The major disorder of water and electrolyte metabolism in children is dehydrating diarrhea. The major advance in the treatment of this condition has been the development of oral rehydration therapy, ie, the enteral administration of a balanced glucose-electrolyte solution. This therapy is effective in patients of all ages, dehydration of all degrees short of hypovolemic shock, with gastroenteritis of all causes, and electrolyte disturbances including hypo- and hypernatremia. This review highlights current experimental and clinical studies that have focused on oral rehydration solutions that ha

Patients were categorized into two groups based on the type of fluid received: 300 patients received balanced crystalloids and 300 received isotonic saline. Outcomes of interest included the incidence of AKI, t …

BACKGROUND: Over the past 16 years, sepsis management has been guided by large-volume fluid administration to achieve certain hemodynamic optimization as advocated in the Rivers protocol. However, the safety of such practice has been questioned because large-volume fluid administration is associated with fluid overload and carries the worst outcome in patients with sepsis. Researchers in multiple studies have declared that using less fluid leads to increased survival, but they did not describe how to administer fluids in a timely and appropriate manner.

Perioperative patients can be highly dynamic and have various metabolic, physiologic, and organ system derangements that necessitate smart monitoring strategies and careful fluid therapy. The interplay between changing patient status, therapeutic interventions, and patient response makes effective monitoring crucial to successful treatment. Monitoring the perioperative patient and an approach to fluid therapy are discussed in this text.

Fluid management remains a cornerstone of early septic shock resuscitation; an accumulating body of evidence highlights the substantial risks associated with excessive fluid administration. Fluid overload is consistently associated to organ dysfunction and adverse outcomes, underscoring the need for a personalized approach to intravenous fluid therapy. Over the past decade, research has progressively refined the concepts of fluid responsiveness and the fluid challenge, moving from fixed-volume boluses guided by static measures to individualized, dynamic assessments supported by multimodal moni…

The primary purpose of perioperative IV fluid administration is to preserve tissue perfusion by maintaining or restoring the effective circulating intravascular volume. Fluids are drugs that produce beneficial or harmful effects dependent upon their composition, osmotic potential, kinetics, and dose. Appropriate dosing requires an understanding of body fluid compartments, fluid balance, and the administered fluids' behavior in the body. Anesthetic drugs and general anesthesia produce CNS, neuroendocrine, and macro-/microvascular hemodynamic effects. These effects modulate the response to IV fl

Emerging evidence from observational studies suggests that both slower and faster net ultrafiltration rates during kidney replacement therapy are associated with increased mortality in critically ill patients with acute kidney injury and fluid overload. Faster rates are associated with ischemic organ injury. The net ultrafiltration rate should be prescribed based on patient body weight in milliliters per kilogram per hour, with close monitoring of patient hemodynamics and fluid balance. Randomized trials are required to examine whether moderate net ultrafiltration rates compared with slower an

The objective of this consensus-based S1 Guideline for perioperative fluid therapy in children is to maintain or re-establish the child's homeostasis. Therefore, the perioperative fasting times should be as short as possible to prevent patient discomfort, dehydration, and ketoacidosis. For the intraoperative background infusion a physiologically composed balanced isotonic electrolyte solution (BS) with 1 - 2.5% glucose is recommended to maintain normal glucose concentrations and to avoid hyponatremia, hyperchloremia, and lipolysis. Additional BS without glucose can be used in patients with cir…

Cerebral autoregulation and the blood-brain barrier are two important mechanisms that attempt to preserve brain homeostasis. The function of either may be disrupted by injury. When autoregulation is impaired, blood pressure and hematocrit determine cerebral oxygen delivery. Injury to the blood-brain barrier impairs brain volume regulation and may contribute to cerebral edema. The choice of intravenous fluid influences cerebral blood flow, cerebral oxygen delivery, brain metabolism, and brain volume.

Enteral fluids administered alone, or in conjunction with intravenous fluids, are reported to be useful for the treatment of dehydration and electrolyte loss associated with diarrhoea in a number of species, following exercise in horses and for feed impaction of the large intestine of horses. Enteral fluids are suitable for treatment of mild to moderately dehydrated patients with some intact intestinal epithelium and motile small intestine. In patients that will drink voluntarily or tolerate nasal intubation the use of enteral fluids may avoid the complications associated with intravenous flui

Virtually all hospitalized pediatric patients require some form of intravenous fluid administration. The foundation of current pediatric fluid therapy practice was formulated in the 1950s when pediatricians were dealing with relatively simple dehydration and normal homeostasis could largely be assumed. Recent advances in pediatric medicine have resulted in increased severity of illness and normal physiology can no longer be assumed. The traditional approach to pediatric fluid therapy has been recently challenged by the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), cerebr…
