[Costa's acrokerato-elastoidosis. Ultrastructural study (author's transl)].

Acrokerato-elastoidosis described by Costa is characterized by numerous little keratotic points and translucent horny wheals on the dorsal aspect of hands and (or) feets. The ultrastructural study shows lesions of the elastic tissue which is very rarefied with an abnormal structure, but also demonstrates alterations of the fibroblasts that contain dense granules at the periphery of their cytoplasm without elastic extracellular fibers, suggesting a trouble in the secretion or the excretion of the elastic material.

A painful thorn in the foot: a case of eumycetoma.

Madura foot or mycetoma is endemic in many developing countries. It is occasionally seen within the United States due to increasing international travel but it may sometimes be acquired within US soil. Herein, we present a case of a patient with a diagnosis of mycetoma acquired through trauma to the foot. In addition, we discuss the epidemiology, etiological agents, clinical presentation, diagnosis, and treatment of mycetomas. Clinicians need to recognize mycetoma early and institute treatment promptly to reduce the substantial morbidity associated with this devastating infection.

Soft-tissue causes of metatarsalgia.

A soft-tissue cause should always be considered when evaluating metatarsalgia. The history, examination, and plain radiographs can all suggest an avenue of pursuit, be it a potential dermatologic, connective tissue, vascular, neurologic, muscular, or neoplastic process.

Acral acanthotic anomaly: a comprehensive review.

Acral acanthotic anomaly (AAA), also referred to as acral acanthosis nigricans (AAN), is a variant of acanthosis nigricans characterized by hyperpigmented, hyperkeratotic plaques localized to acral surfaces, particularly the knuckles, toes, and dorsal aspects of the hands and feet. Unlike other common forms, AAA is not typically associated with obesity, insulin resistance, or endocrinopathy. Its pathogenesis remains poorly understood. The aim of this article is to provide a comprehensive synthesis of the literature on AAA, including its epidemiology, clinical features, histopathologic findings

Finger and toenail onycholysis.

Onycholysis - the separation of the nail plate from the nail bed occurs in fingers and toenails. It is diagnosed by the whitish appearance of the separated nail plate from the nail bed. In fingers, the majority is caused by trauma, manicuring, occupational or self-induced behavior. The most common disease producing fingernail onycholysis is psoriasis and pustular psoriasis. Phototoxic dermatitis, due to drugs can also produce finger onycholysis. Once the separation occurs, the environmental flora sets up temporary colonization in the available space. Finger onycholysis is most common in women.

Treating cosmetically induced nail problems.

Smooth, lustrous nails are a sign of health and beauty in our society and fuel the US$6-billion nail salon industry in the United States. Although many women can use nail cosmetics without adverse consequences, when problems occur it is important to recognize the causes and treat the problem condition. The cornerstone of management of nail cosmetic problems is prevention through education. In spite of efforts to that end, nail salon procedures and materials can cause nail disease that must be recognized and treated in order to restore the nails to health.

Leprosy-associated eccrine syringofibroadenoma of Mascaro.

Eccrine syringofibroadenoma of Mascaro is a rare benign tumour. There are five subtypes, one of which is known to occur in reaction to inflammatory and neoplastic dermatoses. We describe a patient with previous lepromatous leprosy presenting with multiple flesh-coloured verrucous plaques over the right foot. Histology was consistent with eccrine syringofibroadenoma. To our knowledge, this is the first reported case of multiple eccrine syringofibroadenomata occurring in association with leprosy. The tumours in this case are most likely reactive in nature, subsequent to multiple traumatic events

Onychomadesis with Lichen Planus: An Under-Recognized Manifestation.

Onychomadesis or proximal separation of the nail pate usually results from severe, generalized dermatoses like bullous diseases, hand foot and mouth disease, varicella or severe cutaneous drug reactions. Although lichen planus (LP) produces varied nail manifestations (longitudinal onychorrhexis, onychoschizia, nail pigmentation, pterygium etc.), to the best of our knowledge, onychomadesis as a manifestation of nail LP is not recorded. This report presents two children with onychomadesis arising with generalized eruptive LP.
