Lateral short-term prism adaptation in clinical evaluation.

Data were obtained from a normal clinical population regarding heterophoria, associated phoria and short-term prism adaptation in an attempt to differentiate between those subjects with symptoms and those without. Correlation between heterophoria, associated phoria, prism adaptation and symptoms was low. A short prism-adaptation test was found to be of little value in diagnosing decompensated heterophoria in the general population. Subjects diagnosed as having decompensated heterophoria did not have an abnormal short-term prism-adaptation system. The clinical implications of these findings are

Adaptation to prism-induced heterophoria in subjects with abnormal binocular vision or asthenopia.

In a previous study we investigated how normal subjects adapted to prism-induced heterophoria. In this study we have continued this work by investigating this phenomena in 15 patients with abnormal binocular vision and/or asthenopia. It was found that the majority of subjects lacked or had a deficient adaptation system to base-in and/or base-out prisms, but all except two subjects examined adapted normally to vertical prism. These results suggest that it may be possible to treat the horizontal adaptation system independently from the vertical one. Prism vergences, heterophoria measurements, an

Presbyopia and ocular motor balance.

Presbyopic case analysis generally involves determining the need for base-in prism. Common methods for prescribing prism, and components of convergence and the paradoxical lack of symptoms at near in presbyopic patients, and clinical measurement of fixation disparity are discussed in this context. Prism and vision therapy are treatment choices.

Ocular torsion and primary retinal meridians.

I measured ocular torsion during and after head- and body-tilt, ocular convergence, and conjugate gaze. Extensive examination on 15 normal human subjects and additional observations on 400 consecutive patients without extraocular motor defects failed to show static ocular tension. During dynamic head-tilt, intermittent, small-amplitude, saccadic, wheel-like torsional eye movements were recorded as moving faster than the head so that the eyes preceded the head. Between these intermittent saccades, and at the end of the head-tilt, there was no residual ocular torsion. The absence of static ocula

Congenital ocular motor apraxia without head thrusts.

Congenital ocular motor apraxia is a distinctive disorder that may present initially as apparent cortical blindness, because of the patient's inability to make saccadic eye movements to fixate targets moving in the horizontal plane. Affected children develop a characteristic compensatory head thrust that allows them to fixate and allows the physician to make the diagnosis. Two patients, initially thought to be blind because of a lack of fix or following responses in the horizontal plane, are reported. Although head thrusts were not present because of inadequate head control, the diagnosis of c

Decreased uncorrected vision after a period of distance fixation with spectacle wear.

Myopes of low degree commonly report that their vision seems poorer upon removal of their spectacles compared to that after a period without spectacle wear. Notably, this difference in vision can be appreciated after distance fixation. In this paper, we propose and test several alternative hypotheses to explain the phenomenon: an accommodative response to spectacles, sensory adaptation, or altered criteria for blur of psychological origin. We measured visual acuity (VA), refractive error, and lens thickness on 10 subjects with less than 2.00 D of myopia. Testing was performed after two 90-min

[Analysis of energy levels for Nd:YAG laser capsulotomy in secondary cataract].

BACKGROUND: Formation of secondary cataract is influenced by various factors, such as IOL material, IOL design, age, follow-up time and ocular and systemic diseases. It has not yet been studied whether these factors have an clinical impact on the energy used for Nd:YAG laser capsulotomy. PATIENTS AND METHODS: We examined 172 patients, aged 67.3+/-15.9 years, concerning energy levels required for Nd:YAG laser capsulotomy. We analysed the influence of age, implant duration, IOL fixation and ocular conditions on total energy and repetition rate of Nd:YAG laser capsulotomy. Sixty-nine patients (4

Keeping the world in focus: how might this be achieved?

To see clearly, the eye needs to have a sharply focused image on its retina. For distance vision, the eye should ideally develop toward emmetropia, and for near vision, the accommodation system should produce an appropriate increase in ocular power. The characteristics of the emmetropization and accommodation mechanisms are compared. Following Howland (paper presented at the 13th International Myopia Conference, July 26-29, 2010, Tubingen, Germany), it is suggested that emmetropization may be guided by imagery in the peripheral retina, perhaps making use of oblique astigmatism; however, such a

Adaptive neural mechanism for Listing's law revealed in patients with sixth nerve palsy.

PURPOSE: During fixation and saccades, human eye movements obey Listing's law, which specifies the torsional eye position for each combination of horizontal and vertical eye positions. To study the mechanisms that implement Listing's law, the authors measured whether the law was violated in peripheral and central unilateral sixth nerve palsy. METHODS: Twenty patients with peripheral (13 chronic, 7 acute) sixth nerve palsy, 7 patients with central sixth nerve palsy caused by brainstem lesions, and 10 normal subjects were studied with scleral search coils. With the head immobile, subjects made

Accommodative and fusional components of fixation disparity.

Traditional measurements of fixation disparity, like other binocular measurements, confound influences from both blur-driven and disparity-driven components. Measuring fixation disparity with accommodation open-loop eliminates accommodative interactions, and results show fixation disparity is reduced to about half the value observed normally for high levels of forced vergence. Thus fixation disparity curves are shaped by both accommodative and fusional vergence processes.

Exophoria at near in presbyopia.

The exophoria at near working distances through a plus lens addition which exists in presbyopia was investigated in order to explain the paradoxical lack of asthenopic symptoms associated with this condition. Fixation disparity measurements indicate that the exophoria which is measured by the von Graefe technique does not exist under binocular conditions. Further analysis indicates that presbyopes may have unrestrained use of accommodative convergence.

Instrument for measuring the misalignments of ocular surfaces.

A compact and robust instrument for measuring the alignment of ocular surfaces has been designed and used in living eyes. It is based on recording Purkinje images (reflections of light at the ocular surfaces) at nine different angular fixations. A complete analysis on the causes of misalignments of Purkinje images and its relations with those physical variables to be measured (global eye tilt, lens decentration and lens tilt) is presented. A research prototype based on these ideas was built and tested in normal and pseudophakic eyes (after cataract surgery). The new analysis techniques, togeth

[Comments on Cogan's syndrome (congenital oculomotor apraxia) based on a case].

CASE: A 4-month-old girl was seen by a paediatrician for psychomotor delay and defective visual fixation. In order to shift gaze direction, she had to jerk her head in order to drag her eyes in the direction she wanted to look. She also showed non-specific pyramidal impairment. Herpes human virus type 6 (HHV-6) was amplified by PCR. No other findings could explain her troubles with fixation. She was diagnosed with congenital ocular motor apraxia (C.O.M.A.). DISCUSSION: When C.O.M.A. is suspected, other causes of defective fixation and abnormal cephalic movements must be ruled out through the
