-negative coccobacillus, colonizes the nasopharynx and gastrointestinal tract of many animals, including cats and dogs. Most human infections with P multocida are due to animal bites, but the respiratory tract is the second most common site of infection. We describe the third case report (to out knowledge) of acute P multocida epiglottitis. The mode of transmission in this case was inhalation of infectious nasopharyngeal secretions from cats. The patient responded well to treatment with penicillin, the drug of choice for P multocida infections. Therefore, inf…

Epiglottitis in adults.

Acute epiglottitis may be more common in adults than is generally believed, but the diagnosis is often missed. Three cases of adults with epiglottitis are reported. The presenting signs and symptoms included sore throat, dysphagia, symptoms disproportionate to pharyngeal findings, and tenderness over the neck anteriorly. All three patients had a benign course, but acute upper airway obstruction can occur. The diagnosis was established in each case by mirror or flexible fiberoptic laryngoscopy, lateral neck radiographs, or both. Treatment consisted of maintenance of a patent airway and administ…

Epiglottitis and laryngotracheobronchitis.

Epiglottis and laryngotracheobronchitis are potentially life-threatening infections in the pediatric population. Unlike epiglottitis, laryngotracheobronchitis is very common. While both diseases may present with stridor and signs of respiratory obstruction, it is important to distinguish between them because the management strategies differ. Laryngotracheobronchitis usually responds to humidification. Epiglottitis requires an artificial airway and intensive antibiotics. The use of steroids remains controversial.

Epiglottic abscess.

BACKGROUND: Epiglottitis is more commonly seen in children less than 6 years of age, although this entity has also been well described among adults. A coalescence of infection of the epiglottis, or epiglottic abscess, has been infrequently reported in series of epiglottitis. Risk factors for epiglottic abscess include adult age at onset, diabetes, and the presence of a foreign body. METHODS: Case study. RESULTS: We present a case of a woman with a 4-day history of febrile illness, odynophagia, and an altered voice. Clinical examination and computed tomography (CT) demonstrated an epiglottic …

Epiglottitis in the adolescent.

Acute epiglottitis is rare in adolescents. Respiratory compromise may not occur early. Physicians should consider this diagnosis in their adolescent patients who complain of pain and dysphagia, with or without visible pharyngitis. Airway maintenance is paramount. The antibiotic used should include coverage for Hemophilus influenzae, type B. The case presented here illustrates these points.

Pasteurella multocida acute epiglottitis.

INTRODUCTION: Apart from cases related to direct inoculation, pasteurellosis is a rare opportunistic infection occurring in predisposed subjects. Close contact with domestic animals, usually cats, is generally reported. Localized ENT forms are possible and are due to oropharyngeal carriage. CASE REPORT: We present the case of a patient with no notable history, who presented with laryngeal dyspnea and hyperthermia leading to a diagnosis of acute epiglottitis. Bacteremia was detected and blood cultures were positive for Pasteurella multocida. Treatment consisted of the standard treatment for ac…

Death related to epiglottitis.

Although death due to epiglottitis is well-reported in the medical literature, because of vaccines and antibiotics, deaths caused by epiglottitis are rare in the era of modern medicine. This report presents a case of epiglottitis-related death occurring in a middle-aged diabetic man. He initially presented to an emergency department with complaints of a sore throat and bilateral ear pain. Although a quick test for Strep pneumoniae was negative, the work-up was not extensive enough to exclude epiglottitis. He was discharged with a prescription for a decongestant and instructed to drink plenty o…

Acute epiglottitis during pregnancy.

A 26-year-old woman in her third trimester of pregnancy had stridor. Epiglottitis was diagnosed by fiberoptic laryngoscopy. Staphylococcus aureus was the predominant organism isolated from the laryngeal aspirate. Early intubation proved effective in managing this potentially life-threatening disease. Although pharyngitis is the most common cause of sore throat in the adult, acute epiglottitis must be considered in the differential diagnosis when there is unrelenting throat pain and minimal objective signs of pharyngitis. An early diagnosis with aggressive airway management can be life saving t…

[Tuberculous forms of epiglottitis].

This paper discusses three observations of atypical forms of tuberculosis epiglottitis. In two patients, clinical manifestations were similar to abscess formation of acute epiglottitis and in one patient productive inflammation of the epiglottis and its ulceration resembled epiglottis carcinoma. In all the patients these were advanced pathologies that developed together with pulmonary tuberculosis. This indicates that ENT doctors must be very careful when treating patients with atypical epiglottis that shows resistance to anti-inflammatory therapy.

[Treatment of acute epiglottitis].

We analyze 13 children between 1 and 3 years old (mean: 24.6 +/- 6.5 months), who were diagnosed (by direct or radiological examination) of epiglottitis. Seven were male. All of them had fever and respiratory distress. Six blood cultures (46%) were positive for Hemophilus Influenzae, and in 5 cases were resistant to beta-lactamases. Diagnose was made by radiology (70%) or by direct examination (30%). Five patients were not intubated (38%), with a favorable outcome. One died after a cardiorespiratory arrest due to self-extubation. We describe in this study our experience in the conservative tre…
