Delayed Presentation of Gluteal Compartment Syndrome: The Argument for Fasciotomy.

A male patient in his fifties presented to his local hospital with numbness and weakness of the right leg which left him unable to mobilise. … as made the following day and a diagnosis of gluteal compartment syndrome (GCS) was made. Emergency fasciotomy was performed 56 hours after the onset of symptoms. There was immediate neurological improvement following decompression and the patient was rehabilitated with complete nerve recovery and function at eight-week follow-up. This is the first documente …

A Single-Incision Fasciotomy for Compartment Syndrome of the Lower Leg.

Lower leg fasciotomy may be performed with a single lateral incision with or without fibulectomy, or by a double incision technique, with most surgeons preferring the later. The aim of this article is to describe a variation of an existing single-incision technique that will allow for the release of all four compartments with 1 incision. Our approach uses a paratibial route to release the deep posterior compartment (DPC) rather than a transfibular or parafibular route that has already been discussed in the literature. We approach the DPC from the anterior compartment, whereas the parafibular a…

Decompressive fasciotomy in testicular torsion of the spermatic cord in rats.

PURPOSE: To evaluate the effect of decompression fasciotomy on testes undergoing torsion of the spermatic cord. METHODS: Spermatic cord torsion was induced in 40 Wistar testes for six and 12 hours. At the time of distortion, decompression fasciotomy was performed on half of the animals. After 42 days, the testicles were examined both macroscopically and microscopically. A non-parametric Mann- Whitney U- test was used for statistical analysis. The results were considered significant at p≤0.05. RESULTS: Testicular weight; volume and degree of histological alterations were evaluated. When the g…

Percutaneous needle fasciotomy in Dupuytren's contracture: is it a viable technique?

The aim of this retrospective study was to evaluate the results of 44 percutaneous needle fasciotomies for Dupuytren's contracture, performe … dure or would be willing to repeat it if necessary. The recurrence rate was 9%. Percutaneous needle fasciotomy appeared in this study as a minimally invasive, simple and fast technique with low morbidity. These features make this technique a valid alternative in mild stages of Dupuytren's disease.

Compartment syndrome and lower-limb fasciotomies in the combat environment.

Prophylactic and therapeutic treatment of leg compartment syndrome with decompression by double-incision fasciotomy prevents progression of soft-tissue injury in high-energy trauma. This treatment is the standard of care in civilian trauma and combat settings. More controversial is the use of either single- or dual-incision fasciotomy of the foot for prophylactic treatment of foot compartment syndrome. Fasciotomy must be performed in the face of major trauma to the foot with severe swelling and unremitting pain. The role for prophylactic fasciotomy of the foot is unclear and should be consider…

Snakebite in a child: could we avoid the anaphylaxis or the fasciotomies?

INTRODUCTION: We report the unusual presentation and complications of a case of snakebite, involving a child and his mother, who were bitten by the same snake. CLINICAL PICTURE: The mother was well but the child developed worsening local pain and swelling. TREATMENT AND OUTCOME: Test doses of antivenom were administered but he developed anaphylactic shock. Subsequently, he developed compartment syndrome and fasciotomies were performed. CONCLUSIONS: This report serves to alert the public to a few of the avoidable risk factors associated with snakebites (such as avoiding areas known to harbou

The vessel loop shoelace technique for closure of fasciotomy wounds.

Compartment syndrome of the extremity may occur after severe trauma secondary to fractures, vascular ischemia, crush, or electrical injury. Treatment consists of expedient fasciotomy to avoid permanent injury to muscles or nerves. Management of the wounds postoperatively has consisted traditionally of primary closure, healing by secondary intention, or split-thickness skin grafting to cover defects. The fasciotomy wound may remain substantial secondary to soft-tissue swelling and edema. The authors present an alternative protocol for fasciotomy wound management, consisting of gradual closure w…

Fasciotomy in vascular trauma: is it too much, too often?

Fasciotomy has been used as a prophylactic measure against development of compartment syndrome and as a treatment modality when the syndrome has developed in patients suffering vascular trauma. The hospital records of 36 patients who underwent surgical repair of their traumatic vascular injuries were reviewed. All 36 patients had at least one indication for fasciotomy at the time of repair; i.e., ischemic time of more than 6 hours or combined arterial and venous injury. Prophylactic fasciotomies were performed in 18 of the patients at the time of vascular repair; 18 did not have fasciotomies p…

Fasciotomy following total knee arthroplasty: beware of terrible outcome.

There is a paucity of literature on compartment pressure thresholds and complications following fasciotomy in postoperative Total Knee Arthroplasty (TKA) patients. The purpose of this study is to identify the postoperative course in a series of patients who had surgical decompression following TKA. We identified six patients who underwent fasciotomy for proven or suspected compartment syndrome following primary TKA between 2000 and 2010. The etiology of compartment syndrome was indirect (5 cases) vascular injury and one unknown. Complications after fasciotomy were: periprosthetic infection (2)…

Early fasciotomy for acute clinically evident posttraumatic compartment syndrome.

Our understanding of the effectiveness of early decompressive fasciotomy for acute posttraumatic compartment syndrome is incomplete. Thirty-two patients who developed acute clinically evident compartment syndrome (23 in the leg, 9 in the forearm) were treated with decompressive fasciotomy an average of 16 hours after injury. Thirty patients (94 percent) underwent fasciotomy in conjunction with other urgent operative procedures mandated by concomitant injuries. Three patients required early amputation for a failed arterial repair. Only 2 of 29 patients with limb salvage (7 percent) had postoper…

[Treatment of Dupuytren's disease; limited fasciectomy versus percutaneous needle fasciotomy].

OBJECTIVE: We estimated retreatment risk following limited fasciectomy (LF) or percutaneous needle fasciotomy (PNF) for Dupuytren's disease. DESIGN: Prospective cohort study. METHOD: Using frailty models, we estimated retreatment risk within 1,3,5,10 and 20 years. RESULTS: LF showed a 10-year retreatment risk of 8% (95%CI: 3-13%) for men and 4% (95%CI: 1-8%) for women without an affected first-degree relative, initially treated at 60 years. PNF showed higher risks at 69% (95%CI: 52-86%) and 46% (95%CI: 16-77%). Younger age at initial treatment increased retreatment risk, sex and first-degre…

[The Extended Percutaneous Needle Fasciotomy Technique (PNF+) For Dupuytren's Contracture].

The extended percutaneous needle fasciotomy (PNF+) is a minimal invasive technique especially suitable for treating severe Dupuytren's contractures and recurrences. The familiar needle fasciotomy is complemented by a spontaneous tearing of the skin and a full-thickness graft to cover the defect. Skin tears in fingers and palm are not a complication, but an integral part of the PNF+. The neurovascular bundles remain intact. This surgical procedure may be repeated as often as required.

Return to activity following fasciotomy for chronic exertional compartment syndrome.

Diagnosis of chronic exertional compartment syndrome (CECS) is relatively rare but has been well documented in athletes. There are, however, few reports regarding return to athletic activity after surgery among elite-level athletes. We hypothesized that a majority of elite-level athletes would successfully return to their previous level of competition following fasciotomy for CECS. A retrospective chart review was performed to identify elite-level athletes (collegiate or professional sport participation) who underwent fasciotomy for CECS over a 3-year period. Data collected included sport or a…

Uniportal endoscopic plantar fasciotomy: a prospective study on athletic patients.

BACKGROUND: Endoscopic techniques have been described to surgically treat plantar fasciitis in the general patient population. A prospective, single-surgeon study was conducted to evaluate if a uniportal endoscopic plantar fasciotomy technique is effective in allowing athletic patients to return to their desired activities. METHODS: Sixteen patients with plantar fasciitis who were athletically active were compared to a control group that consisted of 10 patients who did not engage in sports. Most of the athletic patients were runners. Mean age at time of surgery was 39.8 +/- 13.1 years for th…

A systematic review of fasciotomy in chronic exertional compartment syndrome.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is an overuse injury typically seen in young and athletic patients. The five card … patients who fail to respond to conservative management or where exercise reduction is unrealistic, fasciotomy can be considered. There are no established guidelines on the management of CECS, and it remains underdiagnosed. The aim of this systematic review was to compare the outcomes of patients suffering from CECS managed with either fasciotomy or nonopera …

Endoscopic Fasciotomy of the Superficial and Deep Posterior Compartments of the Leg.

Chronic exertional compartment syndrome is a rare cause of lower leg pain incurred during sports activities and typically affects young athletes who need to return to their activity level as quickly as possible. Nonoperative treatments are often unsuccessful and fasciotomy of the involved compartment is the treatment of choice. Endoscopically assisted release of the anterior and deep compartments is proven to be safe and effective. Endoscopically assisted deep posterior compartment release via an incision 1 to 3 cm behind the medial tibial border has high risk of injury to the great saphenous …
