Intrauterine fetal volvulus presenting as fetal distress on cardiotocographic monitoring: a case report.

A rare case of intrauterine fetal midgut volvulus is described. The condition was diagnosed post delivery after the patient presented with markedly decreased fetal movements and fetal distress on cardiotocographic monitoring at 35 weeks' gestation. This baby was delivered and resuscitated before proceeding to laparotomy and bowel resection. The infant has subsequently made a good recovery. Intrauterine volvulus is rare, and presentation in this case was with fetal distress which has not previously been described as a presenting feature.

Electronic fetal heart rate monitoring during cesarean section in cases of fetal distress.

Twelve fetuses in whom the diagnosis of fetal distress was made during first stage of labor were included in the study. All 12 patients underwent urgent cesarean section during which fetal heart rate (FHR) was recorded continuously. In 50% of the cases, marked improvement of FHR was registered early in the course of the operation. Improvement in fetal status is attributed to adequate oxygenation and cessation of uterine activity.

Fetal heart rate response to strenuous maternal exercise: not a predictor of fetal distress.

OBJECTIVE: The purpose of this study was to analyze the effects of maternal exercise on fetal heart rate changes to determine prognostic fac … th the level of fitness, maternal body mass index, or fetal weight. Male fetuses were more prone to fetal distress. A significantly higher proportion of distressed babies were born to older women (P <.0001). The percentage of abnormal traces was equal in the vaginal and caesarean delivery groups. CONCLUSION: The results showed no correlation among the incidence …

Electronic fetal heart rate monitoring: early neonatal outcomes associated with normal rate, fetal stress, and fetal distress.

OBJECTIVE: The purpose of this study was to test the ability of a clearly defined classification system for electronic fetal heart rate monitoring to predict early neonatal outcome. STUDY DESIGN: All labors of women with singleton pregnancies > or = 32 weeks' gestation electronically monitored at 2 institutions were examined. Tracings in the final hour before delivery were defined as normal, fetal stress, or fetal distress. After delivery, Apgar scores, cord blood gas values, and admission to the neonatal intensive care unit were examined as measures of early neonatal outcome. RESULTS: Among…

Fetal Heart Rate Extraction Based on Wavelet Transform to Prevent Fetal Distress In Utero.

Источник пометил публикацию как отозванную

In order to improve the effective extraction of fetal heart rate and prevent fetal distress in utero, a study of fetal heart rate feature extraction based on wavelet transform to prevent fetal distress in utero was proposed. This paper adopts a fetal heart rate detection method based on the maximum value of the binary wavelet transform modulus. The method is simulated by the Doppler fetal heart signal obtained from the clinic. Compared with the original curve, the transformed curve can roughly see the change rule of the original signal and identify the peak point of the signal, but due to the …

Umbilical Artery Thrombosis Causing Fetal Distress: A Case Report.

The umbilical cord (UC) is vital to maintain blood circulation between the mother and the growing fetus, which is sometimes disrupted. The u … had an uneventful course of pregnancy except for the premature rupture of membranes and continuous fetal distress in the second stage of labor. As the labor progression was optimal, and prioritizing the patient's desire, she was vigilantly observed under the premise of continuous electronic fetal monitoring (EFM) to facilitate any emergency, ultimately resultin …

Operative versus conservative management for 'fetal distress' in labour.

BACKGROUND: Suspected fetal distress usually results in expedited delivery of a baby (often operatively). The potential harm to a mother and baby from operative delivery may not always be justified especially when fetal distress may be misdiagnosed. Even with a correct diagnosis it is not clear whether an operative or conservative approach is better. OBJECTIVES: The objective of this review was to assess the effects of operative management for fetal distress on maternal and perinatal morbidity. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (15 Febr…

Towards Noninvasive Accurate Detection of Intrapartum Fetal Hypoxic Distress.

Current intrapartum fetal well-being assessment is performed using electronic fetal monitoring (EFM), technically referred to as cardiotocography (CTG), which transabdominally monitors fetal heart rate (FHR) in relationship to maternal uterine contractions. Sometimes the deceleration in FHR following a uterine contraction can be sign of fetal hypoxic distress, but it may also be a normal physiological response. Multiple studies have shown that EFM has a high false positive rate for detecting fetal hypoxia. This has caused a rise in emergency Cesarean section (C-section) deliveries performed in

Transient hypertrophic cardiomyopathy in neonates after acute fetal distress.

We report three cases of transient myocardial hypertrophy, diagnosed by echocardiography, occurring between the second and seventh days of life in neonates with initially normal ventricular myocardial wall thickness. The three term neonates had perinatal injury with acute fetal distress. In all three cases electrocardiographic and biologic signs of myocardial ischemia were present. The first echocardiographic results showed abnormalities in systolic or diastolic left ventricular function, without hypertrophy of the walls. The hypertrophic cardiomyopathy (HCM) occurred between days 2 and 7 and …

Fetal respiratory distress causing CNS and inner ear hemorrhage.

Fifty-two consecutive temporal bones of infants who died neonatally, or in utero of natural causes, were studied. Complete autopsies were performed. Twenty-eight infants had a variety of pulmonary disorders which resulted in severe respiratory distress prior to their death. Of these, the majority had bleeding intracranially and into the inner ear. There were five major pathways of central nervous system and subarachnoid hemorrhage involvement of the inner ear: 1) the modiolus, 2) cochlear aqueduct, 3) retrograde via the cochlear vein, 4) episodes of spontaneous bleeding into various compartmen

Tocolysis for preventing fetal distress in second stage of labour.

BACKGROUND: Prophylactic tocolysis with betamimetics and other agents has become widespread as a treatment for fetal distress. Uterine relaxation may improve placental blood flow and therefore fetal oxygenation. However there may also be adverse maternal cardiovascular effects. OBJECTIVES: The objective of this review was to assess the effects of prophylactic betamimetic therapy during the second stage of labour on perinatal outcome. SEARCH STRATEGY: The Cochrane Pregnancy and Childbirth Group trials register and the Cochrane Controlled Trials Register were searched. Date of last search: Feb…

[Studies on catecholamine and its metabolites in the evaluation of fetal distress].

To determine the usefulness of catecholamine (CA) levels in the evaluation of fetal distress during delivery, norepinephrine (NE), its intermediate metabolite 3,4-dihydroxyphenylethyleneglycol (DOPEG), and 3,4-dihydroxyphenylacetic acid (DOPAC), which is the main intermediate metabolite of dopamine (DA), were simultaneously measured in umbilical arterial and venous blood (UA, UV) and in maternal venous blood (MV). The correlations of these concentrations with blood pH and the 1-min. Apgar score were examined. Of the 124 subjects studied, 24 had elective cesarean section (CS), and 100 had vagin…

Fetal Distress as a Determinant for Refeeding Syndrome in Preterm Neonates.

Background/Objectives: Preterm neonates receiving parenteral nutrition (PN) are at risk of developing refeeding syndrome (RS). Risk factors … lled, consisting of 53 cases and 359 controls. The main prenatal risk factor for RS was found to be fetal distress (p = 0.028). The occurrence of RS was identified as statistically significantly associated (p = 0.010; p = 0.007) with the development of extrauterine growth restriction (EUGR) and retinopathy of prematurity (ROP). Conclusions: Fetal distress is the …

Low placental weight and risk for fetal distress at birth.

UNLABELLED: Abstract Objective: The objective of this study was to assess whether low placental weight is associated with higher risk of emergency delivery (cesarean section and vacuum-assisted delivery). METHODS: This was a retrospective cohort study, including 657 normally developed term pregnancies. A strict methodology was used to provide reliable and reproducible placental weight measurement. Fetal heart rate patterns were analyzed according to the guidelines of the Royal College of Obstetricians and Gynecologists. Linear and logistic multiple regression analyses were used to assess risk
