Estriol containing cream for treatment of climacteric urogenital complaints was used. After 4 weeks local treatment with Ovestin cream atrophy of vaginal epithelium and chronic vaginitis stopped or significantly decreased. During the treatment the ratio of superficial and intermedier cells in the vaginal epithelium increased and the vagina showed a decrease of pH. The subjective complaints relating to the estrogen deficiency (vaginal burning and dryness, itching, dyspareunia and urinary dysfunctions) ceased. BACKGROUND: As an estrogen derivative, estriol is rather effective in the relief of climacteric symptoms due to estrogen deficiency. When given one dose a day, it will not provoke endometrial proliferation and shedding. Thus, it is suitable for postmenopausal women who no longer want to have uterine bleeding and for those with comparatively higher risk of endometrial hyperplasia. The third published case of human placental lactogen (hPL) deficiency in a normal pregnancy is reported. The results of the 3 cases are discussed. In all cases, estriol levels were normal and hPL levels were either unmeasurable or below 1 microgram/ml. The placenta showed no obvious abnormalities. Growth hormone and prolactin determinations did not contribute to the understanding of the deficiency; neither did the glucose levels in the maternal blood. All 3 infants were male.

A radioimmunoassay for serum unconjugated estriol in the menstrual cycle with a sensitivity of about 5 pg/ml is described. 8 cycles were studied. In 2 cycles, single spikes of 22 and 30 pg/ml were obtained. In 3 cycles, concentrations of 4-5 pg/ml were found whereas in the other 3 studies, no estriol was detected. In general, peaks of estriol corresponded to peaks in estradiol plus estrone. Patients in 4-12 weeks of gestation were also studied. This paper reported the determination of urine E3 in 326 normal pregnancies from 28 to 41 gestational weeks, and 80 abnormal pregnancies by E3-125I-RIA. The results showed that E3 levels in normal pregnancies increased with advance of gestational weeks with their peaks at the 38th week and decreased gradually after the 40th week. There was a positive correlation between urine E3 and the 12 hour urine E/C ratio (r = 0.798, P less than 0.001). In 901 pregnancies between 1978-1981 a total of 5281 serum estriol determinations were performed. 596 gravidas were high-risk patients (diabetes mell., toxemia, IUGR, premature labour, prolonged pregnancy, Rh incompatibility). In 142 of these high-risk pregnancies the estriol values were below the 10th percentile or the estriol decrease extended 40%. These cases were analysed individually, in order to answer the question, how often serum estriol values were decisive for active obstetrical management.
