IgG4-related pachymeningitis masquerading as foramen magnum meningioma: illustrative case.

BACKGROUND: Immunoglobulin G4-related disease (IgG4-RD) is an immune-mediated inflammatory condition with potential multiorgan involvement. … nvolving the occipital bone. Magnetic resonance imaging revealed a dural-based lesion involving the foramen magnum and invading the left hypoglossal canal. The patient underwent a far-lateral craniotomy. Histopathological analysis revealed severe lymphoplasmacytic inflammation, storiform fibrosis and rich plasma-cell population positive for IgG4. Serum IgG4 was …

Complete Resection of an Intradural Extramedullary Foramen Magnum Cavernous Malformation.

Only few case reports of intradural extramedullary-located cavernous angiomas are available. This report describes an extra-axial hemorrhagic lesion located on the lateral surface of the medulla oblongata at the foramen magnum. Follow-up magnetic resonance imaing clearly demonstrated growth of the lesion, probably due to repetitive intralesional hemorrhaging. Initially, a right vertebral artery aneurysm or an arteriovenous malformation was suspected. Digital subtraction angiography excluded these differential diagnoses, although doubt remained concerning the possibility of a thrombosed aneurys…

Foramen magnum compression in Coffin-Lowry syndrome: A case report.

Coffin-Lowry syndrome (CLS) is a rare genetic disorder inherited in an X-linked dominant pattern. Common manifestations include intellectual disability, growth retardation, dysmorphic facial features, and variable skeletal anomalies. Here we report a patient who first presented with episodes of apparent life-threatening events (ALTE) found to be caused by hydrocephalus and brainstem compression at the foramen magnum. Together with his small size, short limbs and fingers, and facial appearance, the narrowing of the foramen magnum lead to the initial clinical misdiagnosis of hypochondroplasia. S…

The (extended) achondroplasia foramen magnum score has good observer reliability.

BACKGROUND: Achondroplasia is the most common skeletal dysplasia. A significant complication is foramen magnum stenosis. When severe, compression of the spinal cord may result in sleep apnea, sudden respiratory arrest and death. To avoid complications, surgical decompression of the craniocervical junction is offered in at-risk cases. However, practice varies among centres. To standardize magnetic resonance (MR) reporting, the achondroplasia foramen magnum score was recently developed. The reliability of the score has not been assessed. OBJECTIVE: To assess the interobserver reliability of the…

Foramen magnum schwannoma with an unusual clinical presentation: case report.

This case report illustrates a patient with a unilateral localized epidural schwannoma at C1-2 level, showing contralateral spinal cord symptoms and signs. The clinical presentation was compared with the neuroradiological examinations showing an epidural cord tumor at the right C1-C2 level. However, the neurological examination revealed contralateral hemiparesis and ipsilateral hemihypoalgesia, indicating a contralateral Brown-Sequard syndrome. We presumed with this slow growing tumor, contralateral cord involvement may result from counter pressure on the spinal cord, which may be generated by

Infected intradural hydatid cyst at foramen magnum. A case report.

A case of infected intradural extramedullary hydatid cyst at foramen magnum with recurrent spinomedullary compression is described. The diagnosis was based on operation and histopathology. So far, no case of extraparenchymatous multiple cysts with cervicomedullary compression has been described in literature. Infection within an intact cyst is extremely rare.

Sleep apnea in patients with achondroplasia associated with foramen magnum stenosis.

OBJECTIVE: Sleep apnea is frequently associated with foramen magnum stenosis in patients with achondroplasia and considered a cause of sudden death. The aim of this study was to evaluate the results of respiratory polygraphy (RPG) before and after surgery in patients who underwent foramen magnum decompression (FMD) to elucidate the effectiveness for treatment of sleep apnea. METHODS: The findings of RPG, MRI, and otolaryngological fibroscopy were retrospectively reviewed in 22 children with achondroplasia who underwent FMD at a single institution from 2016 to 2022. The Mann-Whitney U-test was…

Communicating hydrocephalus after resection of a meningioma ventral to the foramen magnum.

BACKGROUND: Tumors in or near the foramen magnum may cause communicating or non-communicating hydrocephalus (HC), depending on their size and location. Here, an 81-year-old female developed communicating HC following the resection of a meningioma ventral to the foramen magnum. CASE DESCRIPTION: An 81-year-old female presented with numbness in the left neck and left hemiparesis. The magnetic resonance revealed an 18-mm tumor ventral to the foramen magnum that significantly enlarged over the past 6 months. She underwent total tumor resection but then presented with progressive HC both clinicall…

Suitability of foramen magnum measurements in sex determination and their clinical significance.

BACKGROUND: The foramen magnum provides a transition between fossa cranii posterior and canalis vertebralis. Medulla oblongata, arteria vertebralis and nervus accessorius spinal part pass through the foramen magnum. In this study, we aimed to make the morphometric measurements of the foramen magnum on computed tomography (CT) and to determine the feasibility of sex determination based on these measurements. Besides sex determination, from a clinical aspect, it is important to know the measurements of the foramen magnum in the normal population in terms of diseases characterised by displacement…

Foramen magnum decompression for Chiari I malformation: a procedure not to be underestimated.

OBJECTIVE: Chiari I malformation may be treated with foramen magnum decompression (FMD). We aim to describe the symptoms with which patients initially present, and to determine the number and type of complications occurring after FMD for Chiari I malformation. METHODS: Retrospective review of medical records for patients who had FMD performed for Chiari I malformation between January 2009 and December 2011. Post-operative outcomes were recorded and analysed. Patient demographic details and other relevant medical conditions were also noted. RESULTS: Between January 2009 and December 2011, 54 …

Anatomical variations of the foramen magnum, occipital condyle and jugular tubercle.

AIM: The foramen magnum (FM) is a unique and complex anatomical region. The occipital condyle (OC) and jugular tubercle (JT) are the main bony structures which obscure the anterolaterally situated lesions of the FM.The aim of this study was to revisit the anatomy of the FM region and assess variations of the surrounding structures. MATERIAL AND METHODS: Observations, on thirty dry skulls (dried specimens, 60 sides) and ten formalin-fixed cadaveric heads with perfused vessels, were carried out to define the microsurgical anatomy of the FM region. Morphometric analysis and variations of the FM,…

Approaches to anterior and anterolateral foramen magnum lesions: A critical review.

Foramen magnum (FM) lesions represent some of the most complex cases for the modern neurosurgeon because of their location near vital brainstem structures, the vertebral arteries, and lower cranial nerves. In particular, anterior or anterolaterally located FM tumors have traditionally been most difficult to resect with high morbidity and mortality resulting from approaches through the posterior midline or transorally. For many neurosurgeons, the far lateral, extreme lateral approach, and more recently, endoscopic endonasal approaches have become the preferred modern methods for the resection o…

Motion of the cerebellar tonsils in the foramen magnum during the cardiac cycle.

Tonsil motion was measured with cine MR imaging in patients clinically suspected of having Chiari I malformation. Cardiac-gated sagittal 2D fast imaging employing steady-state acquisition (2D FIESTA) was performed in 11 patients with a Chiari I malformation and in 6 subjects without tonsil ectopia. Tonsil motion was measured through the cardiac cycle with a pixel-shift program designed for that purpose. Tonsil displacements in patients with Chiari I and those with normal cerebellar tonsils were compared. In patients with Chiari I and subjects with normal cerebellar tonsils, 2D FIESTA showed sm

Choroid plexus papilloma of cerebellopontine angle with extension to foramen magnum.

A case of choroid plexus papilloma resembling meningioma of cerebellopontine (CP) angle with its extension to foramen magnum is presented. Occurrence of this tumour in CP angle is very rare. Its extension towards foramen magnum is further rare. It was a real diagnostic enigma preoperatively as the tumour was resembling meningioma upto some extent on radiological study. Retromastoid craniectomy with microsurgical excision of tumour and its extension was achieved in toto. Tumour was attached to few rootlets of lower cranial nerves which were preserved. Attachment of the tumour with lower cranial…

Foramen magnum decompression in an infant with homozygous achondroplasia. Case report.

Homozygous achondroplasia is a rare yet distinct clinical entity. Most infants succumb to an early death as a result of respiratory compromise due to upper airway obstruction, thoracic cage deformity, and/or cervicomedullary compression. The successful cervicomedullary decompression of a 16-week-old infant with homozygous achondroplasia is described. This report suggests that homozygous achondroplasia is not universally fatal and that these infants are potentially viable if managed by aggressive respiratory and surgical measures.

Accessory Nerve Meningioma of the Foramen Magnum: A Rare Neurosurgical Entity.

Meningiomas, which are the most common primary intracranial tumors, frequently arise from the dura of the convexities, parasagittal regions, and skull base. The accessory nerve (cranial nerve XI) at the level of the foramen magnum is an exceptionally uncommon site of origin. To date, only a few cases have been clearly documented in the literature. We report the case of a 66-year-old woman who presented with dizziness, headaches, and left-sided shoulder weakness. The lesion was initially presumed to be a regular foramen magnum meningioma. Intraoperatively, the spinal accessory nerve was central…

Transoral Penetrating Foreign Body Into the Foramen Magnum: A Case Report.

In this paper, we present the case of a four-year-old boy with a penetrating transoral injury caused by a foreign object directed toward the foramen magnum. Head angiotomography revealed that the object's pathway was in close contact with the vertebral artery, without apparent involvement of the meninges. We discuss the clinical presentation, diagnostic approach, and treatment in this case.

Sleep-Disordered Breathing in an Infant With Achondroplasia and Foramen Magnum Stenosis.

Sleep-disordered breathing (SDB) is a frequently recognized comorbidity in infants and children with achondroplasia due to alterations in craniofacial and upper airway anatomy. Foramen magnum stenosis and cervicomedullary compression can be associated with SDB in this population, requiring prompt evaluation by multidisciplinary teams. Untreated SDB is associated with adverse cardiovascular, metabolic, and behavioral effects in children, necessitating early screening and treatment of underlying causes. Cervicomedullary compression is also associated with increased mortality and sudden infant de…
