An Atypical Case of Exophiala Bergeri Chromoblastomycosis in an Immunosuppromised Individual.

Chromoblastomycosis (CBM) is a rare chronic, granulomatous mycosis affecting the skin and subcutaneous tissue. It is a deep, slow growing, fungal infection that most often presents with epidermal changes that clinically appear as a hypertrophic and verrucous plaque. We describe an atypical presentation of an immunocompromised male patient, age 76 years, who presented with a 4-month history of painless subcutaneous firm nodules of the left fifth finger. The fungal culture grew Exophiala bergeri, and histopathology showed medlar bodies with lack of pseuodepitheliomatous hyperplasia or epidermal …

Cutaneous Phaeohyphomycosis Caused by Exophiala attenuata in a Domestic Cat.

A 7-year-old female-spayed, domestic short-haired cat was presented to her veterinarian with a mass on the hind paw. Histopathologic examination of a tissue biopsy revealed nodular pyogranulomatous panniculitis with intralesional pigmented fungal hyphae. A dematiaceous fungal isolate was isolated with a micromorphological phenotype consistent with the anamorphic genus Exophiala: budding cells, torulose mycelium and annellidic conidiogenesis from simple conidiophores consisting of terminal and lateral cells that tapered to a short beak at the apex. Sequence homology of the internal transcribed …

Subcutaneoous phaeohyphomycosis by Exophiala jeanselmei in a cardiac transplant recipient.

We report a case of phaeohyphomycosis caused by Exophiala jeanselmei in a cardiac transplant recipient maintained on immunosuppressive therapy with mycophenolate mofetil tacrolimus and prednisone. The lesion began after trauma on the right leg that evolved to multiple lesions with nodules and ulcers. Diagnosis was performed by histological examination and culture of pus from skin lesions. Treatment consisted of itraconazole (200 mg/day) for three months with no improvement and subsequently with amphotericin B (0.5 mg/Kg per day to a total of 3.8 g intravenously). After four months of treatment…

Exophiala angulospora infection in hatchery-reared lumpfish (Cyclopterus lumpus) broodstock.

Samples from moribund lumpfish were collected in a marine hatchery in Scotland in 2015. Black nodules were noted on the skin, and gills and fungal hyphae were extensively distributed in musculature and internal organs. Multifocal chronic inflammatory lesions displaced structures in all affected organs. Mortalities commenced on completion of spawning in May and were evenly distributed over the second year in the temperature range 11-15°C. The main systemic infection causing agent was initially identified based on morphological characteristics as an Exophiala species. Ribosomal DNA (rDNA) ITS re…

Exophiala (Wangiella) dermatitidis and cystic fibrosis - Prevalence and risk factors.

The objective of this prospective study was to assess the prevalence of Exophiala dermatitidis in respiratory secretions of patients with cystic fibrosis (CF) and to identify risk factors for its presence. The results of all cultures performed over a 2-year period in non lung-transplant patients in our CF clinic were included in the study. Samples consisted of sputum (whenever possible) or deep pharyngeal aspirate after a session of physiotherapy. Specimens were inoculated onto Sabouraud gentamicin-chloramphenicol agar (SGCA) medium (Becton-Dickinson) and incubated at 35°C for 2 days and then …

Subcutaneous phaeohyphomycosis caused by Exophiala jeanselmei in collagen disease patient.

Phaeohyphomycosis is a rare fungal infection that is more commonly associated with compromised patients. We present herein an 81-year-old man with collagen disease and chronic interstitial pneumonia who developed subcutaneous phaeohyphomycosis caused by Exophiala jeanselmei. The main pathogen of phaeohyphomycosis had been considered to be E. jeanselmei complex. This has recently been divided into several species by using a molecular technique. The main pathogen of phaeohyphomycosis is Exophiala xenobiotica, and E. jeanselmei is rather a rare pathogen of this disease. Although p.o. itraconazole…

Human infection caused by Exophiala pisciphila: case report and review.

One year after receiving a liver transplant and 2 months after treatment with high doses of steroids and monoclonal anti-CD3 for an episode … ms. Culture of material obtained at reexcision yielded a dematiaceous fungus that was identified as Exophiala pisciphila. No evidence of dissemination was found. This represents a unique report of human infection with this fungus, a well-recognized pathogen of fish. Except for the absence of sclerotic bodies, the clinicopathologic features resembled those o …

Exophiala dermatitidis pneumonia successfully treated with long-term itraconazole therapy.

Exophiala dermatitidis pneumonia is extremely rare. Here we report a case of E. dermatitidis pneumonia successfully treated with long-term itraconazole therapy. A 63-year-old woman without a remarkable medical history developed a dry and chest pain. Chest radiographs revealed consolidation in the middle lobe of the lung. Cytologic examination by bronchoscopy showed filamentous fungi and E. dermatitidis was detected in the bronchoalveolar lavage fluid. After 5 months of itraconazole therapy, her symptoms improved and the area of consolidation diminished. Two weeks after discontinuing the itraco…

Orbital abscess caused by Exophiala dermatitidis following posterior subtenon injection of triamcinolone acetonide: a case report and a review of literature related to Exophiala eye infections.

BACKGROUND: Subtenon injection of triamcinolone acetonide (STTA) has been widely adopted in the clinical setting of ophthalmology and its infectious complications are rare. However, orbital abscess following STTA has been reported in seven cases. Furthermore, although eye infections due to Exophiala species are uncommon, there have been 19 cases to date. E. jeanselmei, E. phaeomuriformis, E. werneckii, and E. dermatitidis have been reported to cause human eye infections; however, to the best of our knowledge, orbital abscess caused by E. dermatitidis has not yet been reported. We describe the …

Exophiala asiatica, a new species from a fatal case in China.

We describe a new species, Exophiala asiatica, isolated from an infection of the pharynx in a 20-year-old, immunocompetent woman in Nanjing, China. The infection was initiated by a fishbone prick in the pharynx, soon developed with facial nodules but subsequently seemed to have disappeared. Tonsil ulceration with progressive soreness of the pharynx was observed 3 years later. Dysphagia, headache and paralysis occurred four years after first signs of infection. Hyphae and yeast-like cells were detected in tissue and a black fungus was recovered repeatedly from pharynx tissue. Despite antifungal…

[Subcutaneous phaeohyphomycosis due to Exophiala spinifera in a renal transplant recipient].

INTRODUCTION: Among the dematiaceous fungi responsible for human or animal phaeohyphomycosis, the Exophiala genus is a well-known etiologic agent and presently includes nine species considered as opportunist pathogens. To our knowledge, Exophiala spinifera has been reported as causative agent of only thirteen cases of cutaneous or systemic phaeohyphomycosis. We describe some typical phaeohyphomycotic cysts. CASE-REPORT: A 59 year-old female renal transplant recipient, treated with ciclosporine and prednisone, presented with two painless nodular and suppurative lesions of the leg, extending sl…

A Suspected Case of Allergic Bronchopulmonary Mycosis Associated With Exophiala dermatitidis.

Exophiala dermatitidis is a black fungus that can colonize the airways, but allergic bronchopulmonary mycosis (ABPM) associated with E. dermatitidis is rare. An 83-year-old former smoker without asthma presented with exertional dyspnea. Chest CT showed bronchiectasis and a mucus plug in the left B3 bronchus. One month after discharge for acute cholecystitis, he returned with cough. Chest CT showed enlargement of the mucus plug and an infiltrative shadow in the left upper lobe. Despite piperacillin/tazobactam therapy for suspected bacterial pneumonia, cough and radiological findings persisted. …

Microdilution in vitro antifungal susceptibility of Exophiala dermatitidis, a systemic opportunist.

The in vitro activities of eight antifungal agents were determined against clinical (n = 63 genotype A, n = 3 genotype B) and environmental (n = 2 genotype A, n = 13 genotype B) strains of Exophiala dermatitidis. The resulting MIC(90)s for all strains (N = 81) were, in increasing order, as follows: posaconazole, 0.125 μg/ml; itraconazole, 0.25 μg/ml; voriconazole, 0.5 μg/ml; amphotericin B, 0.5 μg/ml; isavuconazole, 1 μ/ml; caspofungin, 8 μg/ml; anidulafungin, 8 μg/ml and fluconazole, 16 μg/ml. There were no significant differences in the patterns of susceptibility between genotypes A and B, e…

[Exophiala species--a rare cause of endocarditis with cerebral abscess formation].

Exophiala species belong to the dematiaceous fungi. Occurring worldwide, they are a rare cause of human infection. We present the case of a 75-year-old immunocompetent patient with a cardiogenic embolic anterior cerebral artery infarction. Echocardiography revealed endocarditis of the aortic valve. Antibiotic therapy was initiated after susceptibility testing (blood cultures were positive for Staphylococcus aureus, and liquor was normal), with a subsequent fall in serologic markers of infection and resolution of the signs of endocarditis on echocardiography. However, 6 weeks after initiation o…
