Theory is often downplayed or omitted in the research and scholarly literature around public health interventions in carceral settings. The author's experience dates from 1990, during the period when he was in charge of the feminization of the facial skeleton in order to improve the social integration of male transsexual patients. Very quickly, these techniques were extended to genetic women who wanted a more feminine face. This facial skeletal surgery was then supplemented with soft tissue surgery. This skeletal and volumetric surgery has become an important contribution in the search of facial rejuvenation and beauty.

Chin reshaping can provide a more identifiable female appearance for transitioning male to female patients undergoing facial feminization. The "sliding" genioplasty has the most potential for dramatically reshaping the chin, while also avoiding many of the issues that may occur with implants. A chin should be evaluated radiologically and by physical examination to determine what changes should be made to any particular chin. When performing osseous genioplasty, the mental nerve can be protected by performing any osteotomies at least 6 mm below the inferior border of the mental nerve canal.

Primary liver cancer occasionally presents with feminization. The mechanism is unknown. We studied a young man with primary liver cancer associated with feminization that disappeared after removal of the tumor. The ultrastructural findings on the gonad from a patient with testicular feminization syndrome (TFS) are reported. One gonad was the site of dysgerminoma. The other was a dysgenetic testis composed of seminiferoud and Sertoli cell tubules and ovarian cortical stroma-like tissue. Ultrastructurally the tubules were lined by immature Sertoli cells and cells indistinguishable from dark ovarian stromal cells. Similar and/or identical cells were found in the ovarian-like stroma and in the interstitial tissue between the tubules. There were mature and immature Leydig cells in the intertubular areas.
