The patient who could not be discharged. How far should patient autonomy extend?

A male patient was admitted to the acquired immune deficiency syndrome (AIDS) unit for hemodialysis. His history revealed that he was homeless and that he had tested positive for human immunodeficiency virus (HIV+). He also had a history of alcohol and intravenous drug abuse and tuberculosis. Based on the results of a chest X-ray, he was placed in respiratory isolation. During the next few days of his hospitalization, he exhibited nonadherent behavior toward the treatment regime. Because of previous verbal and physical abuse to staff and patients, all local hemodialysis centers refused to acce

Infectious diseases in the workplace: pointers for an ethical management policy.

Infectious diseases in the workplace can present difficult dilemmas for employers, who must balance the rights of infected employees against obligations to protect other employees from infection. Anti-discrimination legislation imposes additional obligations on employers to ensure that any steps taken in response to the risk of infectious disease do not amount to unlawful discrimination against employees who may be disease carriers. This paper analyses the operation of anti-discrimination in this context and points to ways in which employers can formulate an infectious diseases policy that is

Anencephaly and the interruption of pregnancy: policy proposals for HECs.

This article is directed to hospital communities where absolute proscriptions exist against abortion. The assumption is that many will be Catholic institutions and therefore Catholic moral teachings are examined carefully. By addressing anti-abortion arguments in the context of anencephaly, I hope to show that within even the most conservative moral traditions, humane exceptions to absolute proscriptions against abortion can be justified. More specifically, the article is directed to HECs which either have unstated or ambiguous policies on abortion. It will attempt to show what a reasonable po

Incidence of clinical malaria and asymptomatic parasitaemia among African children living with sickle cell disease: a multicentre prospective longitudinal cohort study protocol.

INTRODUCTION: Sickle cell disease is a monogenic blood disorder prevalent in Africa, where malaria poses a health risk for affected individuals. Individuals with sickle cell disease are susceptible to malaria, which can be complicated by vaso-occlusive episodes, severe anaemia and death. Understanding the intersection of malaria and sickle cell disease in African countries with high co-prevalence is essential for developing effective management and prevention strategies, including clinical guidelines on the use of malaria chemoprophylaxis in this population. This study will determine the month

CALME: A Cognitive Loop for Ethical Deliberation During Clinical Action.

RATIONALE, AIMS AND OBJECTIVES: Most established approaches to clinical ethics support deliberation before or after care, through processes such as informed consent, institutional guidance, ethics consultation and retrospective case review. Yet ethically significant decisions often arise during clinical action, when clinicians must respond under time pressure, manage competing technical demands and navigate asymmetries of knowledge and power. Although the literature on everyday ethics shows that routine practice is ethically dense, it does not provide a compact reasoning structure designed for

Protocol for a cervical screening implementation trial comparing two approaches for delivering HPV self-collection in low-resource settings in India: a type 3 hybrid cluster randomised controlled trial (SHE-CAN).

BACKGROUND: Although multiple studies have offered self-collection for human papillomavirus (HPV)-based cervical screening in community settings, there are no randomised controlled trials (RCTs) that have compared implementation outcomes of programme approaches for self-collection. This trial will compare two such approaches in low-resource settings in the states of Tamil Nadu and Mizoram, India. METHODS: A cluster RCT will be conducted over a year, offering self-collection to 3000 women aged 30-49 from 28 clusters (average size 101) in selected districts. Clusters in tribal, rural and urban

Is there a role for ethics in addressing healthcare incivility?

In a healthcare setting, a multitude of ethical and moral challenges are often present when patients and families direct uncivil behavior toward clinicians and staff. These negative interactions may elicit strong social and emotional reactions among staff, other patients, and visitors; and they may impede the normal functioning of an institution. Ethics Committees and Clinical Ethics Consultation Services (CECSs) can meaningfully contribute to organizational efforts to effectively manage incivility through two distinct, yet inter-related channels. First, given their responsibility to promote a

The forms and limits of medical ethics.

As medical ethics has evolved over the past several decades, it has come to be regarded as a domain of applied ethics, that is, the application of a rationally based, philosophical theory to moral problems in health care. But an array of difficulties arise in the attempt to apply general moral theories or norms to concrete problems, difficulties that expose the incompleteness and indeterminacy of philosophical moral theory. The doubtful ability of applied ethics to be practically helpful has led to the development of two main competitors. One is the attempt to reprise and rehabilitate the trad

Changing "Hats": Benefits, Challenges, and Best Practice Proposals for Dual-Role Clinical Ethicists.

Clinical ethics consultation has become increasingly professionalized, yet many ethics consultants continue to maintain active clinical practice as physicians, nurses, or social workers. Despite the prevalence of these dual-role clinical ethicists, the benefits, challenges, and best practices associated with this model remain underexplored. Drawing on the experiences of physician-, nurse-, and social worker-ethicists, we examine how dual-role ethicists approach the intersection of clinical care and ethics consultation. We highlight key strengths of the dual-role model, including enhanced clini

The Potential Benefits of Research May Justify Certain Research Risks.

US regulations allow institutional review boards to approve pediatric clinical trials only when the risks are minimal or (in some cases) a minor increase over minimal, or when the risks are justified by a potential for direct benefit to the participants. But how should an institutional review board determine if the risks of pediatric clinical trials are justified by a potential for participant benefit? In this Ethics Rounds article, we consider which potential benefits can justify which research risks with a focus on randomized clinical trials.

The WHO Guidance on Clinical Ethics: Some concerns from the Indian context.

The World Health Organization (WHO) released a draft guidance on clinical ethics in May 2025. This document covers education and capacity building in clinical ethics, the setting up of clinical ethics services in institutions, and the development of policies to implement clinical ethics review at institutional, national and international levels. The guidance is promising, but there are important contextual issues in India, raising concerns about its adoption and effective implementation. From the experience with research ethics committees, we know that despite more than a decade of mandating e

Long-term care survey reveals challenges. Facilities grapple with five broad issues, including changes in leadership and AIDS.

The Catholic Health Association's 1992 survey of Catholic long-term care (LTC) facilities identified five broad issues LTC facilities face in the 1990s: leadership, system affiliation, community programs, resident issues, and care of persons with AIDS. The transition to lay leadership presents new challenges to the relationship between LTC facilities and their sponsors. Despite the dominance of religious sponsors, an increasing number of laypersons are serving as healthcare administrators both in long-term and acute care. Thirty percent of respondents reported being affiliated with a multi-ins

A call for structured ethics appendices in social science papers.

Ethics in social science experimentation and data collection are often discussed but rarely articulated in writing as part of research outputs. Although papers typically reference human subjects research approvals from relevant institutional review boards, most recognize that such boards do not carry out comprehensive ethical assessments. We propose a structured ethics appendix to provide details on the following: policy equipoise, role of the researcher, potential harms to participants and nonparticipants, conflicts of interest, intellectual freedom, feedback to participants, and foreseeable

The Other Side of the Stethoscope: Clinician-Facing Deepfakes and the Symmetry of Synthetic Deception.

BACKGROUND: Clinical ethics scholarship on deepfakes has focused primarily on patients as targets of synthetic deception, in which fabricated audiovisual material alters patient beliefs about clinicians and care. This focus neglects the reverse epistemic vulnerability: clinicians themselves as recipients of synthetic, adversarially generated clinical information. OBJECTIVE: To develop a conceptual analysis of clinician-facing deepfakes and their implications for clinical epistemology, diagnostic reasoning, and institutional trust. METHODS: Normative and conceptual analysis drawing on virtue

Potential Issues in Mandating a Disclosure of Institutional Investigation in Retraction Notices.

A retraction notice is a formal announcement for the removal of a paper from the literature, which is a weighty matter. Xu et al. (Science and Engineering Ethics, 29(4), 25 2023) reported that 73.7% of retraction notices indexed by the Web of Science (1927-2019) provided no information about institutional investigations that may have led to the retractions, and recommended that Committee on Publication Ethics (COPE) retraction guidelines should make it mandatory to disclose institutional investigations leading to retractions in such notices. While this recommendation would add to the transpare

Consensus, clinical decision making, and unsettled cases.

The model of clinical ethics consultation (CEC) defended in the ASBH Core Competencies report has gained significant traction among scholars and healthcare providers. On this model, the aim of CEC is to facilitate deliberative reflection and thereby resolve conflicts and clarify value uncertainty by invoking and pursuing a process of consensus building. It is central to the model that the facilitated consensus falls within a range of allowable options, defined by societal values: prevailing legal requirements, widely endorsed organizational policies, and professional standards of practice and
